Book excerpt: In collaboration with the Upstate New York Substance Abuse Program (pseudonym), the researchers developed a survey, the Purposeful Treatment Client Satisfaction Survey (PTCSS), and completed a pilot study to determine the effectiveness of its use. The intent was to assist this agency to assess client satisfaction with the services they received as part of an overall program evaluation. As part of the pilot study, the survey was given to both day rehabilitation and outpatient populations at The Upstate New York Substance Abuse Program. The data was compiled and analyzed and modifications were made to the tool. The results of the pilot study reflected that 93.82% of respondents provided positive feedback regarding the effectiveness of the PTCSS: 1) 93.8% stated that the survey provided good feedback regarding the quality of services received; 2) 93.8% stated they were provided the opportunity to share good feedback regarding their facilitators and counselors; 3) 100% stated they were given the opportunity to voice their opinion about their experience; 4) 91.5% stated that the survey did not make them feel uncomfortable in any way; and 5) 90% stated they were able to recall good things they did for themselves while attending the program. At the conclusion of the pilot study, the researchers made slight modifications to the PTCSS. These changes included a change in the Likert scale and the addition of questions to compile demographic and program information. Following those changes, the researchers have provided this survey for The Upstate New York Substance Abuse Program to use as part of their program evaluation, which can then positively influence ongoing program development and improvement.

Book excerpt: Predictors of successful discharge in a residential treatment program for the mentally ill homeless were evaluated. The charts of all program residents during a single year were reviewed (N = 161). Data were collected on demographic, clinical and intake variables. Severity of Psychiatric Illness (SPI) and Acuity of Psychiatric Illness (API) questionnaires were retrospectively completed for each admission. Outcome variables included Planned Discharge, Monetary Linkage, Housing Linkage, Psychological Management, Psychological Stability and Global Functioning. A logistic regression (alpha level =.05) was performed. Across outcome variables, strong positive predictors included: the client's length of stay in the program; and use of external case management. Strong negative predictors included: dual diagnosis status; history of severe residential instability; and higher scores on the API-Nursing section (indicating greater need for nursing contact). Clients receiving individual psychotherapy were one-fifth as likely to have a planned discharge as were other clients. Clients receiving group psychotherapy were seventeen times as likely to demonstrate improved global functioning as were other clients. Findings were largely consistent with the existing empirical and theoretical literature on care for the mentally ill homeless. The strong impact of the "length of stay" variable suggests that residential treatment can be of great use to homeless clients. Indicated is the need for residential treatment programs that will keep clients engaged for longer periods of time.

The lack of empirical research published for this specified population is the reason why this project was established. This population was overlooked based on society's ignorant views that individuals with developmental disabilities are not using or abusing substance. There were two purposes for this research project. The first purpose of the researcher project was to bring awareness and knowledge to the Social Work community about the lack of research in hopes for further studies on persons with developmental disabilities and their use and abuse of substance. The second purpose of this research project was to create a best practice model for treatment facilities/programs that incorporate the special needs of co-occurring disorders. There is lack of empirical data for facilities/programs that serve persons with developmental disabilities and substance abuse. The main goal of this project is to describe current available treatment facilities, as well as focus on developing a best practice for treatment facilities working with this population. The research design used for this project was the Exploratory Research Design. Data was collected through forensic literature from databases for secondary, empirical data through articles, books, journals and websites. The project also focused on a best practice model for treatment facilities of individuals with co-occurring disorders that include developmental disorder, serious mental illness and substance abuse. No human subjects were used in this research project. Analysis of the treatment facility was created by reviewing the established requirements constructed by the Substance Abuse and Mental Health Administration. This researcher established a best practice treatment facility that incorporates and met all 12 Principles of the Substance Abuse and Mental Health Administration guidelines. This treatment facility was used as standard for the best practice model outlined in this project. This researcher also used an already established treatment facility, Autus in Northern California as example of a treatment facility already established. There is a need for awareness of the lack of information, research on persons with developmental disabilities and treatment facilities. Finalization of this project helped bridge the gap between of lack of awareness and information. The forensic literature review in this project illuminates what the Substance Abuse and Mental Health Administration deems is a best treatment facility by proposing the structure of such a treatment facility.

Motivational approaches are based on the principles of person-centered counseling. Counselors' use of empathy, not authority and power, is key to enhancing clients' motivation to change. Clients are experts in their own recovery from SUDs. Counselors should engage them in collaborative partnerships. Ambivalence about change is normal. Resistance to change is an expression of ambivalence about change, not a client trait or characteristic. Confrontational approaches increase client resistance and discord in the counseling relationship. Motivational approaches explore ambivalence in a nonjudgmental and compassionate way.

Book excerpt: With reference to the research questions, several observations can be highlighted from the main study. First, socially and emotionally maladjusted children and adolescents were generally satisfied with the residential home services, although there were several areas deserving attention. Second, the levels of satisfaction in Specific Satisfaction domains were generally higher than the level of satisfaction in the Global Satisfaction domains. Third, there was a relationship between the Global Satisfaction and the satisfaction in the Specific domains. Fourth, satisfaction with the Specific dimensions of Leisure, Regime, Staff and Health were found to be significant predictors of Global Satisfaction. Fifth, contrary to our prediction, client's satisfaction was not significantly related to reasons of admission, length of stay and hostel with or without schools on site. The theoretical and practical implications of the findings are discussed. The study has several theoretical contributions. It is a pioneering scientific study of the Chinese concept of satisfaction amongst maladjusted children and adolescents in residential care. It clarifies the dimensionality of client satisfaction that measures satisfaction amongst socially and emotionally maladjusted children and adolescents in residential care in Hong Kong using the CSSRS-CASEM. Moreover, the study contributes to model building of residential client satisfaction, where four satisfaction predictors have been identified. The practical implications of the project are also discussed.

However, the progress clients make in treatment frequently is undermined by the lack of an alcohol and drug free living environment supporting sustained recovery. This book suggests that treatment providers have not paid sufficient attention to the social environments where clients live after residential treatment or while attending outpatient programs. It also describes the need for alcohol and drug free living environments. We then review the history of communal living for recovering addicts and alcoholics and provide concrete examples of the Oxford House model, which is a widespread communal living option for over 10,000 recovering persons in the US. The structure and philosophy of Oxford Houses are presented along with recent outcome studies providing support for their effectiveness. This book was published as a special issue in the Journal of Groups in Addiction and Recovery.

At the same time, clients often prematurely drop out of substance abuse treatment, negatively impacting their chances of achieving favorable outcomes. Investigating variables associated with treatment retention has become increasingly important considering one of the most robust findings in substance abuse treatment outcome research is the positive relationship between the amount of time spent in treatment and post-treatment outcomes (e.g., decreased drug/alcohol use, decreased criminal activity, improved social functioning). This study examined the relationship between pre-treatment client characteristics and treatment drop-out among 273 adults who were admitted to intensive outpatient substance abuse treatment. An intake assessment battery was administered to all participants in an effort to gain a broad understanding of client attributes at the point of treatment entry. A series of regression analyses were used to investigate if client characteristics could help predict treatment completion status, time to drop-out, and number of treatment sessions attended. Results indicate that age and meeting criteria for an anxiety disorder were statistically significant predictors in all three regression analyses. Meeting criteria for a cocaine disorder was found to be a statistically significant predictor of treatment completion status and time to drop-out. Finally, number of years using alcohol regularly was found to be a statistically significant predictor of the number of treatment sessions attended. The clinical implications of these findings are discussed and recommendations to help improve client retention in the substance abuse treatment program utilized for this study are provided.
