Book excerpt: A 73-year-old female patient status post right total knee arthroplasty was seen for physical therapy in an acute care setting for a total of four sessions over the course of three days beginning on the same afternoon of surgery. Treatment was provided by a student physical therapist under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with goniometry, manual muscle testing, the Numeric Pain Rating Scale, the Wells Score for DVT, the Boston University AM-PAC Basic Mobility Inpatient Short Form, and a plan of care was established. The main goals for the patient were to improve strength and range of motion (ROM), decrease pain and improve functional mobility, including bed mobility, transfers, ambulation and stair climbing. The main interventions used were therapeutic exercise and task specific training. The patient improved strength and ROM, decreased pain, and improved independence in bed mobility, transfers, gait and stair training. The patient was discharged home with a home exercise program and follow up with outpatient physical therapy.

Treatment was provided by a student physical therapist under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with goniometry, manual muscle testing, Wells criteria, the numeric pain rating scale, the Timed Up and Go test, the 6-minute walk test, California Department of Health Care Services assistance levels, Stanbridge Colleges's Graded Posture Movement Ability of Individual, and patient report. A plan of care was then established. Main goals for the patient were to improve level of independence, and return home to continue a morning walking program with her sister. Additional goals included improving lower extremity range of motion, strength, static and dynamic standing balance, functional and bed mobility, walking endurance, transfer and ambulation ability, and decreasing pain. Main interventions used were goal-oriented, functional, and impairment-based, with use of progressive overload, and included strengthening and range of motion exercises, and gait and functional training. The patient improved range of motion, strength, balance, walking endurance, functional mobility, and independence with activities of daily living. The patient was discharged home with a home exercise program, and scheduled to receive home-health physical therapy.

Treatment was provided by a student physical therapist under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with the Timed Up and Go test to measure functional mobility and assess fall risk, the Oxford Knee Score to measure knee function and knee pain, and the Wells Clinical Decision Rule to assess the likelihood of the presence of a deep vein thrombosis. A plan of care was established to fit the individual's impairments determined by the initial evaluation. The main goals for the patient were to improve strength, endurance, gait quality, functional mobility, functional independence, and reduce fall risk. Main interventions used were over-ground gait training, progressive strength training, and functional task-specific training. Throughout the episode of care the patient improved strength, endurance, gait quality, functional mobility, and functional independence. All goals were met in these areas of focus, except the patient remained at high fall risk based on the Timed Up and Go test findings. The patient was discharged home with a home exercise program, a recommendation of supervision as needed, and with home health physical therapy and nursing.

Book excerpt: Postoperative pain is a major concern to patients undergoing surgical procedures but little research has been conducted on pain management after hospital discharge for orthopaedic patients. Since pain medication is a key component of pain management, it is important to study medication usage from a patient's perspective, for greater patient--health care provider concordance. A qualitative descriptive approach was taken to investigate the experience of 14 participants with managing pain at home immediately after total knee arthroplasty. Most participants limited their consumption and weaned themselves off prescription analgesics and used over-the-counter pain medications. The participants adapted their regimens in response to several factors and generally were content to self - manage their pain but required access to professional support. The study suggests that when developing postoperative pain management plans, health care providers may need to increase the time they spend addressing patients' concerns and considering patients' preferences.

Specifically, postoperative pain after total knee arthroplasty remains one of the most important challenges facing patients undergoing this surgery. Pain and comfort management continue to be overlooked and inadequately managed (McCaffery, Herr & Pasero, 2011; Pasero et al., 2009). Despite the availability of numerous effective analgesics and new drug administration technologies, research studies continue to show suboptimal pain management (ASPAN, 2013). A major focus of postoperative nursing care for patients following total joint replacement surgery is pain management. Although education of today’s nurse will require some emphasis on technological advances, fundamental caring practices cannot be neglected. Interventions focusing on the comfort needs of patients and families are entirely within the realm of nursing practice, and are proposed to be strongly related to quality of care and patient satisfaction (Kolcaba, 2012). The purpose of this research study was to examine the relationship between pain intensity, comfort level and patient satisfaction of orthopedic patients following knee surgery on postoperative day one (the first day after surgery). The comfort theory was used as the theoretical foundation for this study. This study was conducted as descriptive/correlational study in which the relationship among variables (postoperative pain, comfort and patient satisfaction) was examined. A convenience sample of 40 male and female adult patients admitted to the hospital for elective total knee replacement /arthroplasty surgery beginning October 2014. The 0-10 Numerical Rating Scale (NRS-11) was used to measure pain intensity. The Perianesthesia Comfort Questionnaire was used to measure comfort. Data collection to assess patient satisfaction was based on questions from the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) Survey and Press Ganey Survey. The results of this study did not reveal statistically significant relationships among patient satisfaction, comfort, and pain intensity. Perhaps this finding is due to the high patient satisfaction scores. Nurses play a key role in the management of pain for patients having knee replacement surgery. They are the primary group of health care professionals responsible for the ongoing assessment, monitoring of patients and initiating interventions that help assure patients’ pain are appropriately managed. To adequately study this phenomenon, replication of the study across different hospital systems using a large sample size with more variability in satisfaction are needed.
