Available in PDF, EPUB and Kindle. Book excerpt: Biomarkers, especially those based on pharmacogenomics testing, have proved to be extremely useful for type A adverse drug reactions. Clinical practice guidelines based on biomarker testing are presently being developed and updated for type A adverse drug reactions. In contrast, little attention has been paid to the potential use of biomarkers in type B adverse reactions, characterized by the occurrence of reactions not directly related to the pharmacological properties of the drug. Drug-induced hypersensitivity belongs to those type B reactions. Drug-induced hypersensitivity reactions involve complex mechanisms that include, among others, the metabolic activation and haptenization of drug metabolites. Hence, factors that influence the pharmacokinetics of drug and metabolites may contribute to the development of some drug-induced hypersensitivity reactions. This implies that processes such as ADME (absorption, distribution, metabolism and excretion) that are typically involved in type A adverse drug reactions, may have a role in hypersensitivity reactions too. In addition to metabolic activation, several signal transduction pathways participate and modulate the development and the clinical presentation of drug hypersensitivity. The diverse mechanisms underlying such drug-hypersensitivity reactions lead to four major groups of reactions according to the Gell and Coombs classification: immediate, cytotoxic, immune complex and delayed. The enormous complexity of drug-hypersensitivity reactions is a consequence of the variety of mechanisms involved, which may be related, among others, to drug metabolism, generation of antigenic signals, stimulation and maturation of dendritic cells, presentation of haptens and mechanisms of cytotoxicity. In addition, a plethora of possible clinical presentations exists, including urticaria, angioedema, anaphylaxis, cytopenias, nephritis, serum sickness, vasculitis, contact dermatitis, drug rash, eosinophilia and systemic symptoms, Stevens–Johnson syndrome, toxic epidermal necrolysis and acute generalized exanthematous pustulosis. The rapid progress in the field in recent years indicates that the combination of several disciplines is essential to understand the mechanisms involved in this particular, and not completely understood, type of adverse drug reactions. The objective of this Research Topic is to present insights obtained from both basic and clinical scientists, which may include studies related to the identification, validation, refinement and clinical implementation of biomarkers for drug-induced hypersensitivity. The Topic aims to include recent findings related, but not limited to, potential phenomic, genomic, proteomic, metabolomic and signal transduction biomarkers. These biomarkers could eventually be used in clinical practice and/or these might contribute, as a proof of concept, to our understanding of the complex events leading to drug hypersensitivity reactions. In addition the Topic will cover recent developments and methodological advances in the diagnosis, prevention and therapeutic management of drug-induced hypersensitivity.

To study the immunological biomarkers of allergic bronchopulmonary aspergillosis (ABPA) in patients with bronchial asthma.Methods. In prospective study were included 44 adult patients (Me - 43 years) with severe asthma. The control group consisted of 12 healthy people, matched by age and sex, without a history of allergic diseases (Me - 33 years). The severity of asthma and level of symptoms control were determined with GINA, 2016 criteria. The pulmonary function was assessed with FEV1 and FVC. An Asthma Control Test (AST) questionnaire was used. Allergy examination included skin testing with A. fumigatus allergen (u00abAllergopharmau00bb, Germany), determination of the serum total IgE level (u00abPolygnostu00bb, Russia) and specific IgE (sIgE) for fungal allergens (u00abAlkorBiou00bb, Russia). Determination of the serum concentration of TARC (u00abR&D Systemsu00bb, USA), TSLP (u00abR&D Systemsu00bb, USA), IL-8 (u00abVector-Bestu00bb, Russia) was performed by ELISA. The obtained data were processed using the software system STATISTICA 10 and presented as the median (Me) and upper and lower quartiles (Lq-Hq). Severe asthma with fungal sensitization (SAFS) and ABPA were diagnosed with R. Agarwal et al. 2013 criteria.Results. SAFS (positive skin test results and fungal allergens sIgE levels (.35 IU/ml) was diagnosed in 14 (32%) patients, ABPA u2013 13 (30%). In ABPA patients were identified significantly lower AST score and FVC, FEV1. The TSLP serum levels did not differ between ABPA patients u2013 12.0 (8.8u00f724.7), SAFS u2013 16.8 (9.7u00f727.7), asthma u2013 22.8 (14.6u00f731.8), and the control group u2013 13,2 (9,1u00f722,1) pg/ml. The serum TARC concentration was significantly higher in ABPA patients u2212 733.5 (540.0u00f7812.0) compared with the asthma group u2013 429.1 (218.0 u00f7 571.3, p=0.001) and control group 202.5 (195.9u00f7256.0, p=0.001) pg/mg. Book excerpt: Asthma is a heterogeneous disorder on molecular, cellular, physiological, and clinical levels, but there is a lack of consensus on how discrete subtypes of asthma should be defined. Gene expression profiling of bronchial samples from asthma patients and healthy controls has enabled the definition of molecular subtypes of asthma which correspond to distinct pathophysiological features. Genomic data from bronchial samples has been used to identify non-invasive biomarkers such as serum periostin, fractional exhaled nitric oxide (FeNO), and blood eosinophil counts which correspond to these molecular subtypes. These biomarkers have been used in recent proof-of-concept clinical trials of investigational biologic therapies directed at the type 2 cytokines interleukin (IL) 5 and 13 to stratify patients according to the activity of the targeted pathways, enabling the enrichment of subsets of patients most likely to show clinical benefit from those interventions. However, translating these biomarkers into companion diagnostic tests subject to regulatory approval will be a complex process, and intriguing initial findings must be verified prospectively in pivotal clinical trials.
