A means of relief in selected cases is afforded by chordotomy. The operation Will not be entirely successful in every case in which it is employed some pain fibers occasionally will escape the knife - but the Operation has now been performed sufficiently often to justify a belief in its efficacy. It is a delicate procedure, requires a skilful technic, and is not to be undertaken lightly by one untrained in the surgery of the spinal Cord. It requires on the part of the Operator a visualization of the microscopic anatomy of the cord, and a realization that a Slight misplacement of the incision may cause motor paralysis of one or both lower limbs, depending on whether the division is unilateral or bilateral. We 1'have written. On this subject previously and therefore do not refer to the literature at the present time. We report in this paper eight cases in Which chordotomy has been performed. All but one have not been reported previously, but in this one case chordotomy for certain reasons mentioned in the notes has been performed three separate times on the same side of the cord. In one case (case the operation was performed to remove the intense hyperesthesia above the anesthetic area produced by transverse myelitis, and was entirely successful. A large cerebral hemorrhage from the marginal artery of the lenticular nucleus or an embolism of the Sylvian artery causes, besides a hemiplegia, at the time of the acute illness a number of grave attendant symptoms, which are to be regarded as collateral effects of the local brain injury. As a result of the direct destruction the hemiplegia continues as the residuary state, but the collateral effects disappear. But exactly the same sort of a hemiplegia may occur, when a tumor of slow growth or a chronic encephalomalacia destroys the same locality, usually in the way that a monoplegia comes on gradually, e. G., of the leg, then a monoplegia of the arm and finally of the facio-lingual region, so that a hemiplegia at last exists. Corresponding to this chronic course severe general symptoms may be completely lacking. But the location of the brain destruc tion is the same in both instances, and hence the right is proven to compare the residuary focus with the chronic focus. We have equally the right to compare the residuary changes of the contents with respect to the localization of the process to the changes of the contents in chronic psychoses. According to Urie Bronfenbrenner, one of the world's foremost developmental psychologists, laboratory studies of the child's behavior sacrifice too much in order to gain experimental control and analytic rigor. Laboratory observations, he argues, too often lead to "the science of the strange behavior of children in strange situations with strange adults for the briefest possible periods of time." To understand the way children actually develop, Bronfenbrenner believes that it will be necessary to observe their behavior in natural settings, while they are interacting with familiar adults over prolonged periods of time. This book offers an important blueprint for constructing such a new and ecologically valid psychology of development. The blueprint includes a complete conceptual framework for analysing the layers of the environment that have a formative influence on the child. This framework is applied to a variety of settings in which children commonly develop, ranging from the pediatric ward to daycare, school, and various family configurations. The result is a rich set of hypotheses about the developmental consequences of various types of environments. Where current research bears on these hypotheses, Bronfenbrenner marshals the data to show how an ecological theory can be tested. Where no relevant data exist, he suggests new and interesting ecological experiments that might be undertaken to resolve current unknowns. Bronfenbrenner's groundbreaking program for reform in developmental psychology is certain to be controversial. Proliferation of knowledge has included a widening base of research data and changing or new concepts and theories regarding classification, measurement methods, and etiology of abnormal behaviors and mental disorders. It has been an active and productive period for biological and behavioral scientists and clinicians, particularly in terms of changing notions of the complex interaction of environmental and biological factors in many disorders. For example, with the classic disorders-such as anxiety and dissociative disorders-our understanding, while far from perfect, has been greatly enhanced in recent years. Whereas there was almost a vacuum of empirical knowledge ten years ago about the personality disorders, concentrated efforts have been undertaken to investigate classification, comorbidities, and expression of the personality disorders, and variants in normal personality traits. In addition, scientific advances in the fields of behavioral medicine, health psychology, and neuropsychology have greatly contributed to our knowledge of psychopathology and the interplay of psychobiological factors. It is now commonly acknowledged that psychopathology is not limited to the traditional mental illness categories; it also plays a significant role in many physical illnesses, such as cancer and AIDS. With these developments, it became clear that the first edition of this handbook was outdated and that a revision was needed.
