Book excerpt: There has been significant progress in the field of neurorehabilitation over the past twenty years, particularly in the assessment and management of cognitive impairment. More recently, the stakeholders in neurorehabilitation - clinicians, researchers, purchasers of services and clients - have become aware of the need to develop systems and services for managing the wide ranging psycho-social sequelae of acquired brain injury (ABI). Mood, behavioural and neuropsychiatric conditions have been found to be highly prevalent. Such disorders, at clinical or sub-clinical levels, are disturbing for clients themselves, and for their families, and present a challenge for enabling survivors to regain social roles. Many individuals also experience difficulties in related areas, such as pain management, drug and alcohol misuse, and in maintaining relationships. Given the wide-ranging psychological, psychiatric, health and social sequelae of ABI, rehabilitation services are often responding to people whose needs are complex and for which the evidence base for practice may be limited. In this Special Issue of Neuropsychological Rehabilitation, leading international experts provide reviews of current thinking on mood, behaviour and neuropsychiatric conditions, along with issues of drug and alcohol use, pain, sexuality and relationships after brain injury. Assessment and management issues are addressed, along with implications for service delivery in developed and in developing world contexts. This Special Issue will be invaluable to a wide range of neurorehabilitation professionals including clinical psychologists, neuropsychologists, speech and language therapists, occupational therapists, neurologists, neuropsychiatrists, clinical nurse specialists, physiotherapists, and case managers. Book excerpt: Neurobehavioral disorders are composed of a large group of behavioral impairments seen in association with brain disease (e.g., stroke, multiple sclerosis, dementia, and neuro-oncological conditions), transient as well as permanent brain impairments (e.g., metabolic and toxic encephalopathies), and/or injury (e.g., trauma, hypoxia, and/or ischemia). The neurorehabilitative assessment and management of such disorders is often poorly addressed in the context of overall neurological, psychiatric, and rehabilitative care. Too often, more basic, yet critical, aspects of behavioral assessment and treatment are not addressed or only superficially addressed by evaluating clinicians. Physicians often overly rely on pharmacological interventions as initial and/or sole treatment approaches rather than taking a pragmatic biopsychosocial approach that focuses on holistic disease state management. This chapter provides readers with an overview of the common behavioral impairments associated with brain dysfunction due to disease, injury, or toxicity. Details regarding the nature of impairments such as localization-related syndromes, affective disorders and personality disorders, among others are expounded. Principles of neurobehavioral assessment and treatment are examined including general guidelines for eliciting a history and physical, behavioral analysis, and functional behavioral assessment. General treatment caveats are provided including discussion of impairment and disability adaptation, and creation of positive behavioral supports. The topic of pharmacological management of neurobehavioral disorders is covered in numerous other references including of this text.

Immediate pain warns of harm or injury to the person with pain. If pain persists over time, more complex meanings can become interwoven with this primitive meaning of threat. These cognitive meanings include thoughts and anxiety about the adverse consequences of pain.
