Book excerpt: Transsexualism refers to the most extreme form of incongruity between an individual's gender identity and anatomic sex. In 1994, the diagnosis of transsexualism was replaced with ”gender identity disorder" by the DSM-IV committee. Transsexualism is associated with distress because the individual, due to his/her physical characteristics, is not treated as a representative of the gender he/she identifies with and cannot therefore live in a gender role with which he/she identifies. The sexual anatomy of the body also feels alien and inappropriate. Transsexualism that causes distress is treated with sex reassignment therapy. The aim is to alter body characteristics and anatomy closer to those of the target gender as well as to facilitate the integration into the social role of the opposite sex. Investigations and surgery are often only available in specialist centres.

L'amélioration de la santé sexuelle fait partie des défis actuels du gouvernement. Dans ce contexte, le rôle du médecin généraliste dans la prévention en santé sexuelle est primordial. Aborder ce sujet avec un jeune adolescent peut être délicat, nous avons donc interrogé une population d'adolescents pour connaître leur point de vue concernant la manière d'aborder la sexualité en consultation de médecine générale. Méthode : Il s'agit d'une étude quantitative descriptive réalisée via un questionnaire anonyme en ligne auquel 202 élèves de quatrième âgés de 12, 13 et 14 ans ont répondu. Résultats : Lorsqu'ils se posent des questions en rapport avec la sexualité, les adolescents se tournaient vers leurs parents pour 37,62% d'entre eux et vers leurs amis pour 38,61%. Bien que 48,51% des adolescents pensaient qu'il soit pertinent de discuter de sexualité avec le médecin traitant, seulement 4,95% des interrogés le citaient comme source d'information privilégiée. Au collège, bien que qualitativement satisfaisantes, les séances d'éducation à la vie sexuelle et affective n'ont été que de 2 maximum pour 51,98% des élèves interrogés (sur leurs 3 années de collège) voire non réalisées d'après pour 37,62% d'entre eux. Avec le médecin traitant, la sexualité n'avait jamais été abordée pour 82,67% des adolescents interrogés et 85,64% ignoraient totalement l'existence de la CCP. Les adolescents étaient freinés à l'idée de parler de sexualité avec leur médecin traitant car ils estimaient mal le connaître (32,67%), avaient peur de son jugement (26,76%) et de voir leurs confidences répétées à leur parents (23,27%). Pour aborder la sexualité en consultation, les adolescents préféraient être reçus seul (65,84%) et que le sujet eut été abordé par le médecin (51,49%). Plutôt qu'une consultation dédiée à la vie sexuelle et affective, 19,80% des adolescents préféraient être guidés sur des sites Internet fiables ou se voir distribués des brochures. Conclusion : Discuter de santé sexuelle en consultation de médecine générale avec un adolescent peut s'avérer laborieux, notamment parce que l'adolescent est souvent mal à l'aise. Afin d'agir sur la prévention sexuelle, le médecin généraliste se doit de créer un lien entre l'adolescent et lui sur le long terme. Ce lien est nécessaire à l'ouverture du dialogue, pour que l'adolescent soit assez en confiance et puisse confier ses questions ou problèmes. Le médecin traitant devra s'attarder à lui montrer sa disponibilité, et à mettre ses représentations subjectives de l'adolescence de côté. Créer du lien ne peut se faire qu'avec une approche respectueuse et patiente, et en considérant l'adolescent, dans sa globalité.

How did it come into being and what are the characteristics of this distinctive field of power that has come to play such a central role in the shaping of all spheres of social, political and economic life? In this major work the great sociologist Pierre Bourdieu addresses these fundamental questions. Modifying Max Weber’s famous definition, Bourdieu defines the state in terms of the monopoly of legitimate physical and symbolic violence, where the monopoly of symbolic violence is the condition for the possession and exercise of physical violence. The state can be reduced neither to an apparatus of power in the service of dominant groups nor to a neutral site where conflicting interests are played out: rather, it constitutes the form of collective belief that structures the whole of social life. The ‘collective fiction’ of the state Ð a fiction with very real effects - is at the same time the product of all struggles between different interests, what is at stake in these struggles, and their very foundation. While the question of the state runs through the whole of Bourdieu’s work, it was never the subject of a book designed to offer a unified theory. The lecture course presented here, to which Bourdieu devoted three years of his teaching at the Collège de France, fills this gap and provides the key that brings together the whole of his research in this field. This text also shows ‘another Bourdieu’, both more concrete and more pedagogic in that he presents his thinking in the process of its development. While revealing the illusions of ‘state thought’ designed to maintain belief in government being oriented in principle to the common good, he shows himself equally critical of an ‘anti-institutional mood’ that is all too ready to reduce the construction of the bureaucratic apparatus to the function of maintaining social order. At a time when financial crisis is facilitating the hasty dismantling of public services, with little regard for any notion of popular sovereignty, this book offers the critical instruments needed for a more lucid understanding of the wellsprings of domination.
