Book excerpt: Since September 11th of 2012, the committee has been focused on the tragedy in Benghazi, Libya, where terrorists killed four Americans that day, including our Ambassador, the first U.S. Ambassador killed in the line of duty since 1979. The focus of today's hearing, which is our fourth, is the troubling lack of accountability we have seen within the State Department since that time. The bottom line is that over 1 year later no State Department personnel have been held accountable for the Department's failure to protect the Benghazi consulate and the U.S. personnel there, not one. As we know, there were so many things with the State Department's decision-making before the fatal attack. In the face of a glaring need, with violence in Benghazi mounting, critical security requests from the field were denied at State. The Department was asleep on 9/11, and this led to the Accountability Review Board to find, in their words, systemic failures and leadership and management deficiencies at senior levels within two State Department bureaus. But no State Department personnel have been fired or even disciplined. No one has missed a paycheck. Accountability can be painful. Those making bad decisions may have long and otherwise good records. But the Department cannot have a culture of accountability, which is what any well-functioning organization needs, and which is essential to protecting its personnel, if no one, literally no one, is held accountable for the mismanagement and poor leadership the ARB itself identified

Book excerpt: "Pursuant to Title III of the Omnibus Diplomatic and Antiterrorism Act of 1986, 22 U.S.C. § 4831 et seq., (the "Act"), Secretary of State Hillary Rodham Clinton convened an Accountability Review Board (ARB) for Benghazi to examine the facts and circumstances surrounding the September 11-12, 2012, killings of four U.S. government personnel, including the U.S. Ambassador to Libya, John Christopher Stevens, in Benghazi, Libya. A series of attacks on September 11-12, 2012 involving arson, small-arms and machine-gun fire, and use of rocket-propelled grenades (RPGs), grenades and mortars, focused on two U.S. facilities in Benghazi, as well as U.S. personnel en route between the two facilities. In addition, the attacks severely wounded two U.S. personnel, injured three Libyan contract guards and resulted in the destruction and abandonment of both facilities--the U.S. Special Mission compound (SMC) and Annex. Four Board members were selected by the Secretary of State and one member from the intelligence community (IC) was selected by the Director for National Intelligence. Ambassador Thomas R. Pickering served as Chairman, with Admiral Michael Mullen as Vice Chairman. Additional members were Catherine Bertini, Richard Shinnick, and Hugh Turner, who represented the IC. The criminal investigation of the September 11-12, 2012, Benghazi attacks, for which the statutory responsibility rests with the Federal Bureau of Investigation (FBI), was still underway at the time of this report. The Board enjoyed excellent cooperation with the Department of Justice and FBI throughout preparation of this report. The key questions surrounding the identity, actions and motivations of the perpetrators remain to be determined by the ongoing criminal investigation. As called for by the Act, this report examines: whether the attacks were security related; whether security systems and procedures were adequate and implemented properly; the impact of intelligence and information availability; whether any other facts or circumstances in these cases may be relevant to appropriate security management of U.S. missions worldwide; and, finally, whether any U.S. government employee or contractor, as defined by the Act, breached her or his duty."--Introduction.

Book excerpt: When Secretary Clinton transmitted the report of the ARB, the Accountability Review Board, to our committee, she noted in her accompanying letter that all of us have a responsibility to provide the men and women who serve this country with the best possible security and support. Most of all she says, ''It is my responsibility as Secretary of State.'' Tragically the Department did not meet its responsibility to our personnel in Libya. The lethal attack on our diplomatic mission in Benghazi was not the result of a protest against an obscure video as was initially claimed. Instead, and as the evidence makes clear, the attack was coordinated and carried out by terrorists targeting U.S. personnel. After the attack that killed Ambassador Stevens, Sean Smith, Tyrone Woods and Glen Doherty, the Secretary of State convened an ARB, which is required by statute due to the fatalities at the post. The ARB states that the attacks on our U.S. Nation in Benghazi on September 11th of this year were, and I quote, ''terrorist attacks.'' Dispatches from the command center of the State Department's Bureau of Diplomatic Security on the day of the attack clearly reported it as a terrorist event, yet officials in Washington refused to recognize and label the attack both during and after September 11th for what it was.
