Book excerpt: Alcohol abuse ranks among the most common and also the most severe environmental hazards to human health. Its significance is heightened by the possibility of prevention by elimination of the habit, however, rarely exerted. The incidence of deleterious effects on human health has relentlessly risen in the past years for a variety of factors. They include migration of populations and, particularly, increased urbanization. Thus, in some parts of the world, population groups previously spared have become involved, which is also re flected in the increasing number of breweries and distilleries in the developing countries. Social, religious, and gender-related barriers to alcohol consumption are loosening, and the financial improvement of some segments of populations now enable them to buy alcoholic beverages. Thus the greatest percentage rise in the United States has recently been in black women. Adolescents and young people drink more alcoholic beverages than ever, and growing alcohol abuse by pregnant women has let to an increase of the incidence of the fetal alcohol syndrome. While the social and behavioral, including psychiatric, consequences of alcoholism are staggering, the gastrointestinal and, particularly, hepatic manifestations are the most widespread somatic effects, and chronic hepatic disease in alcoholics appears to cause the greatest cost to society. Indeed, mortality from liver cirrhosis is considered a reliable index of alcohol consumption in a country.

Canadian Hepatic Foundation, many of the world's experts gathered in Toronto, May 14-15 1976, to focus attention on one of the most important aspects of the alcohol problem -alcohol induced liver damage. The epidemiology of alcohol induced liver disease was discussed and current views on the pathogenesis of the problem were reviewed. New insight into the pathological alterations of the liver was presented and some of our current therapeutic capabilities were discussed. Dr. Hans Popper summarized the Symposium and presented some of his views on those aspects of the problem which will require early attention by the research community. The Symposium achieved its immediate objective -that of bringing together the committed experts of various disciplines for an updating of our understanding of alcohol and the liver and for a discussion of new approaches to the problem. As a backdrop to the Symposium, however, was large writing on the wall to the effect that we are expending our research talents and efforts on a totally unnecessary problem. Right now we probably know enough and have sufficient resources at our disposal to solve the problem.

A publication devoted to alcohol research should serve the broadest existing research community, but perhaps more important, it should also provide a means to recruit new investigators with fresh approaches to the field. We can and must demonstrate that legitimate, high-quality research is being done, but we must also highlight the opportunity for new workers to make a real impact on the problem. Medications and alcohol may affect nutritional status. Section 2 provides a discussion of nutrition-related diseases which occur more frequently among the elderly. Cardiovascular diseases including coronary heart disease and cerebrovascular disease are the leading causes of death in the U.S. The relative importance of cardiovascular diseases, in terms of all deaths for the given age group, rises steadily with age. The death rate from these diseases is 28% for the middle age group (35 to 44) and is 69% for the old age group (age 75 and above. This reflects the continued progression of artherosclerosis with aging. Cancer is the second leading cause of death in the U.S. The death rate from cancer also rises steadily with age. The death rate from diabetes mellitus increases progressively with age and more rapidly after the age of 45. The incidence of diabetes mellitus is 0.23% under age 25 and 6.2% over 45.

Book excerpt: The papers in this book represent the proceedings of the Third International Symposium, which was held at the Addiction Research Foundation in Toronto in July, 1979. The purpose of this meeting was to bring together experts in the field from a wide variety of backgrounds in an attempt to gain some clarity and insight into the problems of alcohol and aldehyde metabolism. One might ask, "Why have such a meeting and a collection of research papers?" The answer is clear. The societal problem of alcoholism is, unlike many other health problems, growing instead of abating. Treatment efforts are largely ineffective (e.g., Griffith Edwards' classic work)', and fundamental research has yet to identify rational therapy based on sound mechanisms for this disease which effects 5% of the population severely and a much greater percentage to a lesser degree or indirectly. I have become impressed with the possibility that this lack of progress may be-in large part-due to a fundamental oversight on the part of investigators in the field of alcohol research. Simply, this possible problem could be stated as follows: an alcoholic is defined the same way as all other alcoholics. Attempts to identify possible subpopulations of alcoholics with defined and treatable diseases have been minimal. One could argue, however, that little evidence, with the exception of the gross psychiatric definitions (e.g., Jellinek) exists that there are different types of alcoholics. This argument is countered, first, by the dismal lack of progress made in this field by classifying an alcoholic equal to other alcoholics, and second, by analogy with cancer research. Now we know that "cancer" is only a generic term which defines a broad group of diseases, some caused by specific environmental chemicals, others by viruses, etc. Progress was only possible after suitable animal models were developed. More importantly, however, is the fact that once specific forms of the disease (i.e., once the diseases themselves were separated from the generic term) were identified, it was a relatively easy task to develop tests for early identification of some forms of the disease (e.g., the pap smear, etc.). This analogy can be extended to alcoholism's. First, let us assume that we have failed to develop tests for early identification of alcoholisms because the population is not carefully defined into subgroups. If we assume that subpopulations of alcoholics exist, we can again turn to the cancer literature for examples of how to proceed. Much progress in this field rests. On the careful development of strains of animals which differ in their susceptibility to certain carcinogens. By analogy, distinct forms of alcoholisms could be identified by the use of genetics (e.g., Goodwin). First, distinct phenotypes need to be developed for certain characteristics suspected to be important in alcoholism. Second, a test specific for early identification of this specific phenotype can then be developed. Third, longitudinal studies must be performed in humans to see if the animal work can be applied to man. Implicit in this rationale is the thought that progress is not possible by studying alcoholics 20 years after primary changes had occurred, nor even by studying pre-alcoholics until specific sub forms of the disease are identified. Thus, the need for fundamental research into the mechanisms responsible for alcoholism is required. Unfortunately, alcohol research appears to suffer some of the stigma of the alcoholic

In thinking about what to write in this preface, it occurred to me that it might be best to deal with that with which I am most familiar and to trace to some extent my own '"road to Rome. '' As I look over my work of the last 37 years, it becomes clear to me that it can be epitomized as a search for patterns. What usually began as a single minded devotion to in-depth analysis of one or a small number of variables always has led to questions of how the results might relate to the whole living unit, whether it is cell, tissue, or organism. For a number of years after my discovery in the vertebrate central nervous system of -y-aminobutyric acid (GABA) and the enzyme which forms it, L glutamate decarboxylase (GAD), and the identification of GABA as a major inhibitory neurotransmitter by others, I felt that my laboratory, largely bio chemical, was wandering in the wilderness of the complexities of the vertebrate CNS without definitively coming to terms with problems related to GABAergic transmitter functions and the roles of GABA neurons in information processing.

Central nervous system pathology, associated with long-standing alcohol ingestion, has particularly deleterious consequences to the individual. Compro mising brain functional integrity ultimately militates against psychosocial ad justment, and this process is inevitably reflected as a substantial economic loss to society in the form of costs for providing medical and social services, as well as disability and absenteeism from work. This book marshals the literature pertinent to the effects of chronic alcohol abuse on brain structure and functioning. The material is divided into two parts: basic research and clinical issues. In the first section, the manifest neurologic consequences are described across the different levels of biologic organization, these being brain morphology, neurochemistry, neurophysiology, and neuro psychology. In recognition of the multifactorial etiology of alcohol-related brain pathology, the influence and role of hepatic, endocrine, and nutritional factors are also examined. The second section addresses clinical syndromes and dis orders. It will be noted that evidence accrued from recent research suggests that neurologic disturbances may actually antedate the onset of drinking in some alcoholics. Other clinically important issues discussed are the effects of alcohol on neurologic development, aging, and dementia. The book concludes with a discussion of the alcohol withdrawal syndrome, its mechanisms and manifes tations. A fundamental objective of the editors was to illustrate that the consequences of chronic alcohol excess can be comprehensively understood within the per spective of interrelated hierarchical systems of brain organization.
