Book excerpt: Stresses and strain in the bone surrounding a dental implant are of interest regarding the study of bone healing following the dental implantation procedure. Techniques for mapping strain in bone have been developed for physical experimentation, but computational methods for finding these strain maps are more desirable for their flexibility in dealing with changing physical properties, loading conditions, and geometries.This project first aims to develop a finite element model of a surgical dental implant and two adjacent teeth. Two implants from a cadaver are scanned by a micro-CT scanner and those images are used to create two models. The first is an implant for tooth 23 and the second for tooth 26. After building the model, the implant is loaded with a pressure and the resultant stress and strain in the teeth and surrounding bone are calculated using the finite element method. The effects of element size are studied. Various boundary conditions are explored. The parameters of the model are then modified to simulate various conditions including differences in the stiffness of the cortical bone, the teeth dentin, and the titanium implant. These differences in material properties arise from an interval of stiffness values in the literature for bone and teeth, and the unknown composition of the implant.The computational model is compared to a physical experiment during which the same implants were loaded by a downward force of 100 N. In the experiment, the implant-tooth complexes were scanned again after loading, and digital volume correlation methods were measured to calculate the strain [1]. The goal for this project is to evaluate the finite element method as a means for studying the mechanical effects of loaded implants in the surrounding bone. The methods developed in this project might be used in future studies of implant-bone complexes.

Book excerpt: BACKGROUND: Knowledge in biomechanics has contributed for better understanding of peri-implant bone behavior under functional loading. Finite Elements Analysis (FEA) is an important tool for biomechanical studies, considering the impossibility of performing u201cin vivou201d mechanical tests. However, reliability and clinical applicability depends on creation of a computational model that reproduces the anatomic site and the mechanical properties of materials involved, including bone tissue.AIM/HYPOTHESIS: Based on the hypothesis that most of biomechanics studies on dental implant has used unreliable simplified computational models, this literature review with systematic search aimed to investigate how accurate are the mechanical proprieties of human jawbone tissue currently used in studies of FEA involving dental implants.MATERIAL AND METHODS: The systematic search was performed according to the PICO strategy (Population, Intervention, Comparator and Outcome) in three databases (Medline/Pubmed, Scopus and Web of Science). Screening of 1580 titles and abstracts identified relevant publications. A protocol for full text reading was developed from the exclusion criteria, and was applied to identify included publications. Data extraction included the identification of bone anatomic region (maxilla or mandible), the method to obtain the computational models, software used and the bone's mechanical properties available (type of anisotropy, elasticity modulus, Poisson ratio, shear modulus and other possible properties described). When the mechanical properties were obtained from the literature, the publications used as reference were recorded and included for evaluation and possible data extraction.RESULTS: The 28 studies included in this review found different values for each load direction, contrary to the studies that considered jawbone as isotropic, homogenous and linearly elastic, which used a constant value for elasticity modulus and Poisson ratio. Most of publications identified as references for jawboneu2019s mechanical properties were u201cin vitrou201d studies, and they obtained the mechanical properties values from mechanical tests performed on peaces of mandible or maxilla, taken from cadavers. Some of them used fresh bone and others frozen bone, which may have had probably some influence on results.CONCLUSION AND CLINICAL IMPLICATIONS: Some researchers have put effort on development of a more accurate computational model of maxilla and mandible for use in implant research. Technologic advance has allowed the construction of models from scanners, tomographic and microtomographic images, and easier mesh generation for FEA. The mechanical properties of bone tissue have been reviewed and anisotropic characteristics have been gradually incorporated into the computational mechanical tests.
