Book excerpt: Excerpt from The Peritoneum, Vol. 1: Structure and Function in Relation to the Principles of Abdominal Surgery This study was begun during my student days, now twenty-five years ago, and has occupied my leisure moments during all these years. Being done in conjunction with an active surgical practice, it naturally partakes of the practical viewpoint. The first volume discusses the more abstract problems, while the second volume has to do with the practical aspects. This being a research I have not felt obligated to treat all chapters equally and in the same way. In those chapters in which my own work is chiefly represented, there was in some instances little literature to consider. In others, for instance, that on Tuberculosis, where my own material was limited, the literature was reviewed extensively. On the whole, my reasons for the selection of the literature quoted are purely personal, and no attempt has been made to judge the permanency of the value of the papers selected for the bibliography. Though I have done most of the work alone, much help was received from the frequent association with Dr. Montrose T. Burrows, Associate Professor of Pathology, Washington University Medical School; and in recent years Dr. Albert C. Eycleshymer, Professor of Anatomy, University of Illinois Medical School, has extended valuable aid and criticism. The ever-growing importance of Abdominal Surgery rendered it advisable that the Operations on the abdomen should be grouped together in the first volume. Some sections have been curtailed or omitted, such as that relating to Lateral Lithotomy, but most of them have been added to, and several are wholly new. A considerable number of fresh illustrations have been inserted, and a series of coloured plates has been introduced to make clearer the anatomy of certain important Operations. These coloured plates have all been drawn Specially for this Edition from dissections in the Museums of the Royal College of Surgeons and the London Hospital Medical College. The present volume comprises the surgery of the stomach and duodenum; and it is hoped that the publication of the second volume will not be long delayed. An immense mass of literature has accumulated within recent years dealing with the surgery of the stomach; and to consult all of this, and to sift the valuable from the worthless, has been no light task. Among these may be mentioned the short lapse of time between injury and surgical assistance; the temperature of the day, which was high, prevented chill ing of the exposed abdominal viscera, thereby diminishing the tendency to shock; and the thorough douching with hot sterile water during the operation and hourly flushing of the abdominal cavity for two days afterwards. Then, too, by the use of Murphy's button much time was saved in making the anastomosis, thus obviating undue manipulation of the bowel, which always tends to produce for a time a certain amount of paresis of its coats which complicates the after treatment of the case.
