Book excerpt: Social reciprocity deficits are a core feature of the autism spectrum disorders (ASD) and a major source of impairment regardless of cognitive or language ability (Carter, Davis, Klin, & Volkmar, 2005). Since these impairments do not naturally remit with age, it is critical to intervene as early as possible to offset potential risk factors (Tantum, 2003). Group training approaches provide children with teaching opportunities with other children and allow for the direct instruction of skills within a structured environment (Bellini, Peters, Benner, & Hopf, 2007). Research in social skills group research has increased, but several questions remain. This study evaluated the outcome of a social skills curriculum for 37 children between the ages of 8 to 14 with a diagnosis of ASD within two different treatment contexts, the camp and clinic model. The camp model simulates a natural setting in which children with ASD spend 5 hours each day for 10 days where social skills are taught through engaging activities and interactions with peers both typical and with ASD. The clinic model, on the other hand, is a one hour a week session spread over 10 to 12 weeks where social skills are taught and practiced while parents observe through a one-way mirror and are trained on the intervention methods. Both clinic and camp model treatments are then compared with a third group who experience both treatments within the clinic and camp setting. Results show that the combined context had the highest treatment effects, followed by the camp model and, finally, the clinic model. Analysis of covariance did not indicate the groups differ from each other significantly in terms of treatment gains. The implications of these results are discussed in terms of translation of research into clinical practice, use of appropriate outcome measures, and generalization of skills through parent training and utilization of training programs within the natural context. While the intervention and results are promising, replication with larger samples and use of a control group are needed.

Research has shown that social skills training provides lasting improvement in social interactions. However, many factors can hinder the availability of intervention groups outside of major metropolitan areas. Individual online social skills interventions have been shown to translate to increases in everyday social skills, and while further investigation is required, current literature suggests that there is great potential for live, interactive online social skills groups to provide similar benefits to in-person intervention groups. Thus, online groups may be one solution to the barriers to accessing available resources. This study sought to explore the feasibility of conducting a live, interactive online social skills group by comparing two groups using the same curriculum; one in-person group that met in a lab on a local college campus, and one piloted online group that met through Zoom, a video conference platform, each for 14 weeks. Each group also participated in person in social activities on campus to gather preliminary generalization data. The online intervention was acceptable to therapists and participants, and a group of therapists who implemented both delivery models reported some advantages and disadvantages of an online format for social skills, expressing a preference for in-person delivery model when possible, but acceptability of online delivery where in-person groups are not available. The online model showed somewhat lower levels of participation, but slightly higher attendance rates than the in-person model. Understanding both the advantages and disadvantages to online social skills, clinicians can utilize the method the best fits their needs.

Book excerpt: This study examines the differential impact a traditional social skills curriculum, SkillStreaming, and a novel, drama-based social skills intervention, the Hunter Heartbeat Method (HHM), on the core social skills deficits associated with autism spectrum disorder (ASD) and the brain-basis of these deficits. Forty children aged 8-14 years with ASD were recruited to participate in a 12-week social skills intervention. Participants were randomly assigned to receive drama-based or traditional social skills intervention once weekly. Previous research on SkillStreaming and HHM in children with ASD had reported improvement in behavioral measures of social functioning from pre- to post-intervention. Despite evidence of treatment response for both interventions, clear differences between these two types of social skills intervention exist. SkillStreaming is a highly structured, curriculum-based intervention during which specific social skills, such as making a friend or having a conversation are explicitly taught via didactic instruction, modeling, and rehearsal. Conversely, in the HHM, although core elements of modeling and repeated practice with feedback are used, there are no “skills” taught, rather, children learn drama-games that implicitly target core deficits associated with ASD (e.g., eye contact, facial emotion recognition, integration of speech and gesture). Research on drama-based interventions for children with ASD is an emerging literature; previous research has indicated that this treatment is well liked by children and, like traditional treatments, is associated with measurable improvement from pre- to post-intervention. Little is known, however, about how differences in treatment modality (didactic versus experiential) and skills taught (higher-level versus foundational) impact skill acquisition and generalization. It is possible that these key differences impact the neurological substrate of social learning, which may have downstream consequences for skill acquisition and generalization. This study aimed to address this important question by investigating not just behavioral measures of social functioning (including direct measures of pragmatic language and parent-completed rating scales), but also brain-based measures. In addition to direct child measures and parent rating scales, participants completed functional magnetic resonance imaging (fMRI) scans before and after treatment. During fMRI scans, participants completed a facial emotion recognition (FER) task to determine differential treatment impact on the amygdala and fusiform gyrus, key brain regions used in FER in typical development. FER is a critical social-cognitive task that is impaired in children with ASD. Results of this study indicate that participants receiving the HHM experienced significantly greater improvement in core social skills deficits associated with ASD and increased activity in the amygdala and fusiform gyrus from pre- to post-intervention, indicative of increased signaling in these critical regions mediating FER in typical development. Participants in the SkillStreaming group, however, evidenced changes in activation in the lingual gyrus and other cortical regions associated with verbally mediated problem solving. Results indicate that despite comparable improvement on behavioral measure of facial emotion recognition, the neurological underpinnings of social-cognitive gains depended on treatment modality. This is the first study of its kind and although considerably more research is needed in this area, findings offer some insight regarding social learning in ASD, which may have implications for future research and practice regarding social skills intervention in this population.
