Changes in the air ambulance (AA) industry's size and structure have led to differences of opinion about the implications for AA use, safety, and services. Some industry stakeholders believe that greater state regulation would be good for consumers. While states can regulate the medical aspects of AA, the Airline Deregulation Act preempts states from economic regulation of AA. Other stakeholders view the industry changes as having been beneficial to consumers and see no need for a regulatory change. This report examined: (1) changes in the industry in the last decade and the implications of these changes on the availability of AA and patient services; and (2) the relationship between fed. and state oversight and regulation of the industry.

Book excerpt: From the unique position of a decade in government service, I was given the opportunity to observe the changes in the provision of emer gency medical care across the country. In 1970, Emergency Medical Service (EMS) systems were a new and much needed development in the national health care delivery system. A systems approach to field casualty care has been progressively improved during each successive military conflict since the Civil War. These improvements were ini tiated after the rnedil:al care and evacuation disaster experienced by the Union Army of the Potomac at Bull Run on July 21, 1861. During the Civil War, major changes in administration, professional personnel, transportation, hospitals, sanitation, and medical records established patterns that have been continually refined and improved. Stimulated by the pressing demands of war surgery and coupled with parallel advances in medical care over the last century, an almost unbelievable level of performance was realized in Vietnam. Advances in field resuscitation, efficiency of aeromedical transportation, and energetic treatment of military casualties have proved to be major fac tors in the decrease in death rates of battle casualties reaching facilities: from 8% in World War I to 4. 5% in World War II to 2. 5% in Korea and to less than 2% in Vietnam.

The Dust Off and Medevac helicopter ambulance units tested and perfected for medical use the Army's new helicopler, the UH- I (" Huey" Iroquois), and developed several new devices that helped save thousands of American lives between 1962 and 1973, The pilots of these helicopters displayed courage and devotion to duty that earned widespread respect from soldiers. This book explains early problems of medevac in Vietnam, recounts the valor of crews, and describes the movement of patients to in-theater Army hospitals. It also shows the effect that helicopters had on traditonal procedures. The widespread use of the helicopter for medical evacuation in America since the Vietnam War testifies to the broader issues raised by this study, and of the relevance of Army history to the civilian community.
