Book excerpt: In spite of a decline in HIV incidence rates among African American women, they still bear the most significant disease burden among U.S. women. Findings from numerous studies indicate probable explanations for the disparity, such as the impact of poverty, limited healthcare access, low literacy, and living in areas with high HIV rates. Additionally, many study results provide insight regarding prevention strategies. However, the aim of this study is to explore African American women's perceptions regarding what HIV prevention communication, if any, occurred with their reproductive health providers (RHPs). In this study, 20 African American women with unknown HIV status participated in face-to-face interviews designed to explore their perceptions about HIV prevention communication with their RHPs. Audio-taped interviews were transcribed verbatim and coded using NVivo10 software. Guided by constructs of the Health Belief Model, inductive and deductive coding yielded four key themes: (1) patients' lack of expectation to receive information; (2) failure of RHPs to initiate and offer information; (3) patients' desire to receive information; and (4) patients' recommendations regarding their preferred methods to receive HIV prevention communication. Results indicated that RHPs missed prime opportunities to initiate and offer HIV prevention information during routine reproductive health visits with women at greatest risk. These findings and recommendations for practice will be useful when designing, implementing and evaluating HIV prevention patient education protocols. The recommendations provide strategies to help RHPs seize every opportunity to address HIV prevention with this highly vulnerable population.

Book excerpt: Intergenerational Dialogue About Sexual Health and HIV Prevention Among African American Women in Rural Mississippi Gayle Robinson The purpose of the study was to explore perceptions of intergenerational dialogue (IGD) among African American women and to investigate whether IGDs about sexual health and HIV prevention could be an effective cultural medium to discuss and communicate HIV prevention strategies. The motivation was to find a culturally appropriate process that would contribute to preventing HIV infections among present and future generations of African American women. This study explored the extent to which African American women in rural Scott County, Mississippi considered IGD as having the potential to decrease the transmission of HIV. This is important because of the ongoing mortality and morbidity of African American women due to HIV. The study sample included 30 African American women aged 18 to 80 who were recruited within the town of Forest, in Scott County, Mississippi (MS). In-depth, semi-structured interviews were audio recorded and transcribed into text for this qualitative study. The data analysis was conducted using conventional content analysis. Research results indicate that IGD is occurring. However, dialogues about sexual health and HIV prevention occur less frequently than dialogues in which more general topics are discussed. My findings also show that key features of an IGD are influenced by the nature and quality of the relationship among the individuals engaged in the dialogue. How the IGD participants relate to each other controls what is talked about in terms of sexual health and HIV prevention. Finally, my research results illustrate that topics related to sexual health and HIV prevention are often raised in the context of discussions on risk of pregnancy, risk for contracting sexually transmitted infections, and the consequences of other personal choices.

African-American women are 23 times as likely to be infected with the AIDS virus as white women. African-American women account for 72% of new HIV cases among women in 29 states. The risk of contracting HIV virus is highest in African-American communities, which inevitably places African-American women at higher risk than other populations of women. The purpose of this study was to advance knowledge regarding what is unknown about risky sexual behaviors among African-American heterosexual women by giving them the "voice" to share their own personal experiences in their natural environments. I examined participants' perceptions of risk for contracting HIV/AIDS in relationships with male partners. This qualitative research design focused on a constant comparative analysis. I conducted one focus group [four members and one recorder] along with seven individual interviews, of African-American heterosexual women involved in the Women In Motion [WIM] HIV/AIDS prevention program. The following three health behavior frameworks were examined as a means of understanding the limitations of existing models of sexual risk behaviors among African-American women: The Health Belief Model (HBM), the Transtheoretical Change Model, and the Black feminist perspective. Gaps in the literature included insufficient knowledge of how cultural taboos and myths influence sexual decision-making. An overview of the findings of this study has been explicated under the following three main headings: (1) Observation, (2) Interpretation, and (3) Application. The results of the study are discussed under the following three main categories 1) Understanding Sexual Decision-Making, 2) Understanding Intimacy, and 3) Understanding HIV/AIDS Prevention With Male Partners. In conclusion, sexual decision-making in this inquiry became an all encompassing construct based on African-American women's perceptions of how they viewed the paradox of sexual needs in intimate relationships with male partners and the risk of contracting HIV/AIDS.

Book excerpt: This project involved the collection and analysis of data from pre-and post-tests and five focus groups with 54 community-dwelling, middle-aged African American women in Eastern North Carolina, in order to explore the reasons why these women underestimated their level of risk for contracting HIV. This research was conducted under the auspices of the SISTER Talk Project, a part of the REACH Out Program administered through the Brody School of Medicine. Analysis involved determining African American women's perceptions of HIV risk, reported partnership behavior, and the influence of traditional gender roles on risk-related behaviors. Partner concurrency was found to be a common behavior in the groups studied and increased risk of contracting HIV. Homosexual behavior was also found to be heavily stigmatized and often carried out in secret, causing women to be unaware that their male partners might also be engaging in sexual relations with other men, thereby increasing the risk of contracting HIV. A key finding of this study was that traditional gender role expectations inhibit women from confronting men about partner concurrency and from requesting condom use for protection. The data collected in this study indicate that although educating women about HIV does help raise awareness about risk-related behaviors, education alone is not sufficient to solve issues of powerlessness in relationships due to perceived male dominance, poverty, and lack of communication. Women who are in these situations need further intervention, which would require involving their partners in education sessions and in discussions about HIV risk. This necessary step could help reduce the risk of HIV for both men and women, as well as reduce risk due to traditional gender role expectations among partners.
