Book excerpt: Cytotoxic T lymphocytes (CTL) and natural killer (NK) cells are powerful effectors of antitumor immunity. CTL recognize tumor antigens presented by human leukocyte antigen (HLA) molecules with antigen-specific T cell receptors (TCR) and are the key effector cells of the adaptive immune response. In contrast, NK cells lack antigen-specific receptors and are regulated by the balance of signals from activating and inhibitory receptors. These two types of cells cooperate and complement each other in eliciting host immune response to cancer and mediating immune surveillance. Moreover, these cells play a crucial role in antitumor immunotherapy, including monoclonal antibodies (mAbs), bispecific T cell engagers (BiTe), as well as adoptive transfer of chimeric antigen receptor (CAR)-modified cytotoxic cells.

The concept and design of the CAR, though originally intended to optimize targeting of innate natural killer cells (NK cells) to tumor-specific antigens, was laterally applied in the past decade to conventional autologous and later third-party allogeneic T cells with first-in-kind successes in leukemia immunotherapy in both adults and children. This approach also spawned the development of an entire industry focused on cancer immunotherapy, laying the foundation for the current paradigm shifts away from toxic chemo/radiotherapy to immune targeting. Aside from cytokine-release toxicities, specific obstacles plaguing existing CAR-T cell therapies include suboptimal killing by fatigued or senescent autologous patient-derived T cells derived, acquisition of tumor resistance to T-cell based immune therapies, and graft-versus-host disease (GVHD) associated with allogeneic conventional T cell therapies.

In addition to emerging understanding of tumor evasion, new immune targets such as CTLA4 blockade, NK stimulatory receptors, manipulation of the antigen processing and presentation, cytokine and costimulatory responses all provide new possibilities for enhancing anti-tumor immunity even in tumors previously felt to be resistant to immune attack. Several of these strategies have already been realized in the clinic. However, increasing evidence suggests that tumor-infiltrating immune cells may have a dual function: inhibiting or promoting tumor growth and progression. Although regulatory T (Treg) cells induce immune tolerance by suppressing host immune responses against self- or non self-antigens, thus playing critical roles in preventing autoimmune diseases, they might inhibit antitumor immunity and promote tumor growth. Recent studies demonstrate that elevated proportions of Treg cells are present in various types of cancers and suppress antitumor immunity. Furthermore, tumor-specific Treg cells can inhibit immune responses only when they are exposed to antigens presented by tumor cells. Therefore, Treg cells at tumor sites have detrimental effects on immunotherapy directed to cancer.

Immune checkpoint inhibitors in particular have generated enormous excitement across the entire field of oncology, providing a significant benefit to a minority of patients.

Book excerpt: Epithelial ovarian cancer (EOC) is the most lethal gynecological disorder due to a lack of effective early detection strategies. Worldwide, approximately 230,000 women are diagnosed annually, whereas 150,000 die. It represents the seventh most commonly diagnosed cancer among women in the world with 5-year survival rate of 46%. More than one-fifth of EOC have been related to hereditary conditions. Considerable efforts have been made to implement screening of the general population to diagnose EOC early; nevertheless, this has been ineffective and there is no approved strategy. Nowadays, new approaches for early diagnosis and prevention based on molecular genomics are in development. Whole genome sequencing has established the potency of the somatic genome, characterised with diverse DNA repair deficiencies that can be used to stratify EOCs into distinct biological groups with predictive signatures of resistance or relapse. The incorporation of next-generation sequencing (NGS) into clinical practice remains challenging for two reasons. Firstly, the EOC risk is not clear for some of the included genes and secondly, the variant of uncertain significance rates increase as more genes are analyzed. Finally, beyond germline pathogenic variants, somatic mutations may also affect therapeutic choices, and as such upfront tumor sequencing may be equally important to NGS, particularly as we continue to challenge treatment paradigms in the first‐line management of EOC.
