A feminist lens is used, seeing social inequalities as mutually reinforcing and interdependent, to examine the interaction of activists and the outcomes of their actions. Their struggle against AIDS and homophobia, and to have a voice in their healthcare, presaged the progressive, multi-issue, anti-corporate, confrontational organizing of the late twentieth century, and deserves to be part of that history.

Book excerpt: This is the first book-length study of the rich fiction that has emerged from the AIDS crisis. Examining first the ways in which scientific discourse on AIDS has reflected ideologies of gender and sexuality-such as the construction of AIDS as a disease of gay men, part of a battle over masculinity, and thus largely excluding women with AIDS from public attention-the book considers how such discourses have shaped narrative understandings of AIDS. On the one hand, AIDS is seen as an invariably fatal weakening of an individual's bodily defenses, a depiction often used to reconfirm an identification between disease and a weak and vulnerable gayness. On the other hand, AIDS is understood in terms of an epidemic attributable to gay immorality or unnaturalness. The fiction of AIDS depends upon these two narratives, with one major subgenre of AIDS novel presenting narratives of personal illness, decline, and death, and a second focusing on epidemic spread. These novels also question the narrative structures upon which they depend, intervening particularly against the homophobia of those structures, though also sometimes reinforcing it.

Farley's advocacy both in the US and Africa on the issue of women and AIDS is renowned, and she will be the best-known contributor. The leading contributor from English-speaking Europe is Linda Hogan from Trinity College Dublin.

In Men at Risk, Shari L. Dworkin argues that the centrality of heterosexual relationship dynamics to the transmission of HIV means that both women and men need to be taken into account in gender-specific HIV/AIDS prevention interventions. She looks at the “costs of masculinity” that shape men’s HIV risks, such as their initiation of sex and their increased status from sex with multiple partners. Engaging with the common paradigm in HIV research that portrays only women—and not heterosexually active men—as being “vulnerable” to HIV, Dworkin examines the gaps in public health knowledge that result in substandard treatment for HIV transmission and infection among heterosexual men both domestically and globally. She examines a vast array of structural factors that shape men’s HIV transmission risks and also focuses on a relatively new category of global health programs with men known as “gender-transformative” that seeks to move men in the direction of gender equality in the name of improved health. Dworkin makes suggestions for the next generation of gender-transformative health interventions by calling for masculinities-based and structurally driven HIV prevention programming. In a probing critique of that culture, Katie Hogan demonstrates ways in which literary and popular works use the classic image of the nurturing female to render "queer" AIDS more acceptable, while consigning women to conventional roles and reinforcing the idea that everyone with this disease is somehow suspect.In times of crisis, the figure of the idealized woman who is modest and selfless has repeatedly surfaced in Western culture as a balm and a source of comfort—and as a means of mediating controversial issues. Drawing on examples from journalism, medical discourse, fiction, drama, film, television, and documentaries, Hogan describes how texts on AIDS reproduce this historically entrenched paradigm of sacrifice and care, a paradigm that reinforces biases about race and sexuality. Hogan believes that the growing nostalgia for women's traditional roles has deflected attention away from women's own health needs. Throughout her book, she depicts caretaking as a fundamental human obligation, but one that currently falls primarily to those members of society with the least power. Only by rejecting the stereotype of the "good woman," she says, can Americans begin to view caretaking as the responsibility of the entire society.
