The study was a quasi-experimental, pre-interventionlpost-intervention, nonequivalent group, controlled pilot designed to measure changes in oral health status and quality of life in residents following oral health interventions with Certified Nursing Assistants (CNAs) providing residents with Activities of Daily Living (ADL). A licensed dental hygienist delivered educational and onsite support to caregivers in Facility A for 2 months. Facility B received education alone, and Facility C served as the control. The Oral Health Assessment Tool (OHAT) and the Geriatric Oral Health Assessment Index (GOHAT) were utilized as measurement tools. Although limited by a small sample, findings suggest dental hygiene champions working onsite in LTC/NH facilities have potential to improve oral health status and self-perceived quality of life in LTCINH residents.

The consequences of these disparities in access to oral health care can lead to a number of conditions including malnutrition, childhood speech problems, infections, diabetes, heart disease, and premature births. Improving Access to Oral Health Care for Vulnerable and Underserved Populations examines the scope and consequences of inadequate access to oral health services in the United States and recommends ways to combat the economic, structural, geographic, and cultural factors that prevent access to regular, quality care. Book excerpt: Purpose: The enigma of poor oral health of residents in long-term care (LTC) homes remains unsolved despite many years of research and intervention. The purpose of this study was to explore why resident oral health in LTC homes remains poor despite many years of research on causes of the problem and interventions to address it. Exploring how institutional processes and social relations influence care providers' experiences around providing oral care has the potential to inform strategies to address the longstanding issue of poor oral health of residents in LTC. Methods: I investigated how oral care happens and why it happens the way it does, using institutional ethnography from the standpoint of the healthcare aide (HCA). The social organization of oral care in the LTC homes was explored through the observation of the HCAs' day to day practices and their interaction with institutional texts. Data were collected at two LTC homes through 96 hours of naturalistic observation, 21 in-depth interviews, and document review. As part of the data analysis, and based on what I was observing, hearing in interviews, and reading in texts, I created text-work-text (TWT) mappings to illustrate how oral care was socially organized in LTC homes. In addition, I created an additional TWT map for bowel care, as a comparison to oral care. Findings: Health care aides were primarily responsible for providing oral care to residents in LTC homes, despite having very little textual guidance as to what, when or how care was to be provided. Texts that were available to inform them in providing oral care were underutilized because the HCAs did not consider the texts helpful. The HCAs used their own discretion, primarily relying on their personal experiences, to inform the oral care they provided for their residents. In contrast, bowel care was an effectively textually mediated process and HCAs relied on several texts to provide them with guidance. Implications: Exploring the social organization of work around oral care in LTC homes has illustrated institutional processes that may contribute to poor oral health for residents in LTC homes. Addressing these processes could help existing efforts of those seeking to improve the state of oral health of residents in LTC. Processes may be improved through a multifactorial approach including revision of texts that guide care, better oral health education for care providers, access to dental professionals in LTC settings, and policy changes at national and provincial levels around oral healthcare in LTC.

Book excerpt: Background: Seniors who are dependent on others to brush their teeth, communicate their dental needs, and provide them access to professional oral health care define dentally dependent nursing home residents in the context of this dissertation. Oral problems in Canadian nursing home (NH) residents demonstrate poor oral health states and inadequate oral health care programmes. A novel gold standard on-site professional oral health care (POHC) programme and its components showing clinical effectiveness and cost-effectiveness outcomes to reduce dental disease does not exist and is required to support policy recommendations. Purpose: This Systematic Review (SR) evaluates the existing evidence on the clinical effectiveness and cost-effectiveness - inclusive of gold standard on-site POHC programme components - for the safety, efficacy, and effectiveness of reducing dental disease when compared to usual care - no on-site POHC or intervention. Methods: The review is based on 13 clinical effectiveness studies. The clinical studies include nine randomized controlled trials (RCT) based out of a total of forty NHs (3 x one, two, three, four, eight, nine, and 11 NHs), two non-RCTs based out of three NHs (one and two), and cross-sectional studies based out of three NHs (one and two). The cost-effectiveness studies include two economic analysis studies based solely out of NHs with the comparator of usual care - no on-site POHC or intervention. Findings and conclusions: Oral health measures were improved showing better oral health outcomes in the experimental group who received the on-site POHC intervention. On-site POHC services once per week by Registered Dental Hygienists (RDH), twice daily oral hygiene care by nursing assistants (NA), and monthly education, improves NH resident safety by reducing preventable bacterial mouth infection, pneumonia, and fatal aspiration pneumonia (AP). In addition, on-site POHC appears to provide motivation, remotivation, staff support, and staff evaluation in NHs which improves guidelines and standards and implements new routines using a contact person for oral care. The policy makers need to consider policy change that provides funding according to the "level of need" to ensure all dependent NH residents receive access to essential oral health care. Successful implementation includes funding the frequency and duration of POHC, implementing and evaluating NH guidelines and standards for daily mouth care and professional oral health care (POHC), using an POHC program to measure nursing staff oral health work in relation to defined standards, and organizing long-term presence and guidance by dental hygienists. Some limitations across the included studies were small sample size, no control group, or not able to provide randomization. Economic analysis found the potential value of access to on-site POHC programme, compared to usual care, weighs heavily on reducing transportation and hospitalization costs and the price point set by the programme planner. On-site POHC is a novel oral health system delivery program that provides on-site access to clinically effective, and likely cost-effective, sustainable, and high-quality care to seniors who reside in Alberta NHs. Future studies are needed to determine the cost-effectiveness of full-time RDHs providing these essential POHC program components using a "level of need" approach for all dependent NH residents.
