Book excerpt: Over the last decades, oropharyngeal cancer treatment has changed due to the introduction of new surgical techniques such as transoral approaches. These new techniques provide oncological results that are comparable to previous treatment but with shorter hospital stays and the preservation of functions. This surgery makes necessary to understand the anatomy of the upper aerodigestive pathway from the inside-out, as opposed to the inverse, which is the way head and neck surgeons have classically understood it. Although descriptions of the anatomy from medial to lateral have been published, a clear anatomic stratification of this area should help in the planning and execution of this surgery. This book is the basic guide for an anatomical course with the purpose to identify anatomic landmarks in the oropharynx and base of the tongue. This can help surgeons identify neurovascular elements of the surgical field during transoral robotic surgery quickly and accurately.

Spinal pathology is often located anterior to the spinal cord and nerve roots in the cervical and thoracic spine, and anterior to the peripheral nerves that emerge from the lumbosacral spine below the first lumbar ver tebra. To treat such pathology one prefers to expose the front of the spine directly and widely enough to eradicate the pathology and to have full control of bleeding throughout the procedure. The posterior elements of the spine are important for mechanical stability of the spine, and therefore for the protection of the neural and vascular structures in the spine that would be threatened by instability. Extensive eradication of pathology posterior to the spinal canal and the intervertebral foraminae, including the transverse processes, may leave no adequate bony bed for the surgical creation of a stabilizing osseous fusion. In such a situation, an anterior fusion procedure is the only viable alternative to a posterior or posterolateral fusion. In situations where it is critically important to obtain a stable fusion, as in tuberculosis of the spine, both an anterior and a posterior fusion operation at the same motion segments is, in almost every instance, a guarantee of a stable osseous fusion. One should know both approaches.

It discusses anterior midline (transoral transnasal), posterior (CVJ craniectomy laminectomy, laminotomy, instrumentation and fusion), posterolateral (far lateral) and anterolateral (extreme lateral) approaches using state-of-the-art supporting tools. It especially highlights open surgery, microsurgical techniques, neuronavigation, the O-arm system, intraoperative MR, neuromonitoring and endoscopy. Endoscopy represents a useful complement to the standard microsurgical approach to the anterior CVJ: it can be used transnasally, transorally and transcervically; and it provides information for better decompression without the need for soft palate splitting, hard palate resection, or extended maxillotomy. While neuronavigation allows improved orientation in the surgical field, intraoperative fluoroscopy helps to recognize residual compression. Under normal anatomic conditions, there are virtually no surgical limitations to endoscopically assisted CVJ and this issue provides valuable information for the new generation of surgeons involved in this complex and challenging field of neurosurgery.
