Book excerpt: Despite progresses made over the last several decades, the prevalence of child malnutrition re-mains alarmingly high. About 149 million children under the age of five years old were stunted (too short for their age) in 2018, of which 55% and 39% lived in Asia and Africa, respectively. Malnourished children, especially stunted ones, may never achieve their full cognitive and non-cognitive potential with implications for their educational and labor market performance among other things. Malnutrition results from several interlinked factors operating at child, parental, household, and landscape level such as inadequate maternal nutrition before and during pregnancy and at the time of lactation, suboptimal breastfeeding practices, lack of nutritious complementary foods, and unhealthy living environments. This study analyzes the correlates of child undernutrition in rural Africa South of the Sahara (SSA) – a region with the least progress in tackling undernutrition.

Updated estimates show the number of people who suffer from hunger has been growing over the past three years, returning to prevailing levels from almost a decade ago. Although progress continues to be made in reducing child stunting, over 22 percent of children under five years of age are still affected. Other forms of malnutrition are also growing: adult obesity continues to increase in countries irrespective of their income levels, and many countries are coping with multiple forms of malnutrition at the same time – overweight and obesity, as well as anaemia in women, and child stunting and wasting.

Book excerpt: Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.

But the two objectives cannot be considered in isolation: they need to be jointly tackled through an integrated strategy. It examines the potential impact of climate change and climate policies on poverty reduction. It also provides guidance on how to create a “win-win†? situation so that climate change policies contribute to poverty reduction and poverty-reduction policies contribute to climate change mitigation and resilience building. The key finding of the report is that climate change represents a significant obstacle to the sustained eradication of poverty, but future impacts on poverty are determined by policy choices: rapid, inclusive, and climate-informed development can prevent most short-term impacts whereas immediate pro-poor, emissions-reduction policies can drastically limit long-term ones.

The 2015 Global Hunger Index (GHI) shows that the level of hunger in developing countries as a group has fallen by 27 percent. Yet the state of hunger in the world remains serious. This marks the tenth year that IFPRI has assessed global hunger using this multidimensional measure. This report’s GHI scores are based on a new, improved formula that replaces the child underweight indicator of previous years with child stunting and child wasting. This change reflects the latest thinking on the most suitable indicators for child undernutrition, one of three dimensions of hunger reflected in the GHI formula. Across regions and countries, GHI scores vary considerably. Regionally, the highest GHI scores, and therefore the highest hunger levels, are still found in Africa south of the Sahara and South Asia. Despite achieving the largest absolute improvements since 2000, these two regions still suffer from serious levels of hunger. Levels of hunger are alarming or serious in 52 countries. Most of the eight countries with alarming GHI scores are in Africa south of the Sahara. While no countries are classified in the extremely alarming category this year, this high level of hunger could still exist. Due to insufficient data, 2015 GHI scores could not be calculated for places that recently suffered from high levels of hunger, including Burundi, Comoros, Democratic Republic of the Congo, Eritrea, Somalia, South Sudan, and Sudan. 2015 Global Hunger Index Interactive App: http://ghi.ifpri.org

The fundamental methods of bias analysis in epi- miology have been well described for decades, yet are seldom applied in published presentations of epidemiologic research. More recent advances in bias analysis, such as probabilistic bias analysis, appear even more rarely. We suspect that there are both supply-side and demand-side explanations for the scarcity of bias analysis. On the demand side, journal reviewers and editors seldom request that authors address systematic error aside from listing them as limitations of their particular study. This listing is often accompanied by explanations for why the limitations should not pose much concern. On the supply side, methods for bias analysis receive little attention in most epidemiology curriculums, are often scattered throughout textbooks or absent from them altogether, and cannot be implemented easily using standard statistical computing software. Our objective in this text is to reduce these supply-side barriers, with the hope that demand for quantitative bias analysis will follow.

This book provides a solid documentary base that can be used to develop an agenda to guide research and health policy formulation on female health--both for Sub-Saharan Africa and for other regions of the developing world. Despite progress made, the level of hunger in the world is still “serious,” with 805 million people continuing to go hungry, according to estimates by the Food and Agriculture Organization of the United Nations. The global average obscures dramatic differences across regions and countries. Regionally, the highest GHI scores—and therefore the highest hunger levels—are in Africa south of the Sahara and South Asia, which have also experienced the greatest absolute improvements since 2005. South Asia saw the steepest absolute decline in GHI scores since 1990. Progress in addressing child underweight was the main factor behind the improved GHI score for the region since 1990.
