Book excerpt: E.MARTIN Acute pain services are now established worldwide and guidelines have been drawn for the management of acute pain resulting from surgical or medical procedures and trauma. However, the treatment of pain after surgery is still inadequate and no progress has been made in recent years in several coun tries, including Germany. There are still innumerable patients who find the is also no early postoperative period to be an unpleasant experience. There doubt that pain plays a role in the pathogenesis of postoperative complica tions that could be avoided with effective pain management. However, concern about side effects and inadequate knowledge of the pharmacokinet ics and -dynamics of drugs is still putting constraints on treatment. An acute pain service should be responsible for adequately treating pain, training medical and nursing staff, and evaluating new and existing methods of treatment. As anesthesiologists deal with pain in the operating theater, it is not surprising that they claim a leading role for themselves in acute pain services choosing from the various postoperative pain treatment options.

Book excerpt: Knowledge of the mechanism of action of drugs at cellular, subcellular, or molecular levels is of vital importance not only in giving the basis of inter pretation of the systemic action of drugs but also in improving existing drugs; in designing new forms of drugs; and in giving the basis of therapeutic applications. Classical pharmacology, concerning the action of drugs at integrated levels, does not necessarily give sufficient information as to the mechanism of action of drugs. A variety of sophisticated concepts utilizing the methods of physics, chemistry, biophysics, biochemistry, and physiology must be synthesized to understand the mechanism of action. Only since the last decade, however, have these techniques been fully applied to pharma cological investigations. It is of utmost importance to realize that a new dimension of pharmacological research has indeed emerged as a result of such a multidisciplinary approach; this approach is encompassed in general and cellular pharmacology. Such recent studies of drug actions have led to a number of important findings. Certain chemicals and drugs were found to possess highly specific actions on cellular functions, so that they are widely being used as powerful tools for the study of a variety of physiological and pharmacological prob lems. Our knowledge of the cellular mechanisms of drug action has provided the basis for interpreting the systemic effects of the drugs and insight into the molecular mechanism involved.

Book excerpt: The most striking fact revealed by investigations of insect neurohormones is that insects are as well supplied with neurohormones as mammals, since neurohor mones regulate not only the functioning of the endocrine glands, prothoracic gland, and corpora allata, but also most physiological processes. Our knowledge of neurohormones developed originally from anat omocytological investigations and experimental studies. Today, accurate bio assays have been devised for studying both in vivo and in vitro physiological processes, and RIA determination has yielded knowledge of titer modifications of humoral factors. Much is also known about neurohormone purification, and several neurohormones have even been identified in different species. Immunocytochemistry has made it possible to demonstrate in their origin and release sites the presence of insect neurohormones whose structure has been elucidated. Moreover, the presence of vertebrate and invertebrate neuropeptides has been demonstrated in insects. As regards biogenic amines, methods of detection have been greatly refined and it is now possible to identify the cell bodies and axons of the main biogenic amines. Other new methods, such as cobalt chloride impregnation or Lucifer yellow staining, have revealed the axonal pathways and the location of particular neurons. The mechanisms of action of neurohormones have been investigated in several cases and the results of these investigations will be related in the chapters which follow.

Book excerpt: Stress has been recognized as an important factor in the development or recurrence of various mental disorders, from major depressive disorder to bipolar disorder to anxiety disorders. Stressful stimuli also appear to exert their effects by acting upon individuals with susceptible genotypes. Over the past 50 years, animal models have been developed to study these dynamic interactions between stressful stimuli and genetically susceptible individuals during prenatal and postnatal development and into adulthood. Stress and Mental Disorders: Insights from Animal Models begins with a discussion of the history of psychiatric diagnosis and the recent goal of moving toward precision psychiatry, followed by a review of clinical research on connections between stressful stimuli and the development of psychiatric disorders. Chapters are also included on neuroendocrine, immune, and brain systems involved in responses to stress. Additional chapters focus on the development of animal models in psychiatry and the susceptibility of the developing organism to stressful stimuli. Subsequent chapters are devoted to animal models of specific stress-sensitive psychiatric disorders, including schizophrenia, autism spectrum disorders, bipolar disorder, anxiety disorders, depression, and post-traumatic stress disorder. These chapters also focus on identification of promising molecular targets for development of new drug therapies. The section concludes with a chapter on animal models of resilience to stress-induced behavioral alterations as a newer approach to understanding why some animals are susceptible to stress and others are resilient, even though they are essentially genetically identical. The final chapter discusses how these basic laboratory studies are providing promising leads for future breakthroughs in the diagnosis, treatment, and prevention of mental disorders.

Book excerpt: One of the most impressive works of scholarship in the field of experimental pharmacology has been the Heffter-Heubner Handbuch der experimentellen Pharmakologie, internationalized some years ago under the title Handbook 0/ Experimental Pharmacology and kept up to date by a series of numbered Ergiin zungswerke or supplementary volumes which have now replaced in importance the original Handbuch. These volumes constitute a valuable and continuously up dated multi author review series of topics important in modern pharmacology and allied sciences. The Editorial Board of the Handbook invited me 2 years ago to undertake, as subeditor, the preparation of a new volume entitled The Cholinergic Synapse. A previous volume in this series, vol. 15, Cholinesterases and Anticholinesterase Agents, edited by GEORGE KOELLE, was published in 1963 and was far wider in scope than its title suggested: it was, in fact an authoritative summing up of the whole subject of cholinergic function and still has some value today as an account of the state of the art as it was at that time. Since then another excellent review, of a specific cholinergic synapse, has appeared in this series: this was vol. 42, Neuromuscular Junction, edited by ELEANOR ZAIMIS and published in 1976. A third volume, vol. 53, Pharmacology o/Ganglionic Transmission, which appeared in 1980 and was edited by D. A. KHARKEVICH, includes important aspects of autonomic cholinergic function.
