L'objectif de cette étude était de vérifier si cet enseignement améliore la compétence communication des étudiants en médecine qui y participent. Matériel et Méthode : C'était une étude observationnelle d'une cohorte d'étudiants de Diplôme de Formation Générale en Sciences Médicales 3 tirés au sort parmi les participants à l'enseignement en 2017. Leur compétence communication a été comparée au début et à la fin de l'enseignement, par l'échelle de Calgary-Cambridge simplifiée. Résultats : 34 étudiants ont été tirés au sort, 27 ont été évalués. Book excerpt: Introduction : La communication empathique dans la relation médecin-patient commence à se distinguer de l'empathie proprement dite, et des méthodes de formation surtout avec des hétéro-évaluations permettent, depuis une vingtaine d'années, non seulement une acquisition, dans une certaine mesure, de compétences qui lui sont propres, mais aussi le recueil de résultats bénéfiques pour le patient, notamment en soins primaires, incitant ainsi le développement de formations universitaires dans de nombreux pays, actuellement en phase d'ébauche sporadique en ce qui concerne la France. Méthodes, matériels et but de l'étude : Un protocole nommé "RECO-ÉMO-RER" a été ainsi élaboré à partir d'un certain nombre d'items issus, pour la plupart, d'études validées en langue anglaise. L'intervention a été composée d'une brochure accompagnée par une présentation vidéo et a été suivie par un questionnaire d'auto-évaluation, visant à sensibiliser médecins et étudiants à une meilleure communication avec les patients. Résultats : Les résultats ont montré un bénéfice net chez les 61 répondants, en majorité médecins généralistes, de répartition territoriale, de genre et d'âge homogènes, avec des différences post-interventionnelles significatives sur chacun des 8 items évalués. L'interprétation des résultats doit tenir compte de la taille d'effet (expérimental), qui variait énormément en fonction des méthodes statistiques conventionnelles employées (phénomène connu). En revanche, une nouvelle approche statistique nommée ODA ("Optimal Data Analysis") a montré des valeurs intermédiaires, exactes et à définition précise. Discussion et Conclusion: Le protocole a été efficace en stimulant une utilisation plus fréquente (quantitative) mais aussi plus consciente (qualitative) des huit compétences communicationnelles explorées par les items. En outre, et indépendamment du protocole, ces items ont été considérés par les participants comme étant pertinents dans les rencontres cliniques. Les améliorations dans les compétences étudiées ont été indépendantes du genre ; elles ont été positivement corrélées à la rigueur d'application du protocole et inversement corrélées à l'âge, au manque de formation et au score de la dimension "préoccupation empathique" ("empathic concern") de l'échelle Davis, le bénéfice étant d'autant plus important que ce score était plus bas. Les résultats statistiques obtenus mériteraient des analyses factorielles ultérieures (avec un effectif plus important). L'utilisation du protocole, sa méthodologie particulière, ainsi que le paradigme statistique d'ODA, facile à utiliser, devraient intéresser de plus les professionnels des soins primaires.

Notre enquête est la synthèse : d'une expérience clinique personnelle élaborée à partir de remplacements en médecine générale ; de l'expérience clinique des médecins généralistes et spécialistes plaçant la relation malade médecin au cœur de leur pratique, et en particulier de celle du Docteur Françoise Rodary ; de stages de communication animés par Jacques Salomé, psychosociologue et fondateur d'un centre de formation en relations humaines ; des animations de groupes "Balint" réalisées par le Docteur Michel Sapir, psychanalyste et psycho-somaticien ; d'une étude pluridisciplinaire de la relation basée sur les apports de la psychologie, de la psychanalyse, de la sociologie, de l'ethnologie et de l'histoire. Nous avons ainsi confirmé les conditions et les techniques permettant à un médecin d'écouter son patient à plusieurs niveaux : celui des faits, des événements objectifs ; celui du ressenti, de l'émotion ; celui du retentissement dans son histoire personnelle (résonance) ; celui de l'intellect enfin. Nous avons vu que cette écoute multiple permet au patient d'établir des LIENS avec sa maladie, son traitement et son entourage ainsi qu'avec lui-même et son soignant. Pour ce qui concerne les conditions d'écoute, nous avons mis l'accent sur la disponibilité du médecin et le retentissement que peut provoquer chez lui le discours de son patient. Pour ce qui concerne les techniques d'écoute, nous avons proposé 4 étapes dans le discours du médecin: 1. STIMULER le discours du patient ; 2. le REFORMULER ; 3. le CLARIFIER par un retour du message permettant au malade de saisir ce que le médecin a entendu ; 4. permettre au patient d'APPROFONDIR le point qui paraît central. Parallèlement aux conditions d'écoute, nous avons mis en évidence des obstacles à la relation, liés au médecin, au malade et au contexte institutionnel et socio-professionnel. Nous avons par ailleurs vérifié qu'une écoute attentive peut permettre au patient, s'il le désire, d'accéder au langage de sa maladie, c'est-à-dire à ce qu'elle signifie pour lui. Nous avons vu que cette approche ne constitue pas une psychanalyse, et demeure très accessible au médecin généraliste, avec l'aide éventuelle d'une démarche symbolique : par exemple, pour un enfant, dire un conte qui représente l'histoire de sa maladie ou de ses symptômes. Mais dans tous les cas, il sera fondamental pour le médecin d'accompagner chaque patient A SON PROPRE RYTHME, sans jamais se risquer à une interprétation sauvage, sans jamais "psychologiser" la relation à outrance. Il n'est donc pas question d'anticiper le désir de clarté à la place de son malade, mais de lui permettre d'y accéder par lui-même, grâce à une démarche d'accompagnement attentive et chaleureuse : il s'agit finalement de le rendre AUTONOME. L'histoire ainsi que l'étude ethnologique de la maladie nous ont permis de redécouvrir que cette démarche d'écoute active et d'accès aux multiples langages (dont le langage symbolique) puise ses origines dans le passé comme le montrent les écrits d'HIPPOCRATE et qu'elle est universelle comme en témoigne l'étude comparée des médecines traditionnelles que nous avons esquissée. Actuellement les médecines dites "parallèles" reprennent le flambeau de la relation. Cela explique peut-être en partie le succès grandissant qu'elles ont auprès de nombreux malades. Nous pensons que la médecine générale et hospitalière pourraient perdre de leur crédit si elles négligent cette approche relationnelle. Pour ne citer qu'un exemple, nous souhaitons que l'opération "nounours", menée à l'hôpital de La Timone à Marseille, ne reste pas seulement expérimentale : dans un service de chirurgie, on opère des ours en peluche devant les enfants, puis on opère... les enfants en diminuant la prémédication... et l'angoisse. Ainsi, face aux pièges de la technologie moderne et de l'institution, le médecin peut répondre par l'écoute et la créativité. Mais encore faut-il qu'il soit formé à cette écoute, la bonne volonté ayant ses limites. Or, il existe une nette discordance entre l'enseignement reçu à la faculté de médecine, orienté essentiellement vers le "somatique" et la réalité du cabinet du praticien où l'essentiel des patients consulte pour des symptômes dont l'origine est souvent d'ordre psychologique. Aussi nous formons le vœu que l'enseignement de la médecine intègre à son programme l'approche relationnelle du patient, au même titre que d'autres disciplines. Il reste à en définir les modalités : par exemple, la création d'un module de médecine relationnelle intégré à l'enseignement du second cycle et auquel participeraient des psychologues, des médecins généralistes, des médecins hospitaliers et... des patients peut-être. Des rencontres, des exposés sur les approches de la maladie et de la mort, étayés par des notions fondamentales de psychologie et d'anthropologie pourraient en constituer la trame. On peut aussi concevoir la création de groupes "Balint", réunissant médecins et étudiants en médecine. C'est peut-être à ce prix que le malade pourra être accepté dans toutes ses dimensions humaines et non plus morcelé dans son corps et dans son esprit par une médecine devenue très technique.

Le but de ce travail est de fournir au médecin en situation transculturelle les outils indispensables pour identifier la représentation du patient sur sa maladie. Le but ultime est d'aboutir à une décision partagée. Matériel et méthode : 15 entretiens individuels ont été réalisés selon la méthode EMIC chez des patients d'origine socio-culturelle différentes, enregistrés puis retranscris. La grille de questions a été inspiré de la liste des 9 questions de Kleinman. Une première analyse vise à faire ressortir le modèle explicatif du patient (illness, sickness et disease) avant de le confronter au modèle explicatif biomédical dans le but de mettre la lumière sur un éventuel décalage entre l'attente et ou les possibilités du patient et la démarche biomédicale. Résultats : Les principaux résultats mettent en évidence l'universalité de la représentation de la maladie selon les 3 principaux axes (cause, retentissement et parcours thérapeutique). Ils confirment également la pertinence de l'approche anthropologique et la médecine narrative. Ils mettent la lumière sur l'effet thérapeutique de la relation médecin traitant-patient lorsqu'elle est basée sur le partage. Par contre ils désignent le manque d'adaptabilité des soignants comme source de décalage entre la démarche biomédicale et l'attente du patient. Discussion : En conclusion, les orientations pédagogiques dans la formation des futurs soignants doivent s'intéresser davantage à tout ce qui concerne l'humain pour aboutir à une approche centrée patient.

In this classic of French cultural studies, Georges Didi-Huberman traces the intimate and reciprocal relationship between the disciplines of psychiatry and photography in the late nineteenth century. Focusing on the immense photographic output of the Salpetriere hospital, the notorious Parisian asylum for insane and incurable women, Didi-Huberman shows the crucial role played by photography in the invention of the category of hysteria. Under the direction of the medical teacher and clinician Jean-Martin Charcot, the inmates of Salpetriere identified as hysterics were methodically photographed, providing skeptical colleagues with visual proof of hysteria's specific form. These images, many of which appear in this book, provided the materials for the multivolume album Iconographie photographique de la Salpetriere. As Didi-Huberman shows, these photographs were far from simply objective documentation. The subjects were required to portray their hysterical "type"—they performed their own hysteria. Bribed by the special status they enjoyed in the purgatory of experimentation and threatened with transfer back to the inferno of the incurables, the women patiently posed for the photographs and submitted to presentations of hysterical attacks before the crowds that gathered for Charcot's "Tuesday Lectures." Charcot did not stop at voyeuristic observation. Through techniques such as hypnosis, electroshock therapy, and genital manipulation, he instigated the hysterical symptoms in his patients, eventually giving rise to hatred and resistance on their part. Didi-Huberman follows this path from complicity to antipathy in one of Charcot's favorite "cases," that of Augustine, whose image crops up again and again in the Iconographie. Augustine's virtuosic performance of hysteria ultimately became one of self-sacrifice, seen in pictures of ecstasy, crucifixion, and silent cries.

Works of modern literature are commonly theorized as symptomatic responses to the trauma of history. In a climate that tends to privilege crisis over critique, Debarati Sanyal argues that it is urgent to rethink literary experience in terms that recall its contestatory potential. Examining Baudelaire's poems afresh, she shifts the focus of critical attention toward an account of modernism as an active engagement with violence, specifically the violence of history in nineteenth-century France. Sanyal analyzes a literary current that uses the traditional hallmarks of modernism—irony, intertextuality, self-reflexivity, and formalism—to challenge the historical violence of modernity. Baudelaire and the committed ironists writing in his wake teach us how to read and resist the violence of history, and thereby to challenge the melancholy tenor of our contemporary "wound culture." In a series of provocative readings, Sanyal presents Baudelaire's poetry as an aesthetic form that contests historical violence through rhetorical strategies of complicity, counterviolence, and critique. The book develops a new account of Baudelaire's significance as a modernist by dislodging him both from his traditional status as a practitioner of "art for art's sake" and from his more recent incarnation as the poet of trauma. Following her extended analysis of Baudelaire's poetry, Sanyal in later chapters considers a number of authors influenced by his strategies—including Rachilde, Virginie Despentes, Albert Camus, and Jean-Paul Sartre—to examine the relevance of their interventions for our current climate of trauma and terror.
