It also consolidates the positioning of the Foundation for an Industrial Safety Culture, highlighting what is currently considered at stake in terms of safety training, taking into account the system of constraints the different stakeholders are submitted to. It reports some success stories as well as elements which could explain the observed plateau in terms of outcome. It identifies some levers for evolution for at-risk industry and outlines a possible research agenda to go further with experimental solutions.

She demonstrates how these texts use risk to question and re-imagine the norms and practices of contemporary global citizenship. Beyond Safety thus brings together three of the central keywords of contemporary literary criticism of the last ten years (cosmopolitanism, precarity, neoliberalism) and shows how their intersection allows for a fuller conception of contemporary life and imagines a new global future.

Many safety management systems are mere “paper tigers” and in reality, just a paper-exercise. Valuable lessons can be learnt from how safety was mismanaged or had become dysfunctional in many organizations so that these pitfalls can be avoided. Lessons can also be learnt from organizations that have successfully navigated their safety management despite the challenges discussed in the book. Stakeholders need to scrutinize the effectiveness of the controls in place, rather than be another participant in the safety charade. To maximize payback and effectiveness, a safety management system must attend to the needs of each different organization. A “one-size-fits-all” standardized solution will invariably be sub-optimal. Engineering has experienced a technological revolution, but the basic engineering techniques applied in safety and reliability engineering, created in a simpler, analog world, have changed very little over the years. In this groundbreaking book, Nancy Leveson proposes a new approach to safety—more suited to today's complex, sociotechnical, software-intensive world—based on modern systems thinking and systems theory. Revisiting and updating ideas pioneered by 1950s aerospace engineers in their System Safety concept, and testing her new model extensively on real-world examples, Leveson has created a new approach to safety that is more effective, less expensive, and easier to use than current techniques. Arguing that traditional models of causality are inadequate, Leveson presents a new, extended model of causation (Systems-Theoretic Accident Model and Processes, or STAMP), then shows how the new model can be used to create techniques for system safety engineering, including accident analysis, hazard analysis, system design, safety in operations, and management of safety-critical systems. She applies the new techniques to real-world events including the friendly-fire loss of a U.S. Blackhawk helicopter in the first Gulf War; the Vioxx recall; the U.S. Navy SUBSAFE program; and the bacterial contamination of a public water supply in a Canadian town. Leveson's approach is relevant even beyond safety engineering, offering techniques for “reengineering” any large sociotechnical system to improve safety and manage risk.

In par-ticular, the dominant traffic violation, speeding, is analyzed. Rothe looks at traffic safety in a new way by presenting it as part of a social scientific framework. He provides a basis for future exploration of this kind. Accident records show a flattening of the safety curve since the early Seventies: instead of new kinds of accident, similar safety deficiencies have become recurrent features in accident reports. This suggests the need to review traditional accident prevention strategies, focused almost exclusively on the action or inaction’s of front-line operational personnel. The organizational model proposed by the authors is one alternative means to pursue safety and prevention strategies in contemporary aviation; it is also applicable to other production systems. The model argues for a broadened approach, which considers the influence of all organizations (the blunt end ) involved in aviation operations, in addition to individual human performance (the sharp end ). If the concepts of systems safety and organizational accidents are to be advanced, aviation management at all levels must be aware of them. This book is intended to provide a bridge from the academic knowledge gained from research, to the needs of practitioners in aviation. It comprises six chapters: the fundamentals, background and justification for an organizational accident causation model to the flight deck, maintenance and air traffic control environments. The last chapter suggest different ways to apply the model as a prevention tool which furthermore enhances organizational effectiveness. The value of the organizational framework pioneered by Professor Reason in analyzing safety in high-technology production systems is felt by his co-authors to have an enduring role to play, both now and in coming decades. Rachel Carson had designed Silent Spring to shock the public into action against the misuse of chemical pesticides. More than anything else, the book also served as an ecological primer, demonstrating the interrelationship of all things and the dependence of each on a healthy environment for survival. Today, Silent Spring is generally credited with providing impetus to the whole range of anti-pollution laws that came into force in the 1970s. It is also perceived as having played a crucial role in the eventual banning of DDT as well as in the restricted use or total phasing out of the most notorious hard pesticides identified in the book. The vigorous growth of the chemical industry geared to the production of newer and ever more powerful pesticides can be traced to the introduc tion of the organochlorine insecticide DDT in the 1940s. These pesticides were meant not only to control insects but also animal pests, disease and weeds. Initially their development was based on the belief that they would provide a definitive solution to pest and vector problems.

In Beyond Fear, Bruce Schneier invites us to take a critical look at not just the threats to our security, but the ways in which we're encouraged to think about security by law enforcement agencies, businesses of all shapes and sizes, and our national governments and militaries. Schneier believes we all can and should be better security consumers, and that the trade-offs we make in the name of security - in terms of cash outlays, taxes, inconvenience, and diminished freedoms - should be part of an ongoing negotiation in our personal, professional, and civic lives, and the subject of an open and informed national discussion. Book excerpt: How black Americans use digital networks to organize and cultivate solidarity Unrest gripped Ferguson, Missouri, after Mike Brown, an unarmed black teenager, was shot and killed by Officer Darren Wilson in August 2014. Many black Americans turned to their digital and social media networks to circulate information, cultivate solidarity, and organize during that tumultuous moment. While Ferguson and the subsequent protests made black digital networks visible to mainstream media, these networks did not coalesce overnight. They were built and maintained over years through common, everyday use. Beyond Hashtags explores these everyday practices and their relationship to larger social issues through an in-depth analysis of a trans-platform network of black American digital and social media users and content creators. In the crucial years leading up to the emergence of the Movement for Black Lives, black Americans used digital networks not only to cope with day-to-day experiences of racism, but also as an incubator for the debates that have since exploded onto the national stage. Beyond Hashtags tells the story of an influential subsection of these networks, an assemblage of podcasting, independent media, Instagram, Vine, Facebook, and the network of Twitter users that has come to be known as “Black Twitter.” Florini looks at how black Americans use these technologies often simultaneously to create a space to reassert their racial identities, forge community, organize politically, and create alternative media representations and news sources.
