Bien que d’efficacité démontrée, ils ne sont pas moins responsables de 17 000 hospitalisations et 4 000 décès par an et constituent la première cause médicamenteuse d’hospitalisation. Le mésusage des AVK, en raison de la complexité pratique de leur prescription quotidienne sure et de leurs risques potentiels en serait la cause. Les personnes âgées, fragiles, sont les premières concernées. Il convient donc d’optimiser la prescription des AVK chez ces patients pour obtenir un gain en matière de santé; d' identifier les écarts au bon usage des AVK chez le patient de plus de 75 ans et établir en retour des propositions d’amélioration des pratiques professionnelles et enfin d'identifier des facteurs prédictifs de survenue d’un effet indésirable lié aux AVK. Un référentiel de bon usage des AVK à été réalisé puis traduit en grille d’évaluation. Une enquête des pratiques professionnelles, avec étude rétrospective des dossiers médicaux de 100 patients de plus de 75 ans traités par AVK à ensuite été effectuée pour identifier les facteurs de mésusage des AVK et proposer des plans d’actions correctives. Une analyse statistique univariée et multivariée en régression logistique a permis d’identifier les facteurs prédictifs de survenue d’un effet indésirable. Les facteurs influant sur le bon usage des AVK interviennent à tous les niveaux de la prise en charge. Des mesures clés facilement reproductibles peuvent néanmoins permettre d’en améliorer la prescription et d’en réduire le risque iatrogène. En conclusion, le médecin généraliste peut facilement améliorer sa prescription des AVK, pour réduire leur iatrogénicité et promouvoir leur utilisation, bénéfique en termes de santé.

Ils exposent les patients à deux risques principaux : l'hémorragie ou la thrombose. Pour limiter ces risques, les pharmaciens sont autorisés à proposer des entretiens pharmaceutiques à ces patients. Nous avons souhaité évaluer l'impact de ces entretiens sur le savoir, savoir-faire et savoir être des patients de plus de 75 ans vis-à-vis de leur traitement AVK. Méthode : Cette étude épidémiologique observationnelle a été réalisée sur une cohorte de 30 patients à l'hôpital et de 30 patients à l'officine. Un questionnaire de 20 questions a été posé à nos patients à 3 reprises : En pré-entretien, en post entretien précoce (dans la journée de l'entretien) et en post entretien tardif (1 mois après l'entretien). L'entretien était adapté aux lacunes constatées lors du questionnaire réalisé avant l'entretien. Une analyse des 2 INR précédents et suivants l'entretien a été également réalisé. Résultats : La cohorte en Officine était composée à 53% de patients de plus de 80 ans et d'une majorité de femmes (60%). La cohorte à l'hôpital était composée à 73% de patients âgés de plus de 80 ans et également d'une majorité de femmes (53%). En Officine, les notes obtenues au questionnaire juste après l'entretien étaient meilleures qu'avant l'entretien (p=4,73 E-15). De plus les notes obtenues 1 mois plus tard n'ont pas diminuées (p= 0,81). A l'hôpital, les notes obtenues au questionnaire juste après l'entretien étaient meilleures qu'avant l'entretien (p=1,2 E-12) et les notes obtenues 1 mois plus tard n'ont pas diminuées (p=0,10). Nous n'avons pas constaté l'impact de l'entretien sur l'INR des patients ayant un INR cible entre 2 et 3, que ce soit en Officine (p= 0,25) ou à l'hôpital (p= 0,27). Conclusion : D'après notre étude, à l'hôpital comme à l'officine, l'entretien pharmaceutique a permis une amélioration significative des connaissances des patients vis-à-vis de leur traitement AVK. Cette amélioration est plus marquée en Officine qu'à l'hôpital. Cependant certaines notions restent difficilement assimilées ce qui justifie le fait d'insister sur ces notions lors d'un entretien pharmaceutique ultérieur.

The purpose of the workshop was to bring together interested scientists to discuss the newer developments and the recent advances in the field of phosphate metabolism and the other related minerals. An Organizing Committee made of Shaul G. Massry (USA), Louis V. Avioli (USA), Philippe Bordier (France), Herbert Fleisch (Switzerland), and Eduardo Slatopolsky (USA) was formed. The First Workshop was held in Paris during June 5-6, 1975 and was hosted by Dr. Philippe Bordier. Its proceeding was already published. The Second Workshop took place in Heidelberg during June 28-30, 1976 and was hosted by Dr. Eberhard Ritz. Both of these workshops were extremely successful scientific endeavors, and the need for them was demonstrated by the great interest they generated among the scientific community. The Or ganizing Committee, therefore, decided to continue with the tradi tion to hold additional Workshops annually or every other year.

With this discovery was born a new scientific field, in which oxygen, necessary for aerobi c 1 ife on thi s planet, had to be cons i dered also in terms of its toxicity and stresses. This stimulated the search for knowledge of active oxygen species in biology and medicine. Superoxide and other reactive oxygen species are now implicated in many disease processes. Major advances have been achieved during these past years with respect to free radical generation and mechanisms of free radical action in causing tissue injury. In parallel, the possibil ity of influencing free radical related disease processes by antioxidant treatment was studied in various in vitro and in vivo systems. This was the unique theme of a conference organized in Paris by the Society for Free Radical Research (December 9-10, 1988) which brought together experts from basic sciences and clinicians in order to evaluate the current status of antioxidant therapy. The conference emphasized fundamental processes in antioxidant action. Among the major topics were superoxide dismutase (SOD) and low molecular weight substances with such activity, called SOD mimics. Other antioxidant enzymes were also considered. Antioxidant vitamins, in particular vitamins E and C, other naturally occurri ng antioxidants and vari ous synthet i c antioxidants were included in the presentations as there is now a rapidly developing series of compounds with potentially interesting clinical applications.
