Book excerpt: Excerpt from Annual of the Universal Medical Sciences and Analytical Index, Vol. 3: A Yearly Report of the Progress of the General Sanitary Sciences Throughout the World IN a discussion before the French Surgical Congress, October 26, 1895, Doyen mill, attributed the comparative rarity of surgical intervention in diseases of the brain to a want of certainty in cerebral localization and to defective operative procedures, which permit of exploring but a small portion of the surface of the brain. In order to judge of the extent of the lesions present in a certain case it is necessary to be able to completely explore the surface of the lobe. This he has successfully done several times. He makes a large flap from the occipital protuberance to the glabella, pro longing the incision laterally in front toward the zygomatic arch and backward toward the external auditory canal. By means of special burrs, worked by hand or by electric motors, he makes five or six incisions into the bone outside the superior longitudinal sinus, above the lateral sinus, and below the temporal fossa. He separates the bony septa obliquely so as to prevent depression of the bone when replaced in position. This method renders cere bral localization useless to the surgeon, it being only necessary to have some indications as to the side to be operated upon. The author has in certain cases been able to operate on both sides at one sitting; He regards the procedure as applicable to numerous diseases, and particularly to microcephalus and epilepsy.
