Book excerpt: Excerpt from A Clinical Manual of Mental Diseases The generous reception accorded the first edition of this manual justifies the inference that a purely clinical presentation of mental disease fulfils a distinct want. The practising physician has too often looked upon insanity as a subject unattractive and obscure, difficult and abstruse, and with which, because of the speculative and metaphysical character of its theories and explanations, he has no immediate concern. The truth is, however, that when viewed from the standpoint of internal medicine it is brought into close and intimate relations with the latter. This becomes especially evident when we realize that many cases of mental disease have their origin in infections and intoxications, while others present problems which are essentially those of disorders of metabolism. Many of the latter have to do with profound nutritional disturbances which have their origin in defensive reactions of the organism to intoxications. Such intoxications may arise from without or, it may be, from within the organism. Among the latter are toxic states due to abnormalities of the various glands of internal secretion or of other structures. In other cases of mental disease, again, the physician has to do with aberrancies and arrests of development, with peculiarities of structure and mental organization often hereditary; and in each of these there are, again, the added and largely unsolved problems of toxic metabolism. A book must stand or fall very much on its own merits, and an apology will not help it. I am not concerned, therefore, to offer an apology for the appearance of this work; but a certain explanation is due to those for whom it has been written. A search may be made in its pages for pathological teaching. Of such there is none. The pathology of insanity has yet to be written. We have had to retrace our steps in this respect, and begin de novo with what in bacteriology are called 'control experiments'; in other words, we required to be certain of normal data before we could determine what are abnormal. Dr. Robert Hutchison obtained from the post-mortem room of the Royal Infirmary, Edinburgh, reputedly normal brains. I refer to the asylum system of treating the insane. Before a patient can be admitted to one of these institutions the disease process must be fully developed, with the result that the earlier stages can only be studied on very rare occasions. In such fully developed cases the mental aspect of the disease attracts attention by the prominence and bizarre character of the mental symptoms, and so it has happened that from distant times up to the present day the tendency to confuse the study of pathological psychology and the under lying physical disease has done much to prevent the advance of knowledge. When psychology is divorced from psychiatry, and the study of psychiatry is prosecuted along the lines of advance in general medicine, our knowledge of mental diseases cannot fail to be added to. The matter contained in the following pages is based on work so conducted: psychology is omitted; pathology, in so far as the changes in the brain cells and nerve fibres are concerned, is practically ignored, because until we know what the causes of such changes are we are.
