Book excerpt: One in five people in the United States had a sexually transmitted infection (STI) on any given day in 2018, totaling nearly 68 million estimated infections. STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.

It was 1988 when most HIV/AIDS education was about epidemiology and the function of T-cells that the CFLE created its groundbreaking first edition of Teaching Safer Sex. Ten years later, many of the innovative strategies from that manual were classics in the field and had been incorporated into hundreds of curricula that aimed to develop the motivation, knowledge, comfort and skills essential for safer sex behaviors. It was time for a second edition, and The NEW Teaching Safer Sex aimed to expand the scope of safer sex education to include the social context of people's sexual behavior. Paulo Freire's ideas put forward in The Pedagogy of the Oppressed were important in the development of the new manual, which recognized that in a society so dangerously ambivalent about teaching its young people about their sexual safety, they needed to learn about the powerful societal, as well as personal, barriers to healthy sexual behavior. Twenty lessons were designed to promote critical consciousness about social messages as well to create a climate where communication about sexuality is normal and the use of safer sex is the expected behavior.

A leading writer on the cultural politics of AIDS, Cindy Patton guides us through the thicket of mass-media productions, policy and public health enterprises, and activist projects as they sprang up to meet the challenge of the epidemic, shaping the nation's notion of what safe-sex is and who ought to know what about it. There is the official story, and then there is another, involving local groups and AIDS activists. Going back to early government and activist attempts to spread information, Patton traces a slow separation between official advice and that provided by those on the front lines in the battle against AIDS. She shows how American anxieties about teen sex played into the nation's inadequate education and protection of its young people, and chronicles the media's attempts to encourage compassion without broaching the touchy subject of sex or disrupting the notion that AIDS was a disease of social and sexual outcasts.
