Book excerpt: The Social Security Administration (SSA) administers two programs that provide disability benefits: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. SSDI provides disability benefits to people (under the full retirement age) who are no longer able to work because of a disabling medical condition. SSI provides income assistance for disabled, blind, and aged people who have limited income and resources regardless of their prior participation in the labor force. Both programs share a common disability determination process administered by SSA and state agencies as well as a common definition of disability for adults: "the inability to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months." Disabled workers might receive either SSDI benefits or SSI payments, or both, depending on their recent work history and current income and assets. Disabled workers might also receive benefits from other public programs such as workers' compensation, which insures against work-related illness or injuries occurring on the job, but those other programs have their own definitions and eligibility criteria. Book excerpt: Computational spine modeling has been employed as a research tool for more than 50 years to better understand the spine and the causes and development of various low back disorders. However, most spine models feature significant limitations that inhibit their utility for research. Clinically focused studies often featured highly detailed finite element models of the spine with great computational complexity that were used to evaluate individual tissue deformations, stresses, and even the impact of surgical instrumentation. Unfortunately, their great complexity limits the types of analyses performed, often requires significant simplifications, and limits the number of models created. Industrial biomechanics studies typically use much lower fidelity musculoskeletal models that evaluate whole body motions and deformations and typically include muscles and other soft tissues. Their simpler nature enables larger numbers of subjects to be evaluated. However, they do not include the same level of spinal detail as finite element models and are limited in the types of possible analyses. A few newer models combine elements of both types, but many still feature significant simplifications, specifically the geometric representation of the vertebrae. Additionally, no models are able to effectively evaluate large numbers of individual subjects to understand the variation of responses in the population. This study sought to address these voids in the use of computational modeling for the study of the biomechanics of the lumbar spine. Given the variability of spine geometry in the population and its ties to various spine disorders, biomechanical models require detailed representation of the structures of the spine and must include sufficient numbers of subjects to account for variability in the population. To that end, an extensive database of detailed digital spine models was developed. CT imaging was used to create three-dimensional geometric models of sixty subjects using a combination of algorithms and manual refinement specifically tuned to minimize partial volume effect errors and capture accurate information from both the anterior and posterior portions of each vertebral body. Geometric measures describing the database showed that there was substantial variability between subjects and as a function of vertebral level, subject age, and gender. A computational study was conducted to evaluate the impact of this subject-specific spine geometry on spinal loads. Biomechanical analysis demonstrated that the variability in spine geometry had a considerable impact on facet joint contact forces and corresponding intervertebral disc endplate moments, forces, and loading sharing between tissues. For most measures, there was a large range of values present in the subject population. Furthermore, there were significant differences in loading as a function of vertebral level, subject age, and between the left and right sides of the vertebrae. The highly complex loading patterns exhibited in each subject demonstrate the importance of including subject-specific geometry and representing numerous subjects in spine modeling in order to accurately understand spinal loads and their variation within the population.

Book excerpt: Low back pain and spinal disorders represent a major clinical apprehension as the population ages. With more than 1.4 million annual spine procedures, operative management is exerting a significant healthcare burden in the United States. Despite the new technologies and the advent of minimally invasive surgeries (MIS), the optimal surgical treatment for many spine pathologies is still controversial. Hence, revision surgeries, due to failure in attaining the surgical goals, have been very common. Comprehensive understanding of each patient condition is crucial in determining the best surgical treatment; however, the available tools used in diagnosis and specifying the treatment are still insufficient to provide such knowledge. Finite element modeling (FEM), an advanced computational method for structural stress analysis, was employed in orthopedic biomechanics applications since 1972 to evaluate the kinematics and kinetics of human tissues. With the advancement of the computational power and the ability to precisely reconstruct 3D models of the spine tissues, FEM is now a well-established tool for basic research in spine biomechanics. However, despite the exceptional capabilities of this method, it is yet not well exploited in patient diagnosis and in optimizing the surgical treatment. In this dissertation, a new theoretical approach that utilizes FEMs of the thoracolumbar spine to evaluate and compare different spine procedures is developed. In this approach, CT scans of a real human subject were reconstructed to build 3D anatomical models that are used in the FEM. Potential spine procedures were virtually performed on the FEMs and normal physiological loading conditions were applied on each surgical alternative. A novel steady state nonlinear biphasic analysis was employed to solve the model, which couples the fluid problem with a solid mechanics problem. Accordingly, the implications of each spine procedures on the biomechanics of the spine were evaluated and the optimal spine procedure, from a biomechanical perspective, was specified. Moreover, the possibility of the development of adjacent segment diseases due the surgical intervention was investigated. Spine procedures used in the treatment of lumbar spinal stenosis (LSS); a degenerative disease that accounts for 5% of patients who present with persistent low back pain, and thoracolumbar burst fractures (TBF); the most common site of spinal injury were particularly assessed. Several decompressive and spinal fusion surgeries through open or minimal invasive techniques, that utilized different sets of implantable devices, were examined. The results provided insights on the consequences of applying each surgical alternative and would definitely help practitioners to optimize the operative management for each patient.

Fusion surgeries restrict the motion at the implanted level there by imposing additional load at the adjacent levels. Many clinical studies have showed that adjacent segment degeneration was observed in patients over time. In order to overcome problems with fusion devices, dynamic stabilization systems are being used to treat disc degeneration related problems. These implants restore intersegmental motion across the implanted level with minimal effects on the adjacent levels. In vitro cadaveric testing was conducted on seven harvested sheep spines using established protocols. Axient was implanted in the spines 3 months prior to sacrificing. Main aim of this testing is to see if the performance is altered by the presence of surrounding muscle tissue. The specimens were prepared and tested under load control protocol. All six loading modes were tested by applying a pure moment of 10Nm (in steps of 2.5Nm) and angular displacement was calculated for the following cases: 1) Intact spine + Axient with surrounding muscle tissue, 2) Intact spine + Axient with muscle tissue removed, 3) Intact spine (with implant removed). Relative motion of L4 vertebra with respect to L5 was calculated. Statistical analysis was performed (on the implanted level data) to see if there is a statistical significance between cases 1 and 2. Biomechanical testing was also performed on 4 human cadavers to observe the trend with Axient compared to FE results. A validated 3-D non linear finite element model of the L3-S1 lumbar spine was used to evaluate biomechanics of various dynamic stabilization systems in comparison with traditional rigid rod system. The model was modified at L4-L5 level to simulate three different dynamic stabilization systems (DSFM-1, DSFM-2 and Axient, Innovative Spinal Technologies Inc., Mansfield, MA). Grade I was simulated at L4-L5 level. Follower preload of 400N and a 10Nm bending moment was applied to simulate physiological flexion, extension, lateral bending and axial rotation. Range of motion (ROM), intra discal pressure (IDP) and facet loads were calculated for all the models. Implant with better performance was then compared with fusion system in both grade I and grade II degenerated spines. In vitro results showed that there is no significant difference in the performance of the Axient with and without surrounding muscle tissue in terms of range of motion. Coming to FE results, Axient performed better over the other two implants (DSFM-1 and DSFM-2). Axient device was able to restore the motion at the implanted level compared to fusion device. Higher motions were observed at the adjacent level (L5-S1) with fusion device compared to intact and injured models. Both devices were able to stabilize the diseased spine and unload the treated disc.
