Abstract: Background Colorectal cancer (CRC) is one of the most common cancers with high morbidity and mortality among both genders and yet it carries a better prognosis when detected early. Colorectal cancer screening using faecal occult blood test (FOBT) is proven to be cost-effective, however worldwide FOBT uptake rate is suboptimal which directly affects the cost-effectiveness of the screening program. Identifying those factors that influence the uptake of colorectal cancer screening using FOBT will allow implementation of relevant measures when planning a population based screening program. Methods A structured electronic search using PubMed and Medline was conducted in order to identify studies that included factors influencing the uptake of CRC screening by using FOBT. Qualities of included studies were assessed by quality assessment checklist STROBE. Results Factors that contributed to the low uptake rate of CRC screening by FOBT were identified and summarized. They were broadly divided into 3 groups. Demographic factors: age, gender, social economic status, insurance status and education, for ethnicity, employment status and obesity further studies in the future may be needed. Subject factors: subject's attitudes and knowledge towards CRC screening, type of FOBT screening, health concerned behavior, frequency of clinical visit and physiciancomment. Provider factors: health care system factor and physicians' factors. Conclusion Different factors, in particular those factors that were associated with low FOBT uptake rate in CRC screening, were reviewed and summarized in this paper. With the continuous effort from worldwide as well as local investigators, timely measures can be implemented to tackle this deathly disease and to ensure cost effectiveness of a screening program. Comprehensive and accurate disease surveillance systems are needed to implement successful efforts which will reduce the burden of chronic diseases on the U.S. population. A number of sources of surveillance data-including population surveys, cohort studies, disease registries, administrative health data, and vital statistics-contribute critical information about chronic disease. But no central surveillance system provides the information needed to analyze how chronic disease impacts the U.S. population, to identify public health priorities, or to track the progress of preventive efforts. A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases outlines a conceptual framework for building a national chronic disease surveillance system focused primarily on cardiovascular and chronic lung diseases. This system should be capable of providing data on disparities in incidence and prevalence of the diseases by race, ethnicity, socioeconomic status, and geographic region, along with data on disease risk factors, clinical care delivery, and functional health outcomes. This coordinated surveillance system is needed to integrate and expand existing information across the multiple levels of decision making in order to generate actionable, timely knowledge for a range of stakeholders at the local, state or regional, and national levels. The majority of CRC is diagnosed in those aged 50 or older. Empirical evidence indicates that early detection of CRC can decease mortality. An annual fecal occult blood test (FOBT) and a flexible sigmoidoscopy every 5 years are the current screening guidelines. Recent studies suggest that the sigmoidoscopy may be a sub optimal approach to screening compared to a colonoscopy. Utilization of these tests is very low, especially among those who are 50 or older and most at risk for developing the disease. The purpose of this study was to identify significant predisposing and enabling factors that predict FOBT and colonoscopy use. The theoretical framework for this study was derived from the Health Belief and the PRECEDE-PROCEED Models. Institutional review board approval was obtained before data collection. The instruments used in the study were previously tested for reliability and validity. Employees of a Midwest-based company were surveyed by mail with a response rate of about 51%. Chi square analysis was conducted to which variables were significantly associated with FOBT compliance (having had one in the last year), ever having had an FOBT, and colonoscopy use. Variables significant at p .25 were entered in to logistic regression analysis. Logistic regression was conducted modeling the odds of having an FOBT or colonoscopy. Those who had an FOBT in the last year were older, reported lower barriers, and reported that their providers had recommended the test to them. Caucasians or those who reported that their providers had recommended the test to them were more likely to have had an FOBT sometime in their lives. Predictors of colonoscopy use were increasing age, having higher benefits, knowledge, self-efficacy, and fear, having lower barriers. Results from this study support previous research with FOBT behavior, although there is no previous research with colonoscopy behavior available for comparison. Future research with a community-based sample is recommended. Results may also provide a guide to developing interventions to increase CRC screening behavior.

Book excerpt: "The American Cancer Society estimates that colorectal cancer (CRC) will be the third most common cause of cancer death for both men and women in the U.S. in 2013. The natural history of the disease suggests that longer delays in CRC diagnosis will negatively influence stage at diagnosis and long-term survival. CRC may be diagnosed by screening asymptomatic patients or by evaluation of symptomatic patients. Previous studies investigating the influence of delays on survival or cancer stage at diagnosis have primarily focused on the evaluation of time from first symptom development in symptomatic patients and have demonstrated inconsistent results. For example, among 13 studies published between 1977 and 2006 included in a 2007 systematic review by Ramos and colleagues, 10 found no association between the symptom-to-diagnosis interval (SDI) and survival and the other three found that increased delays resulted in better chances of survival. As for the relationship between SDI and tumor stage, among 18 studies, 11 found no association, four found that shorter delays were associated with an earlier stage at diagnosis and three paradoxically found that a greater delay was associated with an earlier stage at diagnosis. As noted by Ramos et al., the SDI risk function is likely nonlinear and multifaceted, reflecting a complex interaction between tumor biology and location, the clinical course, patient behavior, and the functioning of the healthcare system, and the studies have varied in their methods for adjusting for these confounding factors. Book excerpt: PURPOSE: We conducted a systematic review of five key questions to assist the U.S. Preventive Services Task Force (USPSTF) in updating its 2002 recommendation for colorectal cancer (CRC) screening in average-risk adults aged 50 years or older using home fecal occult blood testing (FOBT), flexible sigmoidoscopy (FS), FS and FOBT, colonoscopy, or double-contrast barium enema (DCBE). Key questions for this updated review primarily focused on evidence gaps from the previous review: 1) the accuracy (one-time test performance characteristics) and potential harms of newer CRC screening tests--fecal immunochemical tests (FIT), high-sensitivity FOBT, fecal DNA testing, and CT colonography (CTC)--as possible substitutes for currently recommended CRC screening modalities; 2) updating of evidence on the impact of CRC screening on mortality and to estimate the accuracy and harms of colonoscopy and FS in the community setting. A concurrent decision analysis done by others addressed screening program performance, and compared the life-years gained using different CRC screening tests, test intervals, and stopping ages. CONCLUSIONS: Based on currently available evidence, refinements in current CRC screening recommendations to add some fecal tests appear warranted. Given potential harms and variation in test accuracy, emphasis on quality standards for implementation of recommended operator-dependent CRC screening tests also appears prudent. Re-evaluation may be appropriate once ongoing RCTs, particularly evaluating CTC, but also evaluating FS and fecal DNA, report their results. Screening for CRC has a rapidly evolving science base, such that guidance may be expected to change as additional research becomes available.
