Book excerpt: Excerpt from An Inquiry Into the Principles of Treatment of Broken Limbs: A Philosophico-Surgical Essay, With Surgical Notes Soon after the author had invented the method of rapidly immobilizing broken bones by the use of perforated narrow tin strips, he explained the procedure with the aid of photographic illustrations, to a disguised physician from a distant city. His response was: "What does Professor Blank [naming an eminent surgeon] say of the merit of the method?" I then realized that it was not enough to devise an improvement of existing procedure but that it was also important to prove its value independent of the dictum of personal authority. In the following pages I have endeavored to substitute impersonal proof of values in place of the pronouncements of personal authority or the expressions of consensus of opinion. The furthest reach of personal authority, often derived from adventitious circumstance, is to cast a presumption in favor of or against the correctness of a stated conclusion. Some years ago in the discussion of a paper read before the Academy of Medicine on the non-operative treatment of recent simple fractures of the patella the writer was one of a small minority speaking in favor of the open method of suturing. The eminent author of the paper of the evening in closing the debate, declared he would be governed in his action by the practice of the most distinguished surgeons throughout the world and the consensus of opinion as expressed in surgical societies. The text provides a theoretical framework for both evaluating published trauma studies and designing new ones. Experimental trauma research is an area ripe for research, and criteria to consider in choosing which non-human species to use in an actualistic study are offered. Common circumstances in which blunt force trauma is encountered are described. Information is provided on a variety of causes of death due to blunt force trauma. These causes range from accidental deaths to homicides due to blunt force from motor vehicle accidents, falls, strangulation, child and elder abuse, among others. Epidemiological information on whom is most likely affected by these various kinds of blunt force trauma is drawn from both the clinical and forensic literature. Case studies are included to bring the book full circle back to considering the micro and macro bone changes that are seen when bone fails and fractures. The case studies are illustrative both of the concepts described through the book and of the high quality analyses forensic anthropologists contribute to medicolegal investigations of death every day.
