Book excerpt: The Forum on Microbial Threats (previously named the Forum on Emerging Infections) was created in 1996 in response to a request from the Centers for Disease Control and Prevention (CDC) and the National Institutes of Health (NIH). The goal of the Forum is to provide structured opportunities for representatives from academia, industry, professional and interest groups, and government to examine and discuss scientific and policy issues that are of shared interest and that are specifically related to research and prevention, detection, and management of emerging infectious diseases. In accomplishing this task, the Forum provides the opportunity to foster the exchange of information and ideas, identify areas in need of greater attention, clarify policy issues by enhancing knowledge and identifying points of agreement, and inform decision makers about science and policy issues. The Forum seeks to illuminate issues rather than resolve them directly; hence, it does not provide advice or recommendations on any specific policy initiative pending before any agency or organization. Its strengths are the diversity of its membership and the contributions of individual members expressed throughout the activities of the Forum. Recent increased attention to both United States and international public health systems as well as the medical research and treatment infrastructure has revealed significant deficiencies in their capacity to respond to infectious diseases. Medical and public health professionals may be poorly equipped to detect, diagnose, and treat common infectious diseases as well as those diseases that pose an unexpected threat. The need for the development of domestic and international training programs in the expanding field of emerging and reemerging infectious diseases is well recognized. Well-trained infectious disease professionals form the basis of a strong national healthcare system. The Forum on Emerging Infections (now renamed the Forum on Microbial Threats) convened a 2-day workshop discussion-the subject of this summary-to examine the education and training needs to ensure an adequate infectious diseases workforce. The workshop reviewed trends in research training programs and discussed the requirements for establishing successful educational initiatives and training programs to ensure a competent and prepared workforce for current and future challenges in infectious diseases. Some key disciplines explored as case-study examinations included infectious disease epidemiology, vaccinology, vector biology, and public health laboratorians.

Book excerpt: In May 2014, the Internal Audit Service (IAS) recommended that ECDC should 'tailor the training efforts to cover the existing capacity gap'. Specifically, the IAS expressed that ECDC should:  'Estimate, on the basis of a methodology agreed with the Member States and in collaboration with the National Focal Points for Training (NFP-T), the desirable number of specialists and their related skills and take stock of the currently existing resources in the area of disease prevention and control across Europe.' - 'Carry out, in collaboration with the national competent bodies, a comprehensive training needs analysis in the area of disease prevention and control, using a harmonised assessment approach.' - 'As a result, determine the capacity gap at Member State level at short, medium and long-term perspective and tailor further actions, in collaboration with national and regional competent authorities, to address the existing gap.' To fulfil the IAS recommendations, ECDC initiated a training needs assessment by conducting a Training Needs Assessment (TRNA) survey in 2015 and proposing the following action plan:  Action 1: ECDC plans, as far as possible, to use the results of the mandatory reporting from Member States to the Commission on the state of their preparedness under Decision 1082/2013/EU to assess how many specialists in the national workforce in each Member State comply with the EU-competencies for epidemiologists and that are able to fulfil the tasks related to their needs. - Action 2: As this will not cover all the assessment needs, ECDC will in addition rely on the National Focal Points for Training (NFP-Ts) to supply complementary information. - Action 3: ECDC will discuss the results of the survey with NFP-Ts and produce a training roadmap to meet the training needs in epidemiology; a similar analysis will be conducted for other related disciplines in a structured set of dialogues.  Action 4: In coordination with NFP-Ts and the ECDC Advisory Forum (AF), ECDC will conclude the gap analysis and finalise the related roadmap (id est add training activities aimed at closing the gap). This summary report presents the results of this action plan and is divided into two main parts: - Part 1 maps information on expressed public health training needs and identifies the most appropriate methodology for an EU/EEA-wide training needs assessment to complement the data. - Part 2 presents the results of the TRNA survey 2015. A detailed action plan with concrete activities and timeline can be found in Annex 1.

Book excerpt: ECDC has been working with EU Candidate Countries since 2006 and with EU Potential Candidate Countries since 2009 to prepare them for participation in ECDC activities. Work with these countries has been mainly financed by a Contribution Agreement between the Directorate-General for Neighbourhood and Enlargement Negotiations (DG NEAR) and ECDC under the Instrument for Pre-accession Assistance (IPA). Enlargement countries are required to adopt and implement the EU acquis. Strengthening their administrative, institutional, and prevention and control capacities with regard to communicable diseases is part of this process and falls within the remits of ECDC. A country that wishes to join the EU, submits an application for EU membership to the European Council, which asks the European Commission to assess the applicant's ability to meet the necessary EU requirements and a core set of accession criteria.^It is important that an enlargement country makes EU law part of its own national legislation. It is equally important that the country develops appropriate administrative and institutional structures to implement and enforce this legislation. The Commission evaluates the progress made by enlargement countries at regular intervals. To evaluate progress in the area of communicable diseases, the European Commission's Directorate-General Health and Food Safety (DG SANTE) has requested ECDC to develop methodologies and tools for assessing the capacity development, surveillance, preparedness and response, and health governance in this domain for all EU enlargement countries (EU Candidate Countries and Potential Candidates). Following this request, ECDC developed and tested a set of tools which now constitute a complete assessment package (see Annexes).^DG SANTE uses ECDC's assessment of a (Potential) Candidate Country's capacity in the area of communicable diseases to contribute to the annual EU enlargement progress report. Based on the results of their evaluations and assessments, ECDC and DG SANTE will provide guidance to the enlargement countries on how to strengthen communicable disease prevention and control systems and how to ensure the systems' sustainability.

The 'Council recommendation 2009/C of 9 June 2009 on patient safety, including the prevention and control of healthcare associated infections' and the European Commission's related implementation reports, stressed the need for fostering education and improving the training of healthcare personnel at all levels, ensuring adequate numbers of trained infection control and hospital hygiene (IC/HH) staff. Understanding the current trends in Europe in IC/HH training for healthcare workers (HCWs) is an essential pre-requisite for the definition and implementation of policies and programmes aimed at increasing and harmonising capacity and training in IC/HH all over Europe. The 'Improving Patient Safety in Europe' (IPSE) project (2006) and the ECDCcommissioned project 'Training in Infection Control in Europe' (TRICE) (2010) conducted surveys of the status of IC/HH training in Europe and developed a document called 'Core competencies for IC/HH professionals in the European Union' In 2012, ECDC launched the 'Training in Infection Control in Europe - Implementation Strategy' (TRICE-IS) project, which included a third survey to assess IC/HH training capacity in Europe. This technical report describes the results of the 2014 survey on the evolution of the IC/HH capacity and training in Europe, comparing 2014 data with those from the two previous surveys conducted in 2006 (IPSE) and 2010 (TRICE).

The purpose of this review is to contribute to the evidence base on health communication evaluation research in order to aid public health professionals and researchers in the development of future evaluation strategies. The review is divided into two sections. In the first section the focus is on reviewing evaluations of campaigns undertaken in European Union (EU) and European Economic Area (EEA) countries, however, examples from the wider European region are also included. The second section, addressing challenges posed by campaign evaluation, draws on broader international literature pertaining to the identification of health communication campaign evaluation tools, frameworks and models. It would appear that there has been very few high quality European evaluation studies carried out in the last decade in relation to communicable disease prevention campaigns. Nevertheless, it is evident in reviewing the literature that there are valuable existing frameworks and guidelines that can help guide and inform evaluation research development.

Book excerpt: This report presents the analysis of data reported for 2008 by the 27 EU Member States and three EEA/EFTA countries: Iceland, Liechtenstein and Norway. The main aim of this report is to provide some indication, based on the available data, of where the main burden of communicable diseases now lies in the European Union. In these areas, more concerted action is required in order to decrease the present and potential future burden on society, on public health and healthcare systems, and to reduce human suffering. These data contribute to ECDC's task of providing the evidence-base for action, to help identify and share practices, and to suggest methods for follow-up of interventions. Although there has been much progress in improving the quality and comparability of the data, the reader is still cautioned against making direct comparisons of the notification rates between countries. Surveillance systems differ widely, and the relationship between reported or notified and actual incidence varies from country to country for many diseases. For the first time the annual Analysis of Threats monitored in the EU is being reported separately. Possible readership: Public health officials, health professionals, policy makers, surveillance experts.

Book excerpt: ECDC aims to foster and reinforce the public health microbiology system to provide timely and reliable information for infectious threat detection, the assessment of such threats, and their surveillance at the Member State and European Union levels, thereby ensuring the effective prevention and early control of infectious diseases. To ascertain how well this is delivered, ECDC developed, in close collaboration with national microbiology focal points from all European Union/European Economic Area (EU/EEA) countries and the ECDC Advisory Forum, the European Union Laboratory Capability (EULabCap) monitoring programme. The EULabCap bi-annual surveys assess key public health microbiology service capabilities and capacities for EU surveillance and epidemic preparedness. The monitoring results help policymakers at all levels identify possible areas for action and evaluate the functional impact of capacity-strengthening activities and health system reforms. This fifth consecutive EULabCap report presents EU/EEA laboratory capabilities and capacities in 2018 and the trends of previous survey results over the period 2013-2018.
