Although human beings have universally pondered their mortality, 21st century Americans have unique challenges when preparing for their own personal dying experiences. Of the 2.6 million Americans who died in 2013, over 1.9 million of these deaths, or 73.3%, were experienced by Americans aged 65 and older (http://www.cdc.gov/nchs/data). American senior citizens, referred to as a demographic tidal wave, will be in the unique position to see death approaching, because the majority of them will die of lingering, chronic diseases. The Institute of Medicine (IOM) (2015) concluded that 25% of adult Americans have not considered their end-of-life wishes. For those Americans who have identified end-of-life preferences, few have recorded them in the form of advance health care directives. Given an option, the majority of Americans would prefer to die in their own homes, surrounded by the people, pets and objects they love, but the reality is far from that experience. There is a growing perception that the end-of-life experience for 21st century American senior citizens is incongruent with their end-of-life wishes. The purpose of this grounded theory study was to understand current end-of-life issues for senior citizens, identify the elements that comprise an optimal end-of-life experience, and to construct a theory explaining the optimal end-of-life experience for this demographic. Findings from this study led to the construction of a theory that suggests the hospice industry is in the best position to facilitate an optimal end-of-life experience for senior citizens through training medical professionals on end-of-life issues (including the benefits of palliative and hospice care), providing direct care to those who are dying and offering direct support and training to family caregivers. Every human being will face his or her own dying experience at some time, and potentially be involved in a loved one's dying experience. This research is significant because it ultimately impacts all members of society.
