L'apport vasculaire limité et la pression constante des tissus mous environnants entrainent des résorptions importantes et un risque d'exposition des greffons. Plusieurs techniques sont rapportées dans la littérature afin d'obtenir un volume osseux vertical satisfaisant. Elles nécessitent toutes un deuxième temps chirurgical de pose d'implants après un délai de cicatrisation de 4 à 6 mois. La technique « bone ring » permet de réaliser une greffe osseuse verticale et la pose d'implants dentaires en 1 temps, réduisant la durée de traitement. Matériel et Méthode : Combiner la technique « bone ring » intra-sinusienne par voie de Le Fort à une ostéotomie maxillaire d'avancée a rendu possible la correction d'une classe 3 squelettique associée à une atrophie osseuse verticale post-extractionnelle en position molaire en une chirurgie. L'adaptation du protocole chirurgical classique a permis de répondre à une problématique clinique spécifique. Résultats : La restauration du volume osseux et la pose de l'implant dentaire a pu être réalisée dans le même temps que la chirurgie orthognatique. Aucune complication ni résorption osseuse péri implantaire n'a été observée à 6 mois post opératoire. Discussion : L'analyse des avantages et inconvénients des différentes chirurgies pré implantaires visant à corriger les atrophies osseuses verticales permet d'identifier les indications de la technique bone ring. Dans certaines situations cliniques, cette technique est une alternative intéressante à la chirurgie pré implantaire classique.
