Book excerpt: Hospital discharge is a critical and vulnerable period for older patients and a transition that extends beyond hospital to the home. Older patients{u2019} readiness for discharge is important, as management of recovery can be complex. However, shorter hospital stays mean less time for patients, families and health professionals to prepare for the transition. Readiness for discharge involves physical, psychological dimensions and support. Person centred discharge planning includes patient perspectives, however little research has examined patients{u2019} perception of their readiness for discharge with older populations. No research examined perception of readiness, measures of function and support at discharge in relation to support post-discharge. Aim: To examine older patients{u2019} readiness for discharge from hospital to home and to determine whether relationships existed between readiness at discharge and post-discharge use of community supports and services. Method: The research design was quantitative descriptive and correlational, guided by Meleis{u2019}s middle-range theory of transition. A sample of 335 patients over 65 were accessed at two time periods: in hospital at discharge and by telephone interview six weeks post-discharge. Data were recorded using the Demographic and Community Resource Questionnaire; Readiness for Hospital Discharge Scale; Barthel Index; Mini-mental State Examination; and Charlson co-morbidity index, and analysed using SPSS version 15. Descriptive and inferential statistical analyses were employed to test relationships between variables at discharge and post-discharge. Findings: At discharge respondents perceived themselves reasonably ready (RHDS Mean 7.31, SD 1.18); however lower readiness was recorded for those over 80 (median 6.83). Forty eight percent were moderately dependent in activities of daily living and 12% indicated some cognitive impairment. Informal support in place was mainly with household and transport. Formal services in place were General Practitioner, Public Health Nurse and home help. Post discharge, informal support increased. Lower perception of readiness, longer hospital stay, higher co-morbidity, higher dependence in ADL and lower cognitive function were significantly more likely to use informal support. Older age, living alone, lower perception of readiness and shorter stay in hospital were significantly related to formal support. Lower perception of readiness for discharge was also statistically associated with readmission. Conclusion Although older patients{u2019} perception of their readiness may not reflect their clinical preparedness, the inclusion of the patient{u2019}s perception encourages participation in discharge. Results have relevance for clinical nursing practice, education and research.

This report, the summary volume, includes the history of residential schools, the legacy of that school system, and the full text of the Commission's 94 recommendations for action to address that legacy. This report lays bare a part of Canada's history that until recently was little-known to most non-Aboriginal Canadians. The Commission discusses the logic of the colonization of Canada's territories, and why and how policy and practice developed to end the existence of distinct societies of Aboriginal peoples. Using brief excerpts from the powerful testimony heard from Survivors, this report documents the residential school system which forced children into institutions where they were forbidden to speak their language, required to discard their clothing in favour of institutional wear, given inadequate food, housed in inferior and fire-prone buildings, required to work when they should have been studying, and subjected to emotional, psychological and often physical abuse. In this setting, cruel punishments were all too common, as was sexual abuse. More than 30,000 Survivors have been compensated financially by the Government of Canada for their experiences in residential schools, but the legacy of this experience is ongoing today. This report explains the links to high rates of Aboriginal children being taken from their families, abuse of drugs and alcohol, and high rates of suicide. The report documents the drastic decline in the presence of Aboriginal languages, even as Survivors and others work to maintain their distinctive cultures, traditions, and governance. The report offers 94 calls to action on the part of governments, churches, public institutions and non-Aboriginal Canadians as a path to meaningful reconciliation of Canada today with Aboriginal citizens. Even though the historical experience of residential schools constituted an act of cultural genocide by Canadian government authorities, the United Nation's declaration of the rights of aboriginal peoples and the specific recommendations of the Commission offer a path to move from apology for these events to true reconciliation that can be embraced by all Canadians.
