Book excerpt: Infant Feeding is about a controversy which fascinated the medical and scientific world, as well as national and international health authorities, politicians, religious groups and consumer organisations, for more than 11 years. It often disturbed public opinion, being concerned, as it is, with nothing less than the life and death of babies. The infant food industry was directly accused of having caused a decline in breast-feeding through the inappropriate marketing of breast milk substitutes. The problem was said to be particularly acute in poor under-developed communities, because illiterate mothers were unable to understand instructions for its use, water was often contaminated and, in order to "stretch" an admittedly expensive product, it was over-diluted. The inevitable result, said the critics of industry, was malnutrition, gastroenteritis and increased infant mortality. These were very serious charges against companies which had until then been generally considered to provide an important contribution to medical progress and child health. One company was to be particularly singled out: Nestlé SA, the Swiss multinational. Perhaps it became the target because it was the longest establishment, and served well as a symbol of the whole industry. It is a story which is full of confrontations, intrigue and passionately-held opinions, based, nevertheless, on a sizeable body of medical science. After countless twists and turns, it has some sort of "happy ending". Yet a great deal remains to be said, as will be seen throughout the book.

The report's objectives were to identify the social, economic, and political aspects of the controversy and to review the series of events that led Nestle to its current policy on infant formula marketing. The report examines 1) the origin of the controversy, 2) critical issues raised, 3) techniques used by Nestle's critics, and 4) Nestle's response to the issues and the public debate, in some detail. The resolution of the controversy, initiated by the adoption of an International Code of Marketing of Breast Milk Substitutes by the World Health Organization (WHO) in 1981, is described. Gradual acceptance of Nestle's compliance with the WHO code came about largely as a result of the findings of the Nestle Infant Formula Audit Commission and a "Statement of Understanding" in which Nestle outlined methods of dealing with remaining areas of concern. The end of the Nestle boycott was formally announced in October, 1984. The report emphasizes the complexities surrounding the development and resolution of the controversy. It concluded by stressing, the importance of commitment of financial and human resources by industry, government, health, and religious organizations to eventually improve infant health and nutrition in the Third World. Reference and biographic notes are included.

Book excerpt: Intensive and controversial marketing of infant formula is believed to be responsible for millions of infant deaths in low and middle-income countries (LMICs), yet to date there have been no rigorous analyses that quantify these effects. To estimate the impact of infant formula on infant mortality, we pair country-specific data from the annual corporate reports of Nestlé, the largest producer of infant formula, with a sample of 2.48 million births in 46 LMICs from 1970-2011. Our key finding is that the availability of formula increased infant mortality by 9.4 per 1000 births, 95%CI [3.6, 15.6] among mothers without access to clean water, suggesting that unclean water acted as a vector for the transmission of water-borne pathogens to infants. We estimate that the availability of formula in LIMCs resulted in approximately 66,000 infant deaths in 1981 at the peak of the infant formula controversy.
