Book excerpt: The increased provision of facilities for neonatal and paediatric care in the last 25 years has been accompanied only in part by appropriate developments in pathology. Specialist pathologists are many fewer than paediatric departments, and details of the advances in knowledge of the pathogenesis of diseases in childhood and of ways of investigating them are not uniformly available. In many institutions an individual with a special interest rather than a special training will be responsible for paediatric pathology and it is to this group of histopathologists that this text is addressed. For this reason it is not written as a comprehensive text and is not intended for use as a reference volume. Areas which may produce particular difficulties for the individual with little specialist knowledge of the very young (e. g. , the lung) are dealt with in more detail and, in general, entities in which the histopathology does not differ greatly from that of the adult disease are considered briefly. A brief account of developmental processes is included where prenatal considerations are helpful in understanding a particular entity. A number of specialist topics which are known to trouble those who work in nonspecialist departments are described fully (diseases of muscle) or with guides to investigation (metabolic disease). Book excerpt: The incidence of insulin-dependent diabetes mellitus (100M) varies dramatically across racial groups and countries, with annual age-adjusted rates of approximately 40/100,000 per year in Finland, but only 0.51100,000 per year in China. Although reasons for these marked geographic differences are unknown, it is likely that genetic variations across populations play a m~or role. To determine the contribution of genetic factors to the global patterns of 100M incidence, international comparative studies are now being undertaken as part of the WHO Multinational Project for Childhood Oiabetes, known as the DIAMOND Project. It is, therefore, necessary to develop and implement epidemiologic standards for these investigations which can be applied across populations. This will ensure that comparable data are obtained in all countries, and that relevant scientific questions can be properly addressed. The development of standards for molecular epidemiologic studies of 100M is the of the NATO Advanced Research Workshop. During this meeting at the objective University of Pittsburgh, scientists from across the world convened to discuss issues relating to the standardization of: 1. the collection of family history data to assess the risk of 100M in first degree relatives, 2. case-control molecular epidemiology studies of 100M susceptibility, 3. HLA family studies, 4. laboratory methods and ONA technology transfer for genetic marker evaluations.

These severe physical abnormalities which are present at birth and affecting every part of the body happen more often than usually realized, once in every 33 births. The most common, after heart defects, are those of the neural tube (the brain and spinal cord) which happen in as many as one in every 350 births. They have been noted as curiousities in man and beast throughout recorded history and received great attention in our time by various fields of study, for example, their faulty prenatal development by embryologists, familial patterns by geneticists, causation by environmentalists and variability by population scientists. Attention turned much in recent years to the relation of these malformations to deficiency of a particular dietary ingredient, folic acid, a subject this book analyzes in depth. The greatest conundrum of all, which this latest matter like so much else hinges on, is the amazing fact of the tremendous, almost universal decrease in the frequency of these anomalies since early in the 20th century.
