La politique sanitaire d'aujourd'hui est encore trop centrée sur le curatif. Le défi majeur est de faire de la prévention une exigence première de santé publique. Les démarches préventives n'ont pas de limite d'âge. Ce sont en l'occurrence les enfants qui seront nos premiers acteurs de la santé. A partir de données et de résultats statistiques sur l'hygiène de vie et la situation familiale de 130 enfants, nous allons mettre en évidence les besoins actuels de la population en matière de prévention. Notre étude a été réalisée lors de consultations en odontologie pédiatrique au CHRU de Lille, sur une période de 6 mois, entre janvier et juin 2015. Le but de cette étude est de promouvoir le développement d'un programme d'éducation thérapeutique, basé sur l'implication de chaque individu à la gestion de sa santé. Dans le milieu de la dentisterie, ce programme récent serait en faveur d'une amélioration de la santé bucco-dentaire, de la santé générale et de surcroît d'un bien être de vie.

Elle a été réalisée à partir d'un examen clinique de l'enfant et d'un questionnaire aux parents afin de permettre d'établir un profil épidémiologique des patients.

Book excerpt: Cette thèse a pour but de proposer un protocole de prise en charge des enfants lors de leur première visite chez le chirurgien-dentiste en prenant en compte leur âge, leurs spécificités cliniques, et leurs besoins particuliers en matière de prévention. Une recherche systématique a été conduite par un seul investigateur sur la base de recherches PUBMED concernant les protocoles de prise en charge des enfants en Odontologie Pédiatrique. 3282 articles ont été recensés et plus exactement 2351 après suppression des doublons. Après une analyse des titres puis des résumés, 123 articles se rapportant au sujet ont été sélectionnés pour une lecture complète ; puis, en tenant compte des critères d'exclusion, 38 articles ont été inclus dans l'analyse qualitative. A partir des recommandations recueillies, nous avons créé 1 fiche d'anamnèse commune à tous les enfants et 2 fiches d'anamnèse complémentaires pour la petite enfance et l'adolescence. Le praticien pourra alors obtenir de précieuses informations qui l'aideront à objectiver les causes possibles d'une pathologie, faire un diagnostic et proposer une prise en charge adéquate. A cela s'ajoutent 4 fiches cliniques ainsi que 4 fiches de prévention caractéristiques aux âges suivants : 0-3 ans ; 3-6 ans ; 6-9 ans et enfin 9-15 ans. Ainsi, le praticien approfondira tous les items de l'examen clinique sans en oublier aucun, et pourra prodiguer des conseils préventifs à sa jeune patientèle.

Les soins dentaires sous anesthésie générale restent parfois la seule alternative possible aux soins ambulatoires. Notre étude pilote se propose d'évaluer l'état dentaire des jeunes patients soignés sous anesthésie générale (AG) par le service d'Odontologie Pédiatrique entre 2007 et 2010 au CHU de Clermont-Ferrand. Nous avons évalué la récidive carieuse et les changements apportés dans le mode de vie de l'enfant depuis son passage au bloc opératoire. Nous avons exploré par la même occasion, l'aptitude des enfants ayant grandi et évolué, à se faire soigner de manière conventionnelle, depuis leur première expérience de soins à l'état vigile. L'intérêt de cette étude serait, à long terme d'établir un programme de prévention personnalisé et de suivre de manière régulière l'évolution de la santé bucco-dentaire des jeunes patients ayant été pris en charge par AG en créant, au CSD du CHU de Clermont-Ferrand, une consultation à visée préventive exclusivement. Afin de développer ce projet, une collaboration pluridisciplinaire entre les services d'Odontologie Pédiatrique, de Soins Spécifiques, d'Epidémiologie et d'ODF serait nécessaire.

Book excerpt: In combating global warming and other environmental issues over the use of fossil fuels, extensive research has been focusing on developing hydrogen production from biological processes. Biohydrogen is considered a promising future biofuel because of its intrinsic clean and high-energy content properties and the way it is produced. In addition to being produced through environmentally friendly biological means, its conversion to energy yields only pure water, which is an ideal energy carrier in reducing greenhouse gas emissions from fossil fuel combustion. Unlike other well-developed biofuels such as bioethanol and biodiesel, biohydrogen production is still in the early stage of development. A variety of technologies are being developed for biohydrogen production. This chapter presents a review of the state-of-the-art and perspectives of bioprocess design for biohydrogen production research in the context of pathways, microorganisms, metabolic flux analysis, process design, and reactor system. Challenges and prospects of biohydrogen production are also outlined.
