Since face is the most prominent part of the body, fractures of the facio-maxillary skeleton is a common finding with most of the road traffic accidents, industrial accidents and altercations. Rigid internal fixation with various bioresorbable materials has recently been studied by pioneers in the field and reported very high success rates with bioresorbable plates and screws. Biocompatible and bioresorbable poly-alpha-hydroxyl acid derivatives namely poly-L-lactide (PLLA), poly glycolide (PGA) and polydioxanone have been proposed as potential orthopedic repair materials. A new technologic advancement in biomaterials research has provided a copolymer of poly-L-Lactic acid and glycolic acid (PLLA-PGA) in the field of craniofacial surgery. The strength, malleability and resorption profile can be further favourably tailored by incorporation of caprolactone (CL), dioxanone (DI) or trimethylene carbonate (TMC).

Traditional reduction methodologies involve the use of metallic plates, screws, and wires, along with their associated problems. Therefore, biodegradable fixation appliances, which should provide initial rigidity but then gradually weaken so that newly-forming bone is exercised, have been investigated. COmposite plates made from poly (DL-lactide) and reinforced with calcium metaphosphate (CMP) fibers are very strong, but they weaken rapidly when exposed to an aqueous environment because of a disruption of the interface between the reinforcing fibers and polymer matrix. Coating the plates with a hydrophobic, biodegradable film slows this weakening, but more protection is required. Smaller-diameter fibers and better fiber wetting by the matrix also result in more stable composites, but even nonbiodegradable polymers reinforced with CMP fibers rapidly lose strength in saline. Methods to protect the fiber/polymer interface from fluid attack, such as fiber coatings, alternative bioresorbable glasses, and improved composite fabrication techniques have been investigated. Book excerpt: Partially biodegradable composites were prepared from a biodegradable polyester, reinforced with carbon or ceramic fibers, and evaluated for their potential use in fixing mandibular fractures resulting from maxillofacial injuries. The composites, made using a lamination technique, were found to possess the strength and rigidity of bone. Their incubation in phosphate-buffered saline at 37°C resulted in the gradual reduction of both flexural strength and flexural modulus of the samples. Reductions in strength and rigidity were paralleled by commensurate reductions in the viscosity of the matrix resin. Evaluation of the model system demonstrated the potential for improved management of fracture wounds using biodegradable materials.
