Book excerpt: Introduction : la ventilation non invasive (VNI) est fréquemment utilisée pour la prise en charge de l'insuffisance respiratoire aiguë, notamment chez la personne âgée. Son utilisation est peu décrite dans les services d'accueil des urgences (SAU). Matériels et méthodes : nous avons réalisé une étude descriptive au SAU du CHU de Caen durant une période de 16 semaines. Les patients inclus étaient les patients âgés de plus de 75 ans admis au SAU pour une dyspnée ou une détresse respiratoire aiguë, à l'exclusion des patients qui n'ont pas été hospitalisés à la suite des urgences. Le critère de jugement principal était la mortalité intra-hospitalière à 15 jours. Le critère secondaire était la survenue d'une ré-aggravation en cours d'hospitalisation. Résultats : 117 patients ont été inclus dont 31 dans le groupe « avec » VNI. Les caractéristiques générales dans les 2 groupes étaient différentes concernant les critères de gravité cliniques à l'arrivée (polypnée, pH, présence de signes de lutte), les caractères gériatriques et comorbidités sont par ailleurs comparables. La mortalité était de 35,5 % vs 12,7% des patients « avec » VNI et « sans » VNI respectivement. La différence est significative dans les 2 groupes HR 3,21 [1,39-7,40], p = 0,006. L'analyse révélait la présence de signes de lutte comme facteur prédictif de mortalité intra-hospitalière [HR 4,52 [1,58- 12,92], p = 0,005]. Les résultats sur le critère secondaire montrent notamment une ré-aggravation en cours d'hospitalisation chez 45% des patients « avec » VNI vs 23% « sans » VNI. La différence est significative (p = 0,009). Les analyses multivariées mettent en évidence que la survenue d'une ré-aggravation est importante chez les patients qui ont déjà été hospitalisés une fois dans l'année [OR 9,23 [2,69-30,26], p

Book excerpt: La ventilation non invasive aux urgences au cours des détresses respiratoires aiguës d’étiologie inconnue, fréquentes chez les sujets âgés, est un sujet encore peu étudié. Par contre, la littérature est riche de travaux sur la VNI réalisés dans des pathologies précises comme l’œdème aigu pulmonaire, la BPCO. L’objectif principal de ce travail a été d’accomplir une analyse de la pratique de la ventilation non invasive, lorsque se présente au SAU, un sujet âgé en détresse respiratoire aiguë dont l’étiologie est indéterminée, du fait du manque de renseignements sur ses antécédents. Les résultats de notre étude, comme dans la littérature montrent une diminution de l’incidence de l’intubation, de la durée d’hospitalisation et une amélioration, essentiellement de la fréquence respiratoire puis de la gazométrie artérielle tel le pH et la PaCO2. On s’aperçoit également que la VNI est plus efficace dans l’OAP et l’exacerbation de BPCO que dans la pneumonie. On a constaté que l’initiation d’une VNI était faisable aux urgences, en salle de déchocage, chez le sujet âgé, qu’elle ne nécessitait ni personnel surnuméraire ni matériel spécifique. Enfin cette étude met l’accent sur les indications de la VNI qui restent à préciser et sans doute à élargir, sur l’établissement d’un score prédictif d’amélioration, et finalement de la place de la VNI en pré hospitalier.

Book excerpt: La ventilation non invasive (VNI) est une technique de ventilation artificielle au cours de la prise en charge des insuffisances respiratoires aiguës et est devenue la technique de première intention dans les services de Réanimation en France. Nous rapportons un audit clinique sous forme d’évaluation des pratiques professionnelles de l’utilisation de la VNI en Réanimation médicale au CHU de Caen avec l’étude de 20 dossiers médicaux pris au hasard avant puis après interventions. Ces interventions consistaient en la rédaction de protocoles médical (feuille de prescription dédiée à la VNI) et paramédical (feuille de surveillance spécifique) respectant les recommandations des conférences de consensus et en la formation des personnels (médicaux et paramédicaux) du service. Les résultats observés sont favorables à l’utilisation de protocoles dédiés à la VNI. Plusieurs différences significatives sont rapportées : discussion du rapport bénéfice/risque de l’utilisation de la VNI reporté dans le dossier du patient, conditions de mise en œuvre de la VNI conforme au protocole, prescription des surveillances et report de ces surveillances, attention portée aux effets indésirables de la VNI avec report d’une stratégie de prévention et des raisons éventuelles de l’échec de la technique. Ainsi, la bonne pratique de la VNI en Réanimation est favorable à l’utilisation de protocoles dédiés, et ces protocoles pourraient être employés dans la pratique quotidienne au sein du service où l’étude a été menée, et même dans les différents services de Réanimation de l’établissement hospitalier.

Despite its importance in virtually all types of acute severe illness, hypoxaemia is often not well recognized or well managed, more so in settings where resources are limited. Oxygen therapy remains an inaccessible luxury for a large proportion of severely ill children admitted to hospitals in developing countries. This is particularly true for patients in small district hospitals, where, even if some facility for delivering oxygen is available, supplies are often unreliable and the benefits of treatment may be diminished by poorly maintained, inappropriate equipment or poorly trained staff with inadequate guidelines. Increasing awareness of these problems is likely to have considerable clinical and public health benefits in the care of severely ill children. Health workers should be able to know the clinical signs that suggest the presence of hypoxaemia and have more reliable means of detection of hypoxeamia. This can be achieved through more widespread use of pulse oximetry, which is a non-invasive measure of arterial oxygen saturation. At the same time oxygen therapy must be more widely available; in many remote settings, this can be achieved by use of oxygen concentrators, which can run on regular or alternative sources of power. Having effective systems for the detection and management of hypoxaemia are vital in reducing mortality from pneumonia and other severe acute illnesses. Oxygen therapy is essential to counter hypoxaemia and many times is the difference between life and death. This manual focuses on the availability and clinical use of oxygen therapy in children in health facilities by providing the practical aspects for health workers, biomedical engineers, and administrators.
