Resistance by taxpayers to the increasing burden of Medicare and Medicaid, and pressures to restrain medical costs have encouraged new government controls in the health sector. This paper investigates the costs and benefits to the citizens of Georgia of a Georgia policy decision to require all skilled nursing facilities participating in Medicaid (which is state and federally funded) also to participate in Medicare (which is federally funded). Benefits to the state result primarily from the transfer of health service costs from Medicaid to Medicare. The costs to society result from a reduction in the number of skilled nursing home beds, and an increase in the net cost of providing the same service. The findings of this study are that the social costs of a) reduced availability of skilled nursing home beds, and b) increased health care expenditures, outweigh the benefits of state tax dollar savings.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: Objectives: Returning home and remaining in the community is preferred by many older adults who were admitted to skilled nursing facilities (SNFs). The Medicaid Home- and Community-Based Services (HCBS) provide services to Medicaid enrollees in the community, and may facilitate the transitions from SNFs to the community. The objective of this study was to understand the relationship between Medicaid HCBS generosity (breadth and intensity) and SNF-to-community discharge and post-discharge outcomes among Medicare-Medicaid dual eligible SNF patients.Study Design: This study linked the national Medicare and Medicaid datasets for CY 2010-2013. For Specific Aim 1, the study population was dual SNF residents who were 65+ and were newly admitted to SNFs for post-acute care between 2010 and 2013. The key outcome was community discharge from SNF within 100 days. HCBS generosity, measured as breadth and intensity, was the main variable of interest. Linear probability models with SNF fixed effects and robust standard error were estimated, accounting for individual demographics and health status. For Specific Aim 2, the study cohort was those who were discharged from SNF to community. The outcomes were remains in the community, death, NH admission, and hospitalization, measured for both 30 days and 180 days post-discharge. The main variables of interest and the statistic models were similar to Aim 1.For Aim 3, the hypotheses under the first two aims were tested among dually eligible SNF residents with dementia, with population-specific measures of HCBS generosity. Results: Higher HCBS breadth was associated with higher likelihood of community discharge, remaining in the community after discharge, and lower likelihood of NH admission after SNF discharge. Higher HCBS intensity was associated with higher likelihood of remaining in the community and lower likelihood of NH admission and hospitalization. Findings from the subgroup analysis among duals with dementia were similar to the main analysis.Conclusion: This study found HCBS generosity is associated with higher likelihood of community discharge from SNFs and better post-discharge outcomes with a modest effect. Future studies are needed to examine the effect of different types of HCBS and the effect of HCBS among Medicaid managed care enrollees.
