Book excerpt: The relations between depressive symptoms and alcohol use among maltreated youth have been well documented. Two contrasting theories about the mechanisms underlying these relations have received extensive support: the self-medication and impaired-functioning hypotheses. The present study proposed to reconcile these theoretical and empirical inconsistencies using a developmental approach. Specifically, an integrative conceptualization, the bidirectional hypothesis, was advanced, which used developmental timing to explain the longitudinal associations between maltreated youth's alcohol use and depressive symptoms. In addition, this investigation examined the role of parental alcohol problems in the etiology of maltreated youth's alcohol use and depressive symptoms. Data of 657 youth were drawn from NSCAW II, a longitudinal, nationally representative sample of maltreated youth. Findings supported the impaired-functioning hypothesis during early-adolescence, and corroborated the self-medication hypothesis during mid- to late-adolescence, thus supporting the bidirectional hypothesis. Lastly, increased youth depressive symptoms mediated the link between parental alcohol problems and youth's alcohol use.

Based on evidentiary studies, major depression in either parent can interfere with parenting quality and increase the risk of children developing mental, behavioral and social problems. Depression in Parents, Parenting, and Children highlights disparities in the prevalence, identification, treatment, and prevention of parental depression among different sociodemographic populations. It also outlines strategies for effective intervention and identifies the need for a more interdisciplinary approach that takes biological, psychological, behavioral, interpersonal, and social contexts into consideration. A major challenge to the effective management of parental depression is developing a treatment and prevention strategy that can be introduced within a two-generation framework, conducive for parents and their children. Thus far, both the federal and state response to the problem has been fragmented, poorly funded, and lacking proper oversight. This study examines options for widespread implementation of best practices as well as strategies that can be effective in diverse service settings for diverse populations of children and their families. The delivery of adequate screening and successful detection and treatment of a depressive illness and prevention of its effects on parenting and the health of children is a formidable challenge to modern health care systems. This study offers seven solid recommendations designed to increase awareness about and remove barriers to care for both the depressed adult and prevention of effects in the child. The report will be of particular interest to federal health officers, mental and behavioral health providers in diverse parts of health care delivery systems, health policy staff, state legislators, and the general public.

Alcohol is the most widely used drug by adolescents. For some, this may be one or two isolated occasions of youthful experimentation; for others, the use becomes excessive, placing them in danger of immediate adverse consequences such as accidental injury and alcohol poisoning, or encouraging other high-risk behavior patterns including unprotected sex. Moreover, a pattern of heavy drinking established in adolescence and young adulthood may continue into an adult pattern of alcohol abuse. Concerned communities and institutions across the nation are tackling the problem of alcohol use and abuse by young people. Research-based knowledge is urgently needed to inform these efforts and to ensure that limited prevention resources are used as effectively as possible. The origins of youthful alcohol use and abuse are found within the complex interplay of individual characteristics, family and peer influences, the larger societal context for alcohol use, environmental conditions, and maturational processes that accompany adolescence. This volume, which began as a special issue of the Journal of Research on Adolescence, contains all of the material from the journal issue plus additional chapters. It helps researchers to meet the tremendous challenge of disentangling the key determinants of risk, and developing effective interventions. Primary sources of influence on youthful alcohol use are described, ranging from individual expectancies about alcohol effects and cognitive decision processes to parenting practices, peer influences, social environments, and economic factors; and a corresponding range of prevention interventions is discussed. Book excerpt: The link between anxiety disorders and alcohol abuse and dependence has been established in adults, but the relationship between anxiety and adolescent alcohol involvement is less clear. In this dissertation, we compared explanatory models of substance use including the self-medication hypothesis, the social learning model, and the rebound hypothesis to characterize the association between anxiety and alcohol use in groups with various levels of experience with drinking. Additionally, we included depression symptoms in our studies to assess whether the relationships we found between anxiety and alcohol were unique or similar to those found with other forms of negative affect. In chapter 1, we investigated the relationships between several types of social anxiety symptoms and the onset of alcohol use in middle school students. We found that social anxiety significantly predicted initiation of drinking at both extremely high and low levels, and sex differences were observed. Chapter 2 follows with an analysis of the impact of social anxiety, depression, and alcohol expectancies on youths' drinking. We found that the self-medication hypothesis characterized the relationship between depression symptoms and drinking behavior, while the relationship between social anxiety and alcohol use was explained by social learning theory. Chapters 3 and 4 examined alcohol and drug use relapse characteristics in an adolescent substance use disordered treatment sample. In chapter 3 we found that compared to youth with only a substance use disorder, comorbid youth relapsed more often and more rapidly after treatment. In chapter 4, anxiety symptoms were more likely reported to improve than worsen immediately after relapse and in the two weeks following the relapse episode, indicating support for the self-medication hypothesis, while depression symptoms worsened in youth who relapsed with stimulants, which is consistent with a rebound effect. Chapter 5 is a replication of the study in chapter 4 with adults post-treatment for substance use disorders, with and without comorbid psychopathology. Unlike in the adolescent sample, almost no adults reported improvements in any psychiatric symptoms following relapse, however depression symptoms were rated as worsening more than anxiety symptoms.
