Book excerpt: La ventilation non invasive (VNI) est actuellement indiquée dans un nombre croissant de pathologies respirtoires (Insuffisance Respiratoire Chronique Restrictive, Syndrome Obésité Hypoventilation, Broncho-Pneumopathie Chronique Obstructive). De ce fait le nombre de nouveaux patients appareillés en VNI augmente constamment. Un des problèmes essentiels de la VNI reste le choix de l'interface de ventilation. De nombreux modèles standards existent mais malgrè cela certains patients gardent des difficultés pour la ventilation, à cause de leur masque. Il en résulte des problèmes de tolérance et d'observance. Il parait donc intéressant de pouvoir fabriquer un masque nasal pour la VNI de façon personnaliésée, en faisant du sur-mesure. Ceci pourrait améliorer la faisabilité d'une telle pratique. Le masque EMMA est un masque nasal adapté à la VNI fabriqué sur-mesure à partir de la photographie d'un patient prise dans des conditions définies. Le visage du patients est reconstitué en 3 dimensions et une coque personnallisée est conçue pour chaque patient. Des essais préalables à notre étude ont débuté en juin 2003, jusqu'à la mise au point d'une version aboutie du masque EMMA en avril 2004. Un second travail préliminaire réalisé sur 11 patients dans l'unité de surveillance continue de Pneumologie de l'Hôpital de Bois-Guillaume d'avril 2004 à novembre 2005 a permis la mise au point finale du masque. C'est ce masque qui a été testé dans notre étude. Il s'agit d'une étude prospective menée dans l'unité de surveillance continue de Pneumologie de l'Hôpital de Bois-Guillaume sur des patients ventilés au masque nasal de janvier à mai 2006. Le masque EMMA est comparé au masque habituel des patients. Dix patients ont été inclus (6 hommes / 4 femmes). Cinq patients ont une insuffisance respiratoire chronique obstructive, deux patients une insuffisance respiratoire chronique restrictive, un patient un overlap syndrome et deux patients une syndrome obésité hypoventilation apnéique. La tolérance et l'efficacité du masque EMMA dont bonnes. L'efficacité des deux masques n'est pas statistiquement différente. Trois des dix patients auraient souhaité garder le masque EMMA pour le retour au domicile. Le masque EMMA semble être aussi efficace que les masques habituels des patients. Sa tolérance est globalement bonne. Cette interface cutanée pourrait trouver sa place dans l'arsenal des masques de VNI. Il faudrait l'étudier sur une plus large population pendant de plus longues périodes pour mieux préciser s'il existe des indications plus fines et définir sa place dans la stratégie thérapeutique des patients venilés par VNI.

The excep tional achievements of mountaineers who have climbed nearly all peaks over 8,000 m without breathing equipment raise the question of maximal adaptation ca pacity of man to low oxygen pressures. More importantly, the increase in tourism in the Andes and the Himalayas brings over 10,000 people to sites at altitudes above 4,000 and 5,000 m each year. At such heights several kinds of high alti tude diseases are likely to occur, and these complications require detailed medical investigations. Medical authorities need to inform both mountaineers and tourists as to how great a physical burden can be taken in the mountain environment without risk to health. Physicians need to know what kind of prophylaxis is to be employed at high altitudes to prevent the development of diseases and what therapeutic measures should be used once high altitude diseases have occurred. Moreover, the physical condition of the indigenous population living at higher altitudes such as the Andes and the Himalayas, who are exposed continuously to the stress of high altitude, requires our attention. We have become familiar with symptoms characteristic of chronic high-altitude disease: under special conditions this popu lation has a tendency to develop pulmonary hypertension, which is associated with pulmonary edema, pulmonary congestion, and right heart failure.
