In addition, the case history may be confusing as it often includes somatic as well as psychological elements. Moreover, all generally used diagnostic tests are severely limited. Facing these difficulties the clinician has two options. The first is to systematically perform an extensive otoneurological workup including audiological, vestibular and radiological tests. In this case, he risks to perform unnecessary examinations, or to have difficulties in interpreting inconsistent results which reflect the limitations of the diagnostic tests. On the other hand, the clinician may choose to limit the investigation to the minimum, risking to miss information indispensable to the diagnosis. In this controversial issue, the experts reviewing the existing tests from the most simple to the most sophisticated consider that the case history, a careful physical examination and some simple tests remain essential elements of the diagnosis, on condition that the limitations of these tests are taken into consideration. The work in this thesis increases the power of vestibular testing for both diagnostic purposes and its use in helping us understand disease pathophysiology. Although the methodologies used in this thesis could be used to distinguish between many different groups, this work concentrated on developing and applying methods to discriminate between: peripheral vs. non-peripheral disorders, bilateral vs. unilateral peripheral disease, and right vs. left unilateral peripheral disease. The increase in performance was accomplished by incorporating a priori information, age-adjusting data, and applying physiologically based models of vestibular function to reduce the data. Finally, by using multivariate statistical techniques, such as quadratic classification and logistic regression, more objective, precise, and accurate test interpretation procedures were created. Results showed up to a 72% increase in performance over current methods and modest improvement in all cases.
