Book Registries for Evaluating Patient Outcomes

Download or read book Registries for Evaluating Patient Outcomes written by Agency for Healthcare Research and Quality/AHRQ and published by Government Printing Office. This book was released on 2014-04-01 with total page 385 pages. Available in PDF, EPUB and Kindle. Book excerpt: This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.

Download or read book Beyond EHR written by Jeffery P. Daigrepont, EFPM, CAPPM and published by CRC Press. This book was released on 2020-11-29 with total page 198 pages. Available in PDF, EPUB and Kindle. Book excerpt: Today, it is not uncommon for practices and hospitals to be on their second or third EHR and/or contemplating a transition from the traditional on-premise model to a cloud-based system. As a follow-up to Complete Guide and Toolkit to Successful EHR Adoption (©2011 HIMSS), this book builds on the best practices of the first edition, fast-forwarding to the latest innovations that are currently leveraged and adopted by providers and hospitals. We examine the role that artificial intelligence (AI) is now playing in and around EHR technology. We also address the advances in analytics and deep learning (also known as deep structured or hierarchical learning) and explain this topic in practical ways for even the most novice reader to comprehend and apply. The challenges of EHR to EHR migrations and data conversions will also be covered, including the use of the unethical practice of data blocking used as a tactic by some vendors to hold data hostage. Further, we explore innovations related to interoperability, cloud computing, cyber security, and electronic patient/consumer engagement. Finally, this book will deal with what to do with aging technology and databases, which is an issue rarely considered in any of the early publications on healthcare technology. What is the proper way to retire a legacy system, and what are the legal obligations of data archiving? Though a lot has changed since the 2011 edition, many of the fundamentals remain the same and will serve as a foundation for the next generation of EHR adopters and/or those moving on to their second, third, fourth, and beyond EHRs.

Book Key Capabilities of an Electronic Health Record System

Download or read book Key Capabilities of an Electronic Health Record System written by Institute of Medicine and published by National Academies Press. This book was released on 2003-07-31 with total page 36 pages. Available in PDF, EPUB and Kindle. Book excerpt: Commissioned by the Department of Health and Human Services, Key Capabilities of an Electronic Health Record System provides guidance on the most significant care delivery-related capabilities of electronic health record (EHR) systems. There is a great deal of interest in both the public and private sectors in encouraging all health care providers to migrate from paper-based health records to a system that stores health information electronically and employs computer-aided decision support systems. In part, this interest is due to a growing recognition that a stronger information technology infrastructure is integral to addressing national concerns such as the need to improve the safety and the quality of health care, rising health care costs, and matters of homeland security related to the health sector. Key Capabilities of an Electronic Health Record System provides a set of basic functionalities that an EHR system must employ to promote patient safety, including detailed patient data (e.g., diagnoses, allergies, laboratory results), as well as decision-support capabilities (e.g., the ability to alert providers to potential drug-drug interactions). The book examines care delivery functions, such as database management and the use of health care data standards to better advance the safety, quality, and efficiency of health care in the United States.

This book was released on 2010-05-01 with total page 140 pages. Available in PDF, EPUB and Kindle. Book excerpt: Despite the significant benefits of electronic medical records, organizations continue to struggle with successful technology adoption. Beyond Implementation examines the primary reason for poor and failed EMR adoption, explores real-world results from large healthcare organizations, and reveals a new approach for successful adoption and lasting value. The authors, Dr. Heather Haugen and Dr. Jeffrey Woodside, have witnessed the outcomes of poor adoption and are committed to helping organizations successfully adopt an EMR system. Through actual case studies and research, the book investigates the barriers that keep physicians from making EMR part of their routines. Download or read book Electronic Medical Records written by Neil S. Skolnik and published by Springer Science & Business Media. This book was released on 2010-10-20 with total page 161 pages. Available in PDF, EPUB and Kindle. Book excerpt: Physician adoption of electronic medical records (EMRs) has become a national priority. It is said that EMRs have the potential to greatly improve patient care, to provide the data needed for more effective population management and quality assurance of both an individual practice’s patients and well as patients of large health care systems, and the potential to create efficiencies that allow physicians to provide this improved care at a far lower cost than at present. There is currently a strong U.S. government push for physicians to adopt EMR technology, with the Obama administration emphasizing the use of EMRs as an important part of the future of health care and urging widespread adoption of this technology by 2014. This timely book for the primary care community offers a concise and easy to read guide for implementing an EMR system. Organized in six sections, this invaluable title details the general state of the EMR landscape, covering the government’s incentive program, promises and pitfalls of EMR technology, issues related to standardization and the range of EMR vendors from which a provider can choose. Importantly, chapter two provides a detailed and highly instructional account of the experiences that a range of primary care providers have had in implementing EMR systems. Chapter three discusses how to effectively choose an EMR system, while chapters four and five cover all of the vital pre-implementation and implementation issues in establishing an EMR system in the primary care environment. Finally, chapter six discusses how to optimize and maintain a new EMR system to achieve the full cost savings desired. Concise, direct, but above all honest in recognizing the challenges in choosing and implementing an electronic health record in primary care, Electronic Medical Records: A Practical Guide for Primary Care has been written with the busy primary care physician in mind.

Book Clinical Practice Guidelines We Can Trust

Download or read book Clinical Practice Guidelines We Can Trust written by Institute of Medicine and published by National Academies Press. This book was released on 2011-06-16 with total page 217 pages. Available in PDF, EPUB and Kindle. Book excerpt: Advances in medical, biomedical and health services research have reduced the level of uncertainty in clinical practice. Clinical practice guidelines (CPGs) complement this progress by establishing standards of care backed by strong scientific evidence. CPGs are statements that include recommendations intended to optimize patient care. These statements are informed by a systematic review of evidence and an assessment of the benefits and costs of alternative care options. Clinical Practice Guidelines We Can Trust examines the current state of clinical practice guidelines and how they can be improved to enhance healthcare quality and patient outcomes. Clinical practice guidelines now are ubiquitous in our healthcare system. The Guidelines International Network (GIN) database currently lists more than 3,700 guidelines from 39 countries. Developing guidelines presents a number of challenges including lack of transparent methodological practices, difficulty reconciling conflicting guidelines, and conflicts of interest. Clinical Practice Guidelines We Can Trust explores questions surrounding the quality of CPG development processes and the establishment of standards. It proposes eight standards for developing trustworthy clinical practice guidelines emphasizing transparency; management of conflict of interest ; systematic review-guideline development intersection; establishing evidence foundations for and rating strength of guideline recommendations; articulation of recommendations; external review; and updating. Clinical Practice Guidelines We Can Trust shows how clinical practice guidelines can enhance clinician and patient decision-making by translating complex scientific research findings into recommendations for clinical practice that are relevant to the individual patient encounter, instead of implementing a one size fits all approach to patient care. This book contains information directly related to the work of the Agency for Healthcare Research and Quality (AHRQ), as well as various Congressional staff and policymakers. It is a vital resource for medical specialty societies, disease advocacy groups, health professionals, private and international organizations that develop or use clinical practice guidelines, consumers, clinicians, and payers.
