Book excerpt: Accurate measurement of blood pressure (BP) is a major public health concern; it is essential for hypertension screening, as well as for disease management. In the period of 2005-2006, 29 percent of the U.S. adult population was hypertensive or taking medication for hypertension. Blood pressure has been measured with the mercury sphygmomanometer for many years but with new technologies available, alternative blood pressure devices have taken center stage. The rapid pace of the development of automated sphygmomanometers with improving accuracy and reliability combined with increasing affordability has meant that these devices have now replaced the mercury sphygmomanometer in many settings. During the 2007-2008 NHANES cycle, a blood pressure methodology study was successfully completed in order to develop a preliminary understanding of alternative blood pressure devices - namely the OMRON HEM-907 XL. The purpose of the study was to validate the OMRON HEM-907 XL by assessing its accuracy according to industry standards using a convenient sample. Initial findings indicate the OMRON machine passed both the American and British protocols. Because of systematic measurement differences between blood pressure devices, comparison of blood pressure values between different populations or within populations over time is hampered when different blood pressure devices have been used.1 Therefore, one pertinent question still needs to be addressed - how will the national estimate of hypertension prevalence be affected if an automated machine is used to replace the mercury sphygmomanometer? NHANES must be able to compare the measurements taken by the mercury device to those taken by a possible successor device in such a way that trends over time in blood pressure prevalence can be accurately maintained and followed. Furthermore, these results would help to better compare blood pressure values in research studies that used different blood pressure devices.

This book summarizes the state of the art in the whole field of indirect blood pressure monitoring. It is based on two international meetings and on invited papers. We have divided the subject matter into two main areas: 1) Automatic blood pressure devices for discontinuous registration, and 2) Portable, fully automated programmable monitors for continuous monitoring. The availability of all new technologies is described in detail and current technical and physiological problems have been covered in depth. Both topics have been subdivided into a) Methods and Techniques, and b) Clinical Applications. Both parts are updated and have critically evaluated available automatic sphygmomanometers and portable computers equipped with different techni ques (e. g. , auscultation, oscillometry, plethysmography). New blood pressure (BP) diagnostic thresholds for hypertension were released, which were set at 130/80 mmHg. As a consequence, millions of individuals in the world will be diagnosed as hypertensive, recognizing that they are at greater risk of presenting a CV event. Prevention and control of high BP will become the main focus for reducing the burden of CV disease, requiring a changing of cultural beliefs in some way similar to what happened in the last century with smoking. Strategies for prevention of the rising of BP with age, and the BP reduction in individuals already with high levels, are more complex than those related to smoking control. These strategies involve solid evidence to be implemented in populations. The extensive scientific literature dealing with hypertension and BP regulation is among the top dedicated to a single disease. The chapters and contents follow the clinical reasoning pathways. The characterization of the risks of high blood pressure is presented in the first chapter, discussing the evidence that led to changes in diagnostic thresholds and to the recommendations for maintaining BP within these limits in populations. Reasons for BP rising with age will follow, identifying the causes that must be fought to preventing the incidence of hypertension. Diagnosis of hypertension deserves a special chapter. The final chapter presents the fundamentals to select drug and non-drug therapies indicated in the prevention and controlling of high blood pressure.
