Book excerpt: In Canada, the prevalence of adolescent overweight and obesity has increased significantly and currently accounts for about 20% of the adolescent population in the 12 to 17 year old age group. Adolescents with a higher weight status experience a host of physical, social and psychological challenges and have been shown to have an increased likelihood of engaging in various high-risk behaviors. However, the relationship between adolescent overweight and obesity and risky sexual behavior has received scant attention in the literature. Studies have shown that there is a significant association between obesity and lower levels of self-esteem as well as body image satisfaction. In turn, a number of studies have demonstrated that lower levels of self-esteem and body image satisfaction are predictors of risky sexual behavior. Accordingly, the aim of this study was to unveil any direct associations between adolescent overweight and obesity and risky sexual behavior, and explore the possibility of body image satisfaction and self-esteem as psychological pathways. Data from the 2009/2010 cycle of the Canadian Community Health Survey (CCHS) were used, where a total of 8 168 participants between the ages of 15 to 19 were included. The results of this study showed that obese female adolescents were less likely to have ever had sex, both in the 15 to 17 year old age group as well as the 18 to 19 year old age group. In the 15 to 17 year old age group, overweight females were more likely to be diagnosed with a sexually transmitted infection (STI) and in the 18 to 19 year old age group, obese females were less likely to have used a condom the last time they had sex and less likely to have used birth control in the past 12 months. Overweight and obese adolescents had significantly lower levels of body image satisfaction in both age groups, while obese adolescents experienced lower levels of self-esteem in the 15 to 17 year old age group. Implications for intervention and prevention strategies are discussed and recommendations for future studies provided.

Book excerpt: Abstract: This study examined the relationships between sexual behaviors, suicidality, emotional functioning, and the roles of gender/ethnicity among adolescents in the United States. The study methodology was a secondary data analysis using survey data completed by 13,061 adolescents from the 2009 Youth Risk Behavior Surveillance System (YRBSS) conducted by the Center for Disease Control and Prevention (CDC). Findings revealed that adolescent sexual behavior, including risky sexual behaviors, is strongly associated with suicide behaviors and feelings of sadness/hopelessness. Results indicated significant differences among study variables for adolescents who have experienced sexual coercion, including higher rates of suicide behaviors and feelings of sadness/hopelessness. There were statistically significant differences between gender/ethnicity and the main study variables. These findings illuminate the need for developing sex education curricula which include the emotional health component of sexual behavior. Furthermore, this study highlights the importance for social workers to become culturally competent in addressing topics of sex and suicide with adolescent clients.

The suicide rate for individuals 15-19 years of age in 2003, while having recently declined, still remains more than twice that in 1960. At the same time, the prevalence of being overweight has also steadily risen among adolescents, and has tripled since 1960. This study utilizes data from the Youth Risk Behavioral Surveillance System (1991-2003) to explore the causal relationship between overweight status and the perception of overweight on suicide ideation and attempts. Studies have shown a high degree of correlation between overweight, depressive disorders, and suicidal behaviors. This study analyzes these indicators in conjunction with individuals' perception of their weight. The empirical methodology is based on bivariate probit models, propensity score matching, and sample stratification, along with robustness checks, to gauge whether the link between overweight indicators and suicide is causal or whether it is driven by unobserved selection. Results indicate that body dissatisfaction, as measured by the perception of overweight, has a strong causal impact on all suicidal behaviors for girls. It raises the risk of suicide ideation by 6.1 percent, suicide attempt by 3.6 percent, and a serious suicide attempt by 0.5 percent. For males, the association is mostly driven by nonrandom selection, though there is a small positive effect on ideation. Conditional on overweight perception, actual weight does not have an independent effect on suicidal behaviors. Policies aimed at reducing the prevalence of overweight among adolescents will further reduce suicidal behaviors by limiting overweight perception, especially among girls. However, the independent role of perception also highlights the importance of educating youths and fostering healthy attitudes regarding body image. Conservative estimates indicate that the risk of higher female suicides adds about $280 - $350 million to the costs of adolescent overweight. In addition, decreasing the prevalence of being very overweight among girls, consistent with the Healthy People 2010 objectives, would reduce the number of suicide attempts by between 12,000 and 16,000.

The estimated 12-month and lifetime prevalence values for AUD are 13.9% and 29.1%, respectively, with approximately half of individuals with lifetime AUD having a severe disorder. AUD and its sequelae also account for significant excess mortality and cost the United States more than $200 billion annually. Despite its high prevalence and numerous negative consequences, AUD remains undertreated. In fact, fewer than 1 in 10 individuals in the United States with a 12-month diagnosis of AUD receive any treatment. Nevertheless, effective and evidence-based interventions are available, and treatment is associated with reductions in the risk of relapse and AUD-associated mortality. The American Psychiatric Association Practice Guideline for the Pharmacological Treatment of Patients With Alcohol Use Disorder seeks to reduce these substantial psychosocial and public health consequences of AUD for millions of affected individuals. The guideline focuses specifically on evidence-based pharmacological treatments for AUD in outpatient settings and includes additional information on assessment and treatment planning, which are an integral part of using pharmacotherapy to treat AUD. In addition to reviewing the available evidence on the use of AUD pharmacotherapy, the guideline offers clear, concise, and actionable recommendation statements, each of which is given a rating that reflects the level of confidence that potential benefits of an intervention outweigh potential harms. The guideline provides guidance on implementing these recommendations into clinical practice, with the goal of improving quality of care and treatment outcomes of AUD.
