Book excerpt: The purpose of this project was to compare the accuracy and precision of linear measurements made on conventional lateral cephalograms (LC) and simulated LC using cone-beam computed tomography (CBCT) to direct measurements on dentate skulls. The linear distances for 5 mid-sagittal and 4 bilateral measurements between anatomical landmarks on 22 skulls were measured by two observers. The skulls were imaged using a CBCT scanner at 4 scan settings: high resolution (0.2mm) and normal resolution (0.4mm) at 20-sec (306 projections) and 40-sec scans (612 projections). Three simulated LC were generated: Ray sum image reconstructions of the maximum thickness mid-sagittal multiplanar projections, a single frame basis projection, and the scout image. LCs were acquired using a storage phosphor system. Projection images were exported into a cephalometric analysis program and linear measurements determined. Analyses were repeated three times and statistically compared to measured anatomic truth using repeated measures general linear model (p [less than or equal to] 0.05). The mean absolute error was used as an index of precision. While there were statistical differences between skull and modality measurements for 6 of the nine measurements, there were no overall differences between LC and CBCT derived image measurements irrespective of method of construction. CBCT imaging provides clinically reliable 2D simulation images however, dose considerations demand that evidence-based selection criteria should be developed for CBCT in orthodontics that take into account the ALARA principle.

Book excerpt: The purpose of this project was to compare the accuracy and reliability of linear measurements on posterior-anterior (PA) cephalograms made conventionally using photostimulable storage phosphor imaging plates (PSP) and simulated PA cephalometric images from ray sum reconstruction of cone beam computed tomographic (CBCT) volumetric datasets of the same anatomic specimens. The sample consisted of 19 dried dentate human skulls which were imaged using PA cephalometry using PSP imaging plates and three CBCT datasets at 10 s., 20 s. and 40 s. scanning times from which ray sum simulated PA images were produced. Various anatomic landmarks were identified and truth measurements of 13 selected cephalometric linear dimensions were measured directly on the skulls using a high precision caliper. CBCT datasets were imported into a reconstruction program (Dolphin 3D Software) from which ray sum PA images were constructed. Linear cephalometric measurements on PSP and CBCT simulated PA images were performed by one observer using Dolphin Imaging Software. Intraobserver reliability was determined by calculating and comparing Intraclass correlation coefficient. Mean absolute and percentage errors were calculated for each dimension and modality to quantify the reliability error. Mean modality measurements were compared. All other groups were compared using one-way ANOVA with Tukey post hoc comparisons (p [less than or equal to] 0.05). While the ICC was high, PSP cephalometric absolute error was greater than anatomic distances and CBCT image measurements for 8 for 13 measurements. We found that for most measurements, simulated PA images produced from reconstructed CBCT volumetric dataset were accurate and compared well to PSP cephalometric images except for Z-Ag. We also found that PSP cephalometric images consistently underestimated MO-MO dimensions. Measurements on CBCT simulated PA images are more reliabile and as accuracte as PSP cephalometric measurements. The number of projections comprising the reconstructed simulated PA image did not influence reliability or accuracy. Therefore in accordance with ALARA, when CBCT imaging is used to generate simulated PA images, the lowest number of projections are recommended.

Book excerpt: The purpose of this project was to compare the accuracy and reliability of linear measurements made on 2D projections and 3D reconstructions using Dolphin 3D software (Chatsworth, CA) as compared to direct measurements made on human skulls. The linear dimensions between 6 bilateral and 8 mid-sagittal anatomical landmarks on 23 dentate dry human skulls were measured three times by multiple observers using a digital caliper to provide twenty orthodontic linear measurements. The skulls were stabilized and imaged via PSP digital cephalometry as well as CBCT. The PSP cephalograms were imported into Dolphin (Chatsworth, CA, USA) and the 3D volumetric data set was imported into Dolphin 3D (Version 2.3, Chatsworth, CA, USA).Using Dolphin 3D, planar cephalograms as well as 3D volumetric surface reconstructions were (3D CBCT) generated. The linear measurements between landmarks of each three modalities were then computed by a single observer three times. For 2D measurements, a one way ANOVA for each measurement dimension was calculated as well as a post hoc Scheffe multiple comparison test with the anatomic distance as the control group. 3D measurements were compared to anatomic truth using Student's t test (P [less than or equal to] 50.05). The intraclass correlation coefficient (ICC) and absolute linear and percentage error were determined as indices of intraobserver reliability. Our results show that for 2D mid sagittal measurements that Simulated LC images are accurate and similar to those from PSP images (except for Ba-Na), and for bilateral measurements simulated LC measurements were similar to PSP but less accurate, underestimating dimensions by between 4.7% to 17%.For 3D volumetric renderings, 2/3 rd of CBCT measurements are statistically different from actual measurements, however this possibly is not clinically relevant.

To compare the in vitro reliability and accuracy of linear measurements between cephalometric landmarks on CBCT 3D images with varying basis projection images to direct measurements on human skulls. Methods. Sixteen linear dimensions between anatomical sites marked on 19 human skulls were directly measured. Skulls were imaged with CBCT at three settings: 153, 306, and 612 basis projections. The mean absolute error and modality mean of linear measurements between landmarks on 3D images were compared to the anatomic truth. Results. No difference in mean absolute error between the scan settings was found. The average skull absolute error between marked reference points were less than the distances between unmarked reference sites. Conclusion. CBCT measurements were consistent between scan sequences and for direct measurements between marked reference points. Reducing the number of projections for 3D reconstruction did not lead to reduced dimensional accuracy and potentially provides reduced patient radiation exposure.

Book excerpt: The purposes of this study were to: (1) determine whether dento-maxillofacial (DMF) cone beam computed tomography (CBCT) produces accurate MPR images of a temporomandibular joint (TMJ) articulation; (2) compare the accuracy of DMF CBCT with traditional digital extraoral radiography; and (3) compare the reproducibility of measurements made of the TMJ using DMF CBCT with traditional digital extraoral radiography. Ten linear measurements of the TMJ and associated structures were compared using 25 dried skull specimens for DMF CBCT as well as cephalometric projections. Means and standard deviations were compared using ANOVA and post-hoc tests (Dunette's Test) at a significance level of p [less than or equal to] .05. The results show DMF CBCT is significantly more accurate than traditional extraoral radiography for all 10 measurements made of the TMJ and associated structures. Although the measurements were less reproducible for the modalities tested than for the measurements made directly on the skulls, all were within clinically acceptable limits.

Book excerpt: Abstract: Objectives: To assess the ability of using cone-beam computed tomography (CBCT) to diagnose small osseous defects at the mandibular condyle, this study investigated: 1) the impact of defect size and CBCT scan resolutions on the accuracy of qualitative and quantitative diagnoses of condylar defects and, 2) the ability to determine a suspected defect to be true/false based on CBCT measurements. Methods: Cylindrical holes simulating condylar defects of varied levels of diameter (=2mm; 2-3mm; >3mm) and depth (=2mm; >2mm) were created at 22 fresh pig mandibular condyles, with defect number/size per condyle/quadrant randomly determined. With soft tissues repositioned, CBCT scans (0.4mm, 0.2mm voxel-size) of the pig heads were obtained. Reconstructed CBCT data were analyzed independently by two calibrated, blinded raters using Dolphin-3D to diagnose osseous defects qualitatively (defect identification/localization) and quantitatively (geometric measurements of diameter/depth), which were compared to physical diagnoses obtained from polyvinyl siloxane impressions. Statistical tests were conducted for the research objectives.
