Book excerpt: "A text-book for nurses on the use of antiseptics in gynaecology and midwifery, by the obstetrician and member of the Royal College of Physicians, John Shaw, which provides an insight into not only the developing level of antiseptic knowledge and practice at the time, but also into the state of formalised training of nurses. Dedicated to both Semmelweis and Lister, the work is based upon a series of lectures given to nurses between 1887-1889 at the Hampstead Home Hospital, in an effort to combat what Shaw felt was a subject 'still so imperfectly understood by the majority of nurses' (Preface). It is his hope that 'these lectures may be of some use, especially to that large class of nurses who have not had the advantage of the thorough instruction which now characterises the curriculum of the monthly nurse; for this reason, a short glossary of terms, as might possibly present difficulty, is appended' (ibid). Shaw goes on to present in the first chapter a brief historical synopsis of the introduction of antiseptics into general obstetrical practice, noting the importance of Semmelweis in particular, before outlining in the remaining chapters the main causes of infection, and the various antiseptic precautions and practices to be used. The work is illustrated by four plates including a microsopic representation if various germs, together with an evocative photograph showing two obstetric nurses preparing for theatre. A folding statistical table clearly charts the progress of two patients recovering from a blood cyst on the right ovary, and a caesarean section. It is, however, the chromolithographed frontispiece that most catches the attention, depcting a number of series of 'tubes of beef-tea or beef-jelly, which had been successfully sterilised in the manner already described, but were subsequently exposed to certain conditions of germ influence, each of which is distinctly illustrative of some error in obstetric nursing. The term disinfectant may not be used but the custom of making use of some disinfectants is general in every well kept home. A disinfectant is a substance that kills all germs that have power to infect or cause disease. We learned in a previous lesson something about germs and will remember that there are good and bad germs. All germs are not infectious, or disease, germs. Some infectious germs are much harder to kill than others. An antiseptic is a substance that prevents the growth of germs but does not necessarily kill them. Natural Disinfectants.--Without knowing exactly why she does so, except that "it is healthier" the housewife puts bedding, clothing, etc., out to sun and air them. Experiments with different kinds of germs have shown that exposure to bright sunshine for a few hours is one of the best ways of killing a great many kinds of germs. Letting the sunlight flood the room helps to check the growth of germs of all kinds. Dampness favors the growth of germs and therefore, sunlight by drying out a room, helps to keep it free from infectious germs. Soap and water also are important aids in this work. The housewife who uses soap and water liberally in every part of her home, admits the sunshine freely, and prevents dampness, has done much to keep it healthy. Excerpt: ...short space of time The pendulum had swung round again, and antiseptic surgery was once more the order of the day. Various antiseptics had been tried with varying degreesjof success, besides the old familiar friends such as carbolic, perchloride of mercury, iodoform peroxide of hydrogen and the like, newer treatments such as a new preparation of picric acid, quinine lotion, and (in France) powdered sugar, were experimented with, but none proved entirely satisfactory. An antiseptic which will destroy harmful germs is easy enough to find, the trouble is that most of them also irritate and destroy the body tissue with which they come into contact. Some antiseptics--carbolic for instance, and its preparations--are poisonous, and if applied to the surface of very large wounds would be absorbed into the system, and in extreme cases cause death by carbolic poisoning. The discovery of two new groups of antiseptics revolutionised the treatment of septic wounds: (1) the aniline series of which Flavine is the final word, and (2) the use of hypochlorous acid, either by the Carrel system of irrigation with Dakin's fluid, extensively used in France, or in the form of Eusol as used in England. The Bland Sutton Institute of Pathology working at the Middlesex Hospital has discovered Flavine. An American scientist, Dr. Dakin, evolved the fluid which bears his name, an ideal antiseptic which renders a wound sterile without destroying the body tissue, and a Frenchman, Dr. Carrel, has taught the practical method of applying it. These two discoveries form the point"of departure for an entirely new essay in surgery.
