Book excerpt: Introduction : la gonarthrose sur genuvalgum présente des spécificités source de difficultés lors de la mise en place d'une arthroplastie totale du genou. Le but de cette étude est d'évaluer cliniquement et radiologiquement les résultats d'une technique opératoire tentant de répondre à ces difficultés. Matériel et méthodes : 105 genoux ont été revus, il s'agit de 77% de femmes et de 23% d'hommes, d'un âge moyen de 68 ans (39-87) et d'un IMC moyen de 27,2kg/m2 (17-43,1). La goniométrie préopératoire montrait une déviation moyenne en valgus de 9°. Toutes ces arthoplasties ont été réalisées par la voie d'abord antérolatérale passant sous le vaste latéral et associant une ostéotomie de la tubérosité antérieure du tibia et une libération de l'aileron rotulien externe avec plastie d'agrandissement synoviale de l'aileron en fin d'intervention. L'évaluation clinique et radiographique de ces patients a été réalisée avec un recul moyen de 5,9 ans (1-13). Résultats : cliniques- au plus long recul, le score IKS genou moyen est de 92,7 (64-100) avec 97 % de bons et très bons résultats. L'item mobilité de ce score est particulièrement satisfaisant, avec une flexion moyenne de 122,5° au plus long recul. Le score IKS fonction moyen est de 85,9 (45-100) avec 88% de bons et très bons résultats. Ce score est meilleur lorsque le plateau tibial fixe est utilisé et que les valgus préopératoire est inférieur à 10%. Parmi les complications spécifiques de cette voie d'abord, cinq patients ont nécessité une mobilisation sous anesthésie générale pour une raideur postopératoire, trois patients ont présenté un arrachement précoce de la tubérosité antérieure du tibia nécessitant une reprise chirurgicale et trois patients ont présenté une frcture métaphysaire de fatigue de l'extrémité supérieure du tibia fragilisé par l'ostéotomie de la tubérosité. Trois arthrolyses fémoro-patellaires ont été réalisées pour un clunk syndrome et trois cas de pseudarthrose de la tubérosité antérieure du tibia ont nécessité une reprise chirurgicale avec greffe osseuse. Radiographiques-un angle HKA égal à 180+/-2° correspond à l'ave mécanique neutre du genou a été retrouvé dans 85,7% des cas. 96,2% de ces genoux ont une une rotule parfaitement centrée. Conclusion : cet abord chirurgical de la gonarthose sur genu valgum associé à une prothèse de glissement postérostablisée, permet d'obtenir avec un recul moyen de 5,9 ans 97% de bons et très bons résultats avec un taux de satisfaction de 98% des patients. La technique de l'ostéotomie de la tubérosité antérieure du tibia doit être minutieuse, avec une ostéosynthèse solide, parvissage en compression et cerclages, pour éviter les complications d'arrachement ou de pseudarthrose

Dans ce cadre sont réalisées des ostéotomies de correction, en particulier des ostéotomies tibiales de valgisation (OTV) en cas d'arthrose fémoro-tibiale interne sur genu varum. Cette ostéotomie résout le défaut d'axe, décharge le compartiment dégradé, mais ne stoppe pas le processus dégénératif touchant cet interligne; si la symptomatologie réapparaît avec arthrose évolutive, une arthroplastie totale de genou est une solution thérapeutique envisageable. Quarante quatre patients (47 genoux) ont bénéficié d'une ostéotomie tibiale de valgisation suivie d'une prothèse totale de genou (PTG) dans un délai moyen de 7,8 ans au Centre Hospitalier d'Amiens. Il s'agit d'une ostéotomie de fermeture dans 43 cas, à un âge moyen de 57 ans. Le recul moyen des prothèses totales de genou est de 3,9 ans (44 PTG sans conservation du pivot central). Les résultats fonctionnels ont été évalués au moyen du score de l'International Knee Society ; les prothèses après ostéotomies de notre échantillon procurent des résultats fonctionnels faibles (IKS f moyen 57/100). Le score IKS global reste stable dans le temps. Les échecs des PTG regroupent 4 descellements aseptiques et 2 septiques sans corrélation avec l'ostéotomie préalable. L'OTV est d'indication légitime chez le sujet jeune et actif, en précisant que les résultats apportés par une PTG après OTV resteront modestes sur la fonction
