Book excerpt: Little information was available regarding how women make decisions about whether or not to use hormone replacement therapy. The purpose of this study was to understand factors that influenced a woman's decision about hormone replacement therapy. A grounded theory design was used to elicit women's responses to identify those factors. A purposive sample was obtained from ten women through the use of network sampling. Sampling continued until saturation was reached with ten interviews. The target population was women 40 years of age and older, who were perimenopausal or menopausal, and had some form of symptomatology. Information was elicited through interviews lasting approximately 15-30 minutes and were tape recorded for analysis. Demographic data on the subjects was collected for simple description. Data was analyzed for themes using the constant comparative technique that is unique to grounded theory. "Quality of life" was identified as the core variable. Concepts related to quality of life were (a) symptomatology, (b) risk factors, (c) education, (d) safety issues, and (e) ethical issues. The results showed strong implications that women considered their immediate "quality of life" to be of greater concern than long term health. Recommendations include careful attention to what women are saying and continuing education on menopause.

Book excerpt: The purpose of this study was to determine the factors that influence women's decisions to use hormone replacement therapy (HRT) during menopause. Using the Theory of Planned Behavior and constructs of risk perception, this study was able to explore the beliefs and attitudes of women about HRT use during menopause. The study was also able to explore how risk perception of HRT and the conditions that affect women during midlife impact their decision to use HRT during that time. Focus groups were conducted to develop the questionnaire used in the larger study. A community-based sample of women from Houston area churches participated in the study. The major theme that emerged from the focus groups was the weighing of cancer risks and the protection benefits of HRT. The most interesting factor that emerged as a barrier to HRT use was "negative publicity" and myths toward taking HRT. Results from the larger study demonstrated that the construct of attitude was the predominate predictor of intention when direct measures were used in a model to predict intention. Meaning that women's attitudes towards HRT use during menopause (whether they are safe, wise to use, good or bad, beneficial, risky, pleasant, or valuable) played a significant role in their intention. In contrast when the belief-based measures were used in the model, subjective norm and perceived behavioral control were significant predictors of intention. Meaning that the intention to use HRT was based more on the influence of their husbands, physicians, and families. This also meant that the dosage form, cost, negative publicity, family history of cancer, personal fear of developing cancer, and education about HRT would affect their intention to use HRT during menopause more than the advantages and disadvantages of using HRT (advantages such as protection from osteoporosis, relief from hot flashes; or, disadvantages such as risk of breast cancer). Finally, women's perception of risk with regards to HRT was highest for breast cancer followed by heart disease, endometrial cancer, and osteoporosis. This study found that there are many factors that may affect the decision to use HRT during menopause and that overall these factors affect women's attitudes towards HRT and their intention to use it.

A better understanding of the natural history of the menopause is critical to providing better care. If women and their physicians have a better understanding of predictors of risk, they could make more informed decisions about interventions related to menopausal symptoms, CVD, osteoporosis and gynecologic and breast cancer. Few other recently introduced medical interventions have as great a potential of affecting morbidity and mortality as does hormone replacement therapy (HRT). HRT has produced effect on health risk: some are reduced, some are raised, and some uncertain, and these data are interpreted differently by various scientific, medical and consumer groups.

Book excerpt: Long term preventive hormone replacement therapy (LTP-HRT) is attracting considerable attention for its potential in reducing the incidence of coronary heart disease and osteoporosis associated fractures in post-menopausal women. The study objectives were: (1) to describe women's decisions and decisional conflict regarding LTP-HRT after being presented with the evidence of benefits and risks and clarifying values; (2) to describe the difference between those who accept or decline LTP-HRT in terms of: (a) expectations of LTP-HRT benefits and risks, and (b) the importance women attach to the benefits and the risks; and (3) to describe the factors women identify as contributing to their decision to take or not take LTP-HRT or being unsure about taking LTP-HRT. The typical woman who participated in the study was 57 years of age; had some post secondary education; and was currently using HRT. Over half the women had at least one CHD risk factor and had a hysterectomy. Less than 15% of the respondents reported having osteoporosis or a first degree relative with breast cancer. Women's decisions to take LTP-HRT were distributed as follows: 56% yes; 26% no; and 18% unsure. Women who accepted or declined LTP-HRT did not have statistically significant differences in expectations of risk of CHD, osteoporosis, and breast cancer with and without hormone therapy. In contrast, women's importance ratings of LTP-HRT benefits and side effects did differ significantly between those accepting and declining LTP-HRT. The reasons most often identified by women who chose LTP-HRT were prevention of CHD and osteoporosis (61%) and relief of hot flashes (54%). Those who chose not to take LTP-HRT gave reasons such as dislike of taking pills (47%), and risk of cancer (11%). (Abstract shortened by UMI.).

In addition to medical issues, psychological, social, ethical and legal issues pertinent to transgender individuals add to the complexities of successful treatment of these patients. A final chapter includes extensive additional resources for both transgender patients and providers. Thus, an endocrinologist providing care to a transgender person will be able to use this single resource to address most of the patient’s needs.
