Book excerpt: En France, on estime chaque année pour 100 000 habitants, 600 réalisations de tentatives de suicide dont 20 décèderont à l’issue de cette tentative. Les tentatives d’autolyse balistiques représentent 24% des suicides. Le profil psychologique des suicidants de la face est assez particulier: impulsivité, alcoolisme chronique, peu d’antécédents psychiatriques ou de tentatives de suicide. L’arme la plus souvent utilisée est le fusil de chasse et l’orifice d’entrée est généralement intra-oral ou sous-mental. Les délabrements maxillo-faciaux résultant sont pluritissulaires, tridimensionnels et concernent un ou plusieurs étages de la face dont l’issue est souvent le décès du suicidé. Chez les survivants, les conséquences sont esthétiques avec éclatement du visage, fonctionnelles avec troubles de la respiration, de la mastication, de la déglutition, de la phonation; socioprofessionnelles, familiales, psychologiques, compromettant toute réinsertion sociale ultérieure. Une fois l’urgence vitale écartée (libération des voies aériennes supérieures, hémostase, lutte contre les complications neurologiques) la prise en charge de ces suicidants sera pluridisciplinaire et s’articulera en trois principales phases. Le traitement primaire dont le principal objectif est de maintenir le volume de la face en empêchant la coaptation des moignons osseux. Le traitement secondaire dont le principal objectif est de combler les déficits osseux et les déficits des tissus mous afin de minimiser les séquelles esthétiques et fonctionnelles. Le traitement tertiaire correspond à la prise en charge des séquelles. Il a pour but d’améliorer l’esthétique et les fonctions. La prothèse maxillo-faciale a un rôle à jouer à toutes les phases de traitement en association avec les techniques chirurgicales. Nous exposons quelques cas cliniques mettant en évidence l’apport de la prothèse maxillo¬faciale dans la prise en charge de ces patients.

Ceux-ci sont généralement associés à une intoxication alcoolo-tabagique ? Ces cancers et les traitements qui en découlent, notamment les protocoles de chirurgie d’exérèse, sont l’une des principales étiologies des pertes de substances vélo-palatines. L’étude des régions anatomiques concernées permet de prendre conscience des rapports et de la faible épaisseur des structures et nous montre le risque élevé d’aboutir à une communication bucco-nasale. Les modifications morphologiques causées par les cancers buccaux et leurs traitements provoquent des troubles fonctionnels lors de la mastication, la déglutition et la phonation. Par ailleurs elles entrainent un manque de soutien des tissus sains et ont donc un retentissement esthétique et psychologique. On comprend facilement le rôle important des obturateurs qui, mis en place précocement vont permettre de rétablir les fonctions orales, l’esthétique et ainsi agir sur le psychisme du patient. Ce travail s’attache à décrire les différents types d’obturateurs existant ainsi que les étapes cliniques nécessaires à leur confection. Le propos est illustré par l’étude d’un cas clinique du service odontologique de l’hôpital Saint André de Bordeaux. Enfin, la prothèse maxillo-faciale est une discipline en constante évolution : l’apport de la chirurgie et de l’implantologie améliore de manière considérable la qualité et le confort des prothèses obturatrices.

La plupart des soins sont portés sur des patients atteints de cancer des voies aéro-digestives. Les traitements de ces pathologies sont lourds et les pertes de substance consécutives sont très mutilantes. La perte de substance a des conséquences sur la fonction et l’esthétisme. Ces deux composantes sont étroitement liées et sont à l’origine d’une modification au niveau psychologique sur le patient du à leur fort impact. Etant donné la position de la prothèse maxillo-faciale dans la prise en charge du patient atteint de cancer, et l’impact psychologique qui accompagne ses traitements, il semble intéressant d’évaluer la qualité de cette prise en charge tant au niveau fonctionnel que psychologique. L’élaboration d’un questionnaire et son étude à suivre devrait nous permettre d’avancer une réponse, et d’améliorer encore la qualité de cette prise en charge

La prothèse maxillo-faciale fait partie du groupe de prothèse du corps humain, elle peut être externe (épithèse de nez ou du pavillon de l'oreille) ou même interne avec une plaque en titane pour remplacer l'os perdu après hémi-mandibulectomie.
