While HB-HTA has gained significant momentum over the past few years, expertise in the field, and information on the operation and organization of HB-HTA, has been scattered. This book serves to bring this information together to inform those who are currently working in the field of HTA at the hospital, regional, national or global level. In addition, this book is intended for decision-makers and policy-makers with a stake in determining the uptake and decommissioning of new and established technologies in the hospital setting. HTA has traditionally been performed at the National/Regional level by HTA Agencies, typically linked to governments. Yet hospitals are the main entry door for most health technologies (HTs). Hospital decision-makers must undertake multiple high stakes investment and disinvestment decisions annually for innovative HTs, usually without adequate information. Despite the existence of arms-length HTA Agencies, inadequate information is available to hospital decision-makers either because relevant HTA reports are not yet released at the time of entry of new technologies to the field, or because even when the report exists, the information contained is insufficient to clarify the contextualized informational needs of hospital decision makers. Therefore, there has recently been a rising trend toward hospital-based HTA units and programs. These units/programs complement the work of National/Regional HTA Agencies by providing the key and relevant evidence needed by hospital decision makers in their specific hospital context, and within required decision-making timelines. The emergence of HB-HTA is creating a comprehensive HTA ecosystem across health care levels, which creates better bridges for knowledge translation through relevance and timeliness.

Book excerpt: Even though artificial insemination is a simple technique that has been practiced for over a century, it has long been carried out under poor conditions due to an inadequate understanding of repro ductive physiology and antagonistic socio-ethical attitudes. Accor dingly, until fairly recently it was a medical act with a limited scientific basis which was practised more or less clandestinely. The development of semen preservation has totally changed the conditions of artificial insemination, especially in regard to flexibility and safeguards in its application. Although the use of fresh semen continues, it is now clear that the future of arti ficial insemination is closely linked to semen preservation. During the past two decades, semen banks have been developed in many countries. This has most often been the result of the initiative of individual physicians in either the private or public sectors. In France, a national system of semen banks (CECOS) was begun in 1973. Although there has been cooperation within this system in the areas of both research and management, a need to com municate and compare experiences with those from other countries was perceived. Thus, the first International Symposium on Artifi cial Insemination and Semen Preservation was planned and held in Paris, France in April 1979. Thirty-seven countries were represented byihdividuals from many concerned disciplines.

Book excerpt: Aspergillus is among the economically most important fungal genera. Aspergillus species are used in the fermentation industry for the production of various enzymes, organic acids and pharmaceutically important compounds, and in Oriental food fermentations. At the same time, Aspergilli can cause opportunistic infections, and produce a range of secondary metabolites also known as mycotoxins which are harmful to humans and animals. Due to its importance in biotechnology, medicine and foods, Aspergilli are in the forefront of studies dealing with various aspects of fungi. This is well illustrated by the fact that projects aiming at sequencing the genomes of nine species are in progress. In this book, some aspects of the biology of this genus are covered, including the biotechnological importance of Aspergilli, their role in mycotoxin contamination of food products including grapes and coffee products, and implications of biodiversity of Aspergillus populations for agriculture. Among the clinical aspects, the role of Aspergilli in eye infections, the mechanisms of antifungal drug resistance and their molecular identification in the clinical setting are covered. Several chapters deal with the genomic aspects of Aspergilli, including the genomics of pigment biosynthesis, extracellular enzyme production, aflatoxin biosynthesis and the identification of genes taking part in sexual processes. Additional chapters cover the variability and inheritance of mycoviruses in Aspergilli and multilocus approaches are given to clarify the evolutionary history of the genus. Finally, a new taxonomic classification of the various Aspergillus sections is given based on multilocus sequence data.
